Predominant cortical dysfunction in Guadeloupean parkinsonism.

Atypical parkinsonism is extremely frequent in Guadeloupe and may have an environmental cause. One-half of the patients with this tauopathy have dopa-resistant parkinsonism, tremor, subcortical dementia and abnormal eye movements suggestive of progressive supranuclear palsy (PSP). They also have hallucinations, dysautonomia, which are not characteristic of PSP. Furthermore, the oculomotor abnormalities and the tremor, which is jerky, differ from what is observed in classical PSP patients. We therefore undertook an electrophysiological study to characterize these features in greater detail. Nin

Discharge patterns of human genioglossus motor units during sleep onset.

STUDY OBJECTIVES: Multiunit electromyogram recordings of genioglossus have demonstrated an abrupt reduction in the muscle's activity at sleep onset. Recent evidence from single motor unit recordings indicates that the human genioglossus muscle consists of motor units with a variety of discharge patterns. The aim of the present study was to determine the effect of sleep onset on the activity of individual motor units as a function of their particular discharge pattern. DESIGN: Genioglossus activity was assessed using intramuscular fine-wire electrodes via a percutaneous approach. Sleep onsets

Acute low single dose of apomorphine reduces periodic limb movements but has no significant effect on sleep arousals: a preliminary report.

STUDY OBJECTIVES: To investigate the effects of apomorphine on the frequency of periodic limb movements during sleep (PLMS) and on sleep architecture. DESIGN: Nine patients presenting PLMS (including eight patients with restless legs syndrome) underwent three consecutive night sleep recordings. They received a single dose of 0.5 mg subcutaneous apomorphine at bedtime the third night. RESULTS: When computing PLMS during four 2-h periods of sleep, a significant period by apomorphine-effect was demonstrated, with a marked reduction of PLMS during the first 4 h post-injection (P < 0.01). No sign

[Vestibular diagnostics in childhood - methods and assessment].

BACKGROUND: Children with vertigo are thought to be difficult to examine. Vestibular test batteries are used less frequently for children than in adult patients. METHODS: Within the context of a retrospective study 136 patients in age between 0 and 15 years, who were examined by Frenzel's glasses, ENG and VOG between 1996 and 1999 were evaluated. Caloric test and frequency selective rotation chair tests were performed. RESULTS: Workability of VOG examination (91 %) was higher than Frenzel glasses (86 %) and ENG (78 %). Videooculography is the method of choice in childhood. Comparing diagnosi

Oculomotor abnormalities in boys with tourette syndrome with and without ADHD.

OBJECTIVE: To assess saccadic eye movements in boys with Tourette syndrome (TS) with and without attention-deficit hyperactivity disorder (ADHD), comparing performance with that of an age-matched group of male controls. METHOD: Three different saccade tasks (prosaccades, antisaccades, and memory-guided saccades) were used to examine functions necessary for the planning and execution of eye movements, including motor response preparation, response inhibition, and working memory. The study included 14 boys with TS without ADHD (TS-only), 11 boys with TS and ADHD (TS+ADHD), and 10 male controls.

Sleep patterns and blood pressure variability in patients with pure autonomic failure.

Sleep patterns and 24-h blood pressure variability were studied in four female patients (age range: 56-82 years) with pure autonomic failure. All patients had severe symptomatic postural hypotension, without neurological deficits. In these patients the following patterns were observed: (i) a reversed diurnal blood pressure pattern, with the highest values observed at sleep onset; (ii) a prolonged sleep latency and increased amount of stage 3 sleep; (iii) difficulty with getting up after awakening in the morning, due to severe postural hypotension; (iv) an absence of prominent respiratory abnor

Impairment of vertical motion detection and downgaze palsy due to rostral midbrain infarction.

We present two cases with acute onset of vertical gaze palsy, mainly consisting of impaired downgaze and apraxia of downward head movements, together with neuropsychological deficits (hypersomnia, impaired attention and disorders of memory and affective control). CT and MRI revealed bilateral post-ischaemic lesions in the dorsomedial thalamus and the mesodiencephalic junction, dorsomedial to the red nucleus, thus being restricted to the territory of the posterior thalamosubthalamic paramedian artery, which includes the region of the rostral interstitial nucleus of the medial longitudinal fasci

Pattern dystrophy of the retinal pigment epithelium.

The authors describe a family in which one member had pattern dystrophy of the retinal pigment epithelium. The proband also had neovascular membranes which resulted in decreased visual acuity. Two other family members had abnormal electro-oculographic findings, but a clinical normal-appearing fundus that could represent an early stage of the disease. The incidence of this condition in this family indicates autosomal dominance as the inheritance pattern of this dystrophy.

Coexistent Meige's syndrome and myasthenia gravis. A relationship between blinking and extraocular muscle fatigue?

We studied five patients with a combination of Meige's syndrome (blepharospasm-oromandibular dystonia) and myasthenia gravis. The coexistence of two disorders impairing eyelid opening led to diagnostic confusion and delayed appropriate therapy. Detailed oculographic monitoring of one patient indicated that eye position drifting due to myasthenic oculomotor fatigue was corrected by eye blinks, and that blinks tended to occur with slower saccades. Our observations suggest that fatigue of extraocular muscles may lead to synkinetic blinking and perhaps eventually to autonomous blepharospasm.

Suppression of the hyperosmolarity response after cataract surgery.

The hyperosmolarity response was recorded with the electrooculogram in the dark before and after cataract extraction, to investigate a possible disorder in the retinal pigment epithelium after cataract extraction. The hyperosmolarity response was suppressed one week after surgery, especially after intracapsular extraction. The response recovered up to the preoperative level in most cases one or two months after surgery. These findings indicate reversible functional disorders of the pigment epithelium after cataract extraction, particularly intracapsular. This dysfunction can be revealed by the

Uvulopalatopharyngoplasty: a comparison of two techniques.

Two groups of patients undergoing uvulopalatopharyngoplasty were studied. Group I patients underwent en bloc resection of tonsils and palate with the line of excision less than 1.5 cm anterior to the uvula-soft palate junction. Group II patients underwent tonsillectomy followed by palatectomy with a margin 1.5 to 2.0 cm anterior to the uvula-soft palate junction. Only 1 of 7 patients in Group I was objectively improved, whereas 11 of 13 patients in Group II improved. We conclude that a generous resection of soft palate and tonsillar tissue is mandatory for success of uvulopalatopharyngoplasty

Disturbances of the sleep-waking cycle in patients with vascular brain stem lesions.

Continuous polygraphic recordings of six patients with vascular brain stem lesions were evaluated to investigate the relationship between 1) the localization or extent of the lesions and the alteration of the sleep-waking cycle, and 2) the evolution of the clinical condition and the sleep-waking cycle. Absent REM sleep and severely disorganized NREM sleep were associated with extensive pontine lesions involving the pontine tegmentum. In "locked-in" syndrome, the neural mechanisms responsible for both REM and NREM sleep continued to function, but the sleep-waking cycle was disturbed and marked

Identification of the location of vestibular lesions on the basis of vestibulo-ocular reflex measurements.

Quantitative analysis of electro-oculographic recordings of eye movement in response to precise visual and vestibular stimuli makes possible the differentiation of three categories of vestibular syndromes due to pathological changes in three different parts of the visual vestibulo-ocular reflex arc: (1) decreased vestibulo-ocular reflex gain (e.g., decrease in slow component velocity), but normal fast components and visual-vestibular interaction (labyrinth and eighth nerve); (2) normal slow component velocity but abnormal fast components to all stimuli (pontine or medullar reticular formation)

Ophthalmological aspects of chronic CS2 intoxication.

As a pilot study preliminary to a large scale survey on chronic CS2 toxicity 30 workers of a viscose rayon industry in Belgium have been examined. Twenty of them were working in the xanthation spinning department where chronic CS2 intoxication is more likely to occur and ten controls were chosen form the supposedly safe bleaching division. Ophthalmological evaluation included visual fields, coulour vision testing, fluoro-angiography, ERG and EOG. In both groups fundus anomalies, abnormal EOG's and subnormal or supranormal ERG's were found. The fundus anomalies consisted either of discrete pigm

Application of linear system analysis to the horizontal vestibulo-ocular reflex of the alert rhesus monkey using pseudorandom binary sequence and single frequency sinusoidal stimulation.

Horizontal eye movements of the alert rhesus monkey resulting from both pseudorandom binary sequence (PRBS) and single frequency sinusoidal rotational stimulation were analyzed using a PDP 11/40 computer in order to generate gain, phase, and coherence estimates at discrete frequencies between 0.008 and 1.28 Hz. A computer simulation of vestibular induced eye movements was used to validate our analysis procedures and to determine the effects of digital noise. Frequency domain transfer functions derived from gain and phase estimates revealed that the responses to PRBS stimulation and to single f

Dominantly inherited cystoid macular edema.

An apparently autosomal-dominant macular dystrophy occurred in three pedigrees with the presenting signs of typical cystoid macular edema due to leaking perimacular capillaries. Other striking features were retinal capillary leakage all over the posterior pole of the eye, whitish punctate deposits in the vitreous body, a normal electroretinogram, a subnormal electro-oculogram, and moderate to high hyperopia. In later or more advanced stages the macula developed a central zone of "beaten bronze" atrophy. Strabismus occurred frequently.
