Postburn Contractures of the Elbow and Heterotopic Ossification.

Postburn contractures of the elbow are uncommon debilitating sequelae of severe burn injuries, which result from thermal injury to both deep and superficial tissues. When periarticular heterotopic bone forms in association with burn injuries, severe and rigid contractures may develop that prohibit basic functions of daily living and are often refractory to nonoperative intervention. Surgical intervention is aimed at releasing or excising all pathologic anatomy limiting elbow motion. In patients with proper indications, surgical intervention can result in substantial improvement in elbow motion…

Arthroscopic Excision of Osteoid Osteoma of the Elbow.

Osteoid osteoma has been reported infrequently around the elbow joint, and is usually treated with radiofrequency ablation or open excision. Elbow arthroscopy is useful for excision of accessible lesions, and the accompanying elbow stiffness can be treated concurrently. This report describes an arthroscopic excision of an osteoid osteoma located in the juxta-articular distal humeral bone. An initial adhesiolysis and capsulectomy is performed to gain access to the region above the capitellar articular margin. A 70° arthroscope is necessary to visualize this region via the anteromedial portal. A…

The Assessment and Management of Simple Elbow Dislocations.

BACKGROUND: Simple elbow dislocations are a commonly seen joint dislocation involving a sequential disruption of the soft tissue stabilisers without a significant associated fracture. METHODS: A selective literature search was performed and personal surgical experiences are reported. RESULTS: The majority of these injuries can be treated with expedient closed reduction, with the intact bony congruency of the elbow joint conferring early stability. Early mobilisation after reduction results in a faster recovery with good functional outcomes. Surgical intervention for persistent instability or…

Intra-articular capacity of the elbow joint.

The intra-articular capacity of the elbow joint is reported to be 23 ± 4 ml on cadaveric elbows. During years, this value was the standard. The aim of this observational study was to reanalyze the volume of the elbow joint on live patients. Measurement of the intra-articular capacity and pressure of the elbow joint was performed on 30 patients (mean age: 43.8 years) undergoing elbow arthroscopy. Intra-articular capacity was recorded when the elbow moved to the maximum lose packed position and/or when there was a sudden drop in pressure, indicating a capsular rupture (maximum capacity). Indicat…

"Liquid elbows" due to afatinib administration.

Non-small cell lung cancer adenocarcinoma in the past decade has targeted therapies as the cornerstone for therapy. In specific patients with epidermal growth factor receptor mutation have three different therapy approaches with the tyrosine kinase inhibitors: erlotinib, gefitinib and afatinib. Nowadays we can use tyrosine kinase inhibitors as second line treatment for squamous cell carcinoma. We present a case with a patient with squamous cell carcinoma receiving afatinib tyrosine kinase inhibitor who presented elbow bursitis or olecranon bursitis in both elbows.

Biomechanical considerations in the pathogenesis of osteoarthritis of the elbow.

Osteoarthritis is the most common joint disease and a major cause of disability. Distinct biological processes are considered crucial for the development of osteoarthritis and are assumed to act in concert with additional risk factors to induce expression of the disease. In the classical weightbearing joints, one such risk factor is an unfavourable biomechanical environment about the joint. While the elbow has long been considered a non-weightbearing joint, it is now assumed that the tissues of the upper extremity may be stressed to similar levels as those of the lower limb, and that forces ac…

Absence of radiographic abnormalities in nursemaid's elbows.

BACKGROUND: Disagreement exists between physicians on the usefulness of a prereduction radiograph for diagnosis and treatment of nursemaid's elbows in children. Some evidence suggests that nursemaid's elbows have identifying features on radiographs. This study compares the radiographs of nursemaid's elbows to normal, control elbows in children and hypothesizes that differentiating features do not exist on radiograph. METHODS: For this retrospective case-control study, hospital billing records were searched to identify all patients under age 6 treated with closed reduction for a nursemaid's el…

Chronically unreduced posterior dislocation of the elbow.

BACKGROUND: Chronically unreduced posterior dislocation of the elbow is a complex and difficult problem for the surgeon and its treatment significantly influences future functional performance of the patient. There are few reports in the literature regarding chronically unreduced dislocations, and most of them are based on observations of individual patients. MATERIAL AND METHODS: A total of 9 patients with chronically unreduced posterior dislocations of the elbow were treated in the Department of Orthopaedics and Traumatology between 2004 and 2012. The mean time between injury and reduction …

Imaging of sports ligamentous injuries of the elbow.

Ligamentous injuries of the elbow are common in sports. The diagnosis is usually based on a combination of the assessment of the mechanism of injury during the sporting activity and physical examination. Imaging with radiographs and MR imaging has an important diagnostic role, particularly when the clinical presentation is unclear and to evaluate for other important potential associated injuries such as myotendinous injuries, fractures, dislocations, and osteochondral lesions. Treatment includes conservative and surgical measures depending on the severity of the instability and associated inju…

Extraskeletal osteochondroma on a cat´s elbow.

A solitary extraskeletal osteochondroma was diagnosed in a 6-year-old, castrated male Burmese cat, positive for feline leukaemia virus (FeLV). The cat presented with a rapidly growing, solid, non-painful mass on the craniolateral aspect of the left elbow. Radiographs revealed an oval, well circumscribed 2.0 cm × 1.5 cm × 1.5 cm mineralised mass separated from the underlying bone. Surgical excisional biopsy confirmed the diagnosis. Feline extraskeletal osteochondromas are benign tumours frequently seen in FeLV-positive cats which can transform into osteosarcomas or chondrosarcomas. Radiographic…

The biceps muscle from shoulder to elbow.

The biceps brachii plays an integral role in movement of the shoulder and elbow, and pathology can occur in athletes of all ages. Injuries of the proximal biceps tendon can be seen in overhead athletes as a result of chronic impingement, tendon instability, or tensile overload, often with accompanying lesions of the labrum or rotator cuff. Presentation may be insidious or can be precipitated by an acute event. Injuries to the distal biceps are more common in athletes involved in strength training such as weightlifters, or occasionally in contact sports. Although injury to the proximal and dist…

New mechanism of the posterior elbow dislocation.

PURPOSE: To suggest a new model on the most common kind of posterior elbow dislocation using MRI findings on acute elbow injuries. METHODS: Fifteen patients with simple elbow dislocation (Group A) and 19 patients sustaining pure ligament injuries (Group B) were enrolled in this study. Using MRI scans, bony contusion at radial head and posterior capitellum (lateral bone contusion) and medial aspect of the ulnohumeral joint (medial bone contusion) were investigated with the injury patterns of the collateral ligament and overlying muscles. In Group A, the injury patterns of the ulnar and lateral…

Sonography of benign palpable masses of the elbow.

The purpose of this pictorial essay is to show the sonographic appearances of benign masses found in and around the elbow, including fat-containing tumors, fibrous tumors, vascular lesions, selected skin lesions, and other miscellaneous lesions. Sonographic features, including the use of dynamic and color Doppler imaging, can be helpful in characterizing these masses and in narrowing the differential diagnosis.

Early mobilisation for elbow fractures in adults.

BACKGROUND: A fall on the outstretched arm can result in an elbow fracture. Loss of elbow function is a common problem with these fractures and can have major implications for functional capabilities. It is unknown whether early mobilisation can improve functional outcome without increasing complications. OBJECTIVES: To compare the effects (benefits and harms) of early mobilisation versus delayed mobilisation of the elbow after elbow fractures in adults. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (August 2010), the Cochrane Central Regi…
