OBJECTIVES: Many patients with dementia develop agitation or aggression in the course of their disease. In some severe cases, behavioral, environmental, and pharmacological interventions are not sufficient to alleviate these potentially life-threatening symptoms. It has been suggested that in those cases, electroconvulsive therapy (ECT) could be an option. This review summarizes the scientific literature on ECT for agitation and aggression in dementia.
METHODS: We performed a systematic review in accordance with PRISMA guidelines. A search was conducted in Ovid MEDLINE, EMBASE, and PsycINFO. …

We report a patient who experienced atypical symptoms in the course of electroconvulsive therapy (ECT). During ECT treatment patient experienced psychotic symptoms which should be differentiated with prolonged delirium and nonconvulsive status epilepticus. 46-year-old female was referred to hospital with a diagnosis of major depressive disorder with no psychotic features in the course of recurrent depression. Despite several changes of pharmacological treatment no improvement was achieved, therefore it was decided to initiate ECT. Physical and neurological examination revealed no deviations fr…

OBJECTIVE: The aim of the study was to describe the successful treatment of delirium with electroconvulsive therapy (ECT).
METHODS: The method of the study was a case report.
RESULTS: A 75-year-old man, with a recently diagnosed carcinoma of the parotid gland, was admitted with a fluctuating psychiatric syndrome. Delirium was diagnosed, although an acute underlying somatic cause could not be readily established. Antipsychotics and benzodiazepines were not effective. After 7 sessions of ECT, all symptoms ceased. This enabled him to receive radiotherapy for his tumor and enjoy a good quality o…

OBJECTIVES: Little is known regarding the safety of electroconvulsive therapy (ECT) in the presence of cranial metallic objects (cMO) such as medical devices or metallic foreign bodies. The presence of cMO raises 3 theoretical concerns toward the safety of ECT: (1) cMO may significantly alter the ECT-induced electric field distribution in the brain regarding field strength and focality, (2) vascular complications at the location of the cMO due to the ECT-induced hyperdynamic state may occur, and (3) possible development of a prolonged seizure/status epilepticus during ECT as a consequence of a…

Electroconvulsive therapy (ECT) in the presence of metal in the skull may cause concern among clinicians. The literature is sparse, with only a few case reports in this area. We present here a case where ECT was administered in a patient with metallic internal fixation for the fracture of mandible. The 37-year-old man presented with severe depression, suicide risk, and alcohol dependence. Administration of ECT was uneventful, with no complications during ECT or at follow-up; and there was successful resolution of symptoms. This case report demonstrates that ECT may be safe in the presence of m…

OBJECTIVE: To determine the relative efficacy of electroconvulsive therapy (ECT) in the treatment of bipolar (BP) and unipolar (UP) depressive illness and clarify its role in BP depression.
METHOD: Patients referred for ECT with both UP and BP depressions. [classified by Structured Clinical Interview for DSM (SCID-I) criteria for history of mania] were included in a multi-site collaborative, double-masked, randomized controlled trial of three electrode placements - right unilateral, bifrontal or bitemporal - in a permutated block randomization scheme.
RESULTS: Of 220 patients, 170 patients (…

Electro-convulsive (ECT) therapy is frequently the treatment of choice in pregnant patients. It has been reported that administration of ECT in pregnancy is safe and effective. However, there are few controlled studies of the effects of ECT on pregnancy. The literature is not conclusive regarding ECT-related complications in pregnancy. We describe here the case of a primigravida with bipolar disorder who underwent nine sessions of ECT during pregnancy. The patient experienced vaginal bleeding after each session of ECT. Thiopental and succinylcholine were administered as an anesthetic (4 mg/kg

The study of psychiatric disorders co-occurring with rare genetic syndromes has important therapeutic and pathophysiological implications. Electroconvulsive therapy (ECT) in schizophrenia with comorbid Marfan syndrome is challenging in view of multiorgan involvement. We report successful use of ECT in a patient who presented to us with catatonia and Marfan syndrome. The report highlights the importance of careful evaluation of patients with schizophrenia and Marfan syndrome before ECT in liaison with other specialties.

BACKGROUND: Methods used to evaluate subjective effects of electroconvulsive therapy (ECT) have relied on self-report about discrete aspects of memory. Although objective deficits are demonstrable, patients generally report improved memory after ECT. Patients have not been asked to evaluate the global impact of ECT on mood or memory. This study was undertaken to compare patients' evaluations of ECT outcomes using direct questioning about global impact compared with standard methods.
METHODS: A prospective, naturalistic study was conducted in seven hospitals. Patients completed clinical and ne…

We carried out a systematic study of bibliographical review of scientific evidence provided by clinical trials that assessed the short, medium and long-term efficacy of electroconvulsive therapy (ECT) from 1965 until June 2003. The studies with the following features have been excluded: a) those in which ECT is not the aim of the research; b) those that do not compare ECT with another different treatment; c) those in which the aim of the research is not to evaluate the efficacy of ECT, and d) those in which the studies are not randomized clinical trials. We have used the biomedical databases M…

OBJECTIVE: Missed or abortive seizures during electroconvulsive therapy (ECT) may preclude completion of an effective course of treatment in some cases. Seizure augmentation, using proconvulsant agents, has been used to overcome resistance to the induction and continuation of seizure activity. In this review, we analyze published clinical data on the effects and safety of seizure augmentation techniques.
METHOD: Clinical studies and case reports were obtained through a MEDLINE literature search from 1966 to 2001, cross-referencing ECT and proconvulsant agents. Article references were also sca…

Resting state, eyes closed, 19-lead EEG recordings were obtained at pre-ECT baseline and just prior to penultimate treatment and during the week following the ECT course in 59 patients with major depression. Patients had been randomized to ECT conditions that varied in electrode placement and stimulus intensity. The EEG data were submitted to power spectral analysis, and global and topographic effects were characterized for the delta and theta frequency bands. Relations between the EEG changes and scores on three cognitive measures were examined. The period of disorientation immediately follow

The place of electroconvulsive therapy (ECT) in the treatment of affective disorders is presently important and even inequaled in some depressive illness forms. But ECT is still controversial and its indications in the treatment of schizophrenia remain debated. In order to evaluate the place of this "vielle thérapeutique du futur" (39) in the treatment of schizophrenia, we reviewed the literature of the two last decades and tried to answer these questions. 1) Is ECT effectiveness in treatment of schizophrenic symptoms proved? 2) What is the effectiveness of ECT compared with other efficacious …

The efficacy of ECT was investigated in a double-blind trial. Twenty-two patients with schizophrenia received trifluoperazine and were randomly allocated to receive eight real or eight simulated ECTs. In the first eight weeks, the group receiving real ECTs showed significantly more improvement as measured on the Brief Psychiatric Rating Scale. However, the groups showed no significant differences from the twelfth week onwards. The superiority of real ECT was not confirmed at the end of six months.

Patients diagnosed as reactively or endogenously depressed were given electroconvulsive therapy (ECT). The endogenously depressed patients tended to respond to the treatment more quickly and favourably. Measures of age and severity of depression were as predictive as the classification into endogenous or reactive depression using the Newcastle rating scale. The degree of psychological stress ascertained and presumed to be operating did not contribute to the prediction of outcome.
