Nitric oxide as a modulator of intestinal water and electrolyte transport.

The role of nitric oxide in intestinal fluid and electrolyte secretion depends upon whether the conditions under study are physiological or pathophysiological. In physiological conditions, endogenous nitric oxide seems to be a proabsorptive molecule, based on the findings that nitric oxide synthase inhibitors reverse net fluid absorption to net secretion in mice, rats, guinea pigs, rabbits, and dogs. This proabsorptive mode involves the enteric nervous system, the suppression of prostaglandin formation, and the opening of basolateral K+ channels. However, in some pathophysiological states nitr

Regulation of body fluid volume and electrolyte concentrations in spaceflight.

Despite a number of difficulties in performing experiments during weightlessness, a great deal of information has been obtained concerning the effects of spaceflight on the regulation of body fluid and electrolytes. Many paradoxes and questions remain, however. Although body mass, extracellular fluid volume, and plasma volume are reduced during spaceflight and remain so at landing, the changes in total body water are comparatively small. Serum or plasma sodium and osmolality have generally been unchanged or reduced during the spaceflight, and fluid intake is substantially reduced, especially d…

Fetal gastrointestinal composition: implications for water and electrolyte absorption.

In the late-gestation human or ovine fetus, daily swallowed volumes (100-300 ml/kg body weight) are markedly greater than adult values (40-60 ml/kg). This volume of amniotic fluid water, electrolytes and cellular debris is almost completely resorbed within the fetal gastrointestinal (GI) tract, resulting in the formation of meconium within the distal fetal colon. Despite this capacity for fetal GI water resorption, little is known regarding the sites or processes by which absorption occurs. We examined the composition of GI contents of 13 ovine fetuses (121-148 days). Fetuses were operatively …

Developmental aspects of fluid and electrolyte secretion in salivary glands.

The salivary glands of rodents undergo considerable cytodifferentiation after birth and are useful models for the study of functional development, including the mechanisms of fluid and electrolyte secretion. In the rat submandibular gland, secretion of salivary fluid cannot be elicited until approximately 2 weeks of age. The currently accepted model of salivary fluid secretion indicates that this process depends on the activation, on stimulation of cholinergic receptors, of several ion transport systems, resulting in a net transport of osmotically active ions (primarily Cl- and Na+) across the

Electrolyte-induced demyelination in rats. 2. Ultrastructural evolution.

This study presents the electron microscopic evolution of lesions in electrolyte-induced demyelination (EID) in rats, a lesion which bears striking histological and clinical similarity to central pontine myelinolysis. The earliest change was observed during the hyponatremic phase and consisted of minimal intracellular edema present throughout the brain. Following the injection of hypertonic saline, additional changes were observed which were restricted to sites previously reported to be frequently involved in EID. Early dilatation of the inner tongue of oligodendrocyte cytoplasm in myelinated

Severe electrolyte disturbances in lethal subclavian catheter dialysis.

Syncope, irreversible shock, and respiratory and circulatory arrest developed in a 54-year-old diabetic man chronically dialyzed with a subclavian catheter (SC) minutes after the end of a dialysis session. Resuscitation attempts remained unsuccessful. During the resuscitation attempt, a blood analysis showed severe hyponatremia, acidosis, and hypochloremia. Respiratory and cardiac arrest developed during dialysis in a 64-year-old woman on chronic SC dialysis. Resuscitation was unsuccessful; chloremia levels were 79 mEq/L, and calcemia levels were 20 mg%. Both patients were dialyzed with a stan

Acid-base and electrolyte abnormalities in febrile patients with bacteraemia.

OBJECTIVES: Very commonly febrile patients with bacteraemia develop a variety of acid-base and electrolyte disturbances which play a significant role in the morbidity and mortality of these patients. This study was undertaken to describe the pathogenetic mechanisms of these abnormalities in febrile patients with bacteraemia. METHODS: Fifteen febrile patients with bacteraemia, aged 24-62 years, were studied. In all patients blood cultures revealed Gram-negative rods. None of them had septic shock, diabetes mellitus, renal or liver failure and none was receiving drugs influencing acid-base bala

[3 dangerous electrolyte disturbances: hyponatremia, hyperkalemia and hypomagnesemia].

Three cases of acute and potentially dangerous electrolyte disturbances provide a basis for discussion of selected aspects of pathogenesis, clinical manifestation and correction of certain electrolyte disorders (hyponatremia, hyperkalemia, hypomagnesemia). A case of diuretic-induced hyponatremia illustrates the controversy surrounding correction of this disorder. The regulatory mechanisms of transcellular potassium equilibrium are discussed with the help of a case of exercise-induced hyperkalemia. A separate case is dedicated to the discussion of the multiple causes and symptoms of magnesium d

Electrolyte balance in heart failure and the role for magnesium ions.

It is well established that clinically significant changes in a number of electrolytes occur in patients with congestive heart failure (CHF). Magnesium ions are an essential requirement for many enzyme systems, and evidence is rapidly emerging that magnesium deficiency is a major risk factor for survival of CHF patients. In animal experiments, magnesium has been shown to be involved in several steps of the atherosclerotic process and, although in humans the situation is somewhat more complex, magnesium ions play an extremely important role in CHF and various cardiac arrhythmias. A number of dr…

The ability of physicians to predict electrolyte deficiency from the ECG.

The ECG is considered to be a fairly accurate reflection of a patient's electrolyte status. Two full-time academic emergency physicians, each board certified in two specialties, interpreted the ECGs of 97 consecutive alcoholic patients presenting to the emergency department. Serum potassium, calcium, and magnesium values were obtained on all patients within one hour of ECG performance. The physicians attempted to predict abnormalities of serum electrolytes from the ECG tracing alone when blinded to all laboratory values, the clinical situation, and each other's readings. The combined results o…

Cystic fibrosis: an inborn error of cellular electrolyte transport?

Abnormal epithelial transport in cystic fibrosis (CF) appears to provide a unifying hypothesis to explain the varying clinical manifestations of CF. The major abnormality is the cell regulation of epithelial Cl- secretion; however, a number of other abnormalities of electrolyte transport has been observed. A description of the normal physiology of secretion and the current state of our knowledge of the abnormalities of epithelial secretion is discussed.

Disturbance of fluid and electrolyte transport in cystic fibrosis epithelia.

Studies of the disease CF suggest that the basic defect is related to impaired electrolyte movement in the epithelia of a variety of organs with exocrine function. The disturbances of electrolyte secretion in the organs classically involved in CF range from 1) a decrease in secretion- or uptake of chloride ions in all the organs studied; 2) an increase in sodium uptake in nasal airway epithelium and 3) a decrease in bicarbonate output of the pancreas. In this review an overview is presented of the expression of the CF defect, the abnormalities of fluid and electrolyte secretion in each CF affe

Hormonal status and fluid electrolyte metabolism in motion sickness.

In the first experimental series, 10 healthy male test subjects with a high susceptibility to motion sickness showed a significant increase of ACTH, cortisol, STH, prolactin, ADH, aldosterone concentrations, and plasma renin activity after vestibular tests. The 10 subjects with a moderate susceptibility exhibited a still higher increase of the hormones, except plasma renin. The 8 test subjects with a low susceptibility displayed a considerable increase in ACTH, cortisol, and STH after vestibular stimulation. In the second experimental series, the increase of STH, cortisol, ADH, aldosterone and

Effect of corticosteroids on electrolyte transport in the descending colon of the rat.

The effect of corticosteroids on electrolyte movement was studied in an everted sac model from distal rat colon. Corticosterone had a significant stimulatory effect on sodium and fluid transfer in a dose-related manner. At the low dose of 10(-9) M corticosterone increased sodium absorption by 29.4 microEq X g-1 X h-1, a value that was statistically greater than the effect of 10(-9) M dexamethasone. At the higher concentration of 10(-7) M the effect of corticosterone on sodium movement was twice that of aldosterone or dexamethasone in equimolar concentrations. By contrast, the effect of cortico
