Abdominal gynaecological emergencies in the surgical unit.

We have studied two types of emergency cases that occurred in the surgical unit who were found to have gynaecological aetiology: 1) patients with the so-called "morbus acutum dexter", i.e. appendicitis, or a gynaecological disease and 2) late complications in patients who had earlier benign gynaecological disease, usually such as tumour and surgery for it. The material with seven years follow-up consisted of 97 patients, who were treated during the periods 1959--60 and 1969--70. The relative numbers in both groups increased during the latter period. Of the patients in the reproductive age with

Problems in general anaesthesia. Emergencies and trauma.

Emergencies resulting in death of the patient are sufficient reason for insisting on only medically qualified people giving anaesthetics. Unlike in most other specialities, complications in anaesthesia usually will not await the arrival of the expert. Trauma provides the anaesthetist with some of his most testing occasions. Most problems are related, as with emergencies, to basic derangements of respiratory and circulatory physiology However, there is a steady progression with severe trauma cases that is not a common feature of the ordinary anaesthetic disaster. Respiratory problems are either…

Neuro-oncological emergencies requiring critical care intervention.

Patients with primary or metastatic brain tumours can deteriorate abruptly, and the acute physician is often the first to respond. This review covers the recognition and initial management of the neurological emergencies most likely to need critical care: raised intracranial pressure from peritumoural oedema and herniation; seizures and status epilepticus, including non-convulsive status epilepticus (NCSE); intracranial haemorrhage; acute hydrocephalus; and venous thromboembolism. It also covers two treatment-related emergencies that the general physician must now recognise: immune effector ce…

Cannabis-Associated Emergencies in the Emergency Department.

BACKGROUND: Now that the Cannabis Act legalizing the use of cannabis has come into effect in Germany, the frequency of cannabis use in Germa … LUSION: The legalization of cannabis use can be expected to lead to an increase in cannabis-related emergencies in Germany as well. As the clinical manifestations are often nonspecific, the targeted investigation of possible cannabis use is indicated so that the affected patients can be given appropriate treatment and assistance.

Injuries and medical emergencies among international travellers.

BACKGROUND: Tropical infectious diseases and vaccine-preventable emergencies are the mainstay of pre-travel consultations. However, non-communicable diseases, injuries and accidents that occur during travel are not emphasized enough in these settings. METHODS: We performed a narrative review based on a literature search of PubMed, Google Scholar, UpToDate, DynaMed and LiSSa and on reference textbooks and medical journals dedicated to travel, emergency and wilderness medicine. Relevant secondary references were extracted. We also aimed to discuss newer or neglected issues, such as medical tour…

[Surgical emergencies during the COVID-19 pandemic].

BACKGROUND: Within a few months coronavirus disease 2019 (COVID-19) spread around the world. Some studies showed a significant reduction in medical consultations. Due to new discussions about restrictions in the case of rising COVID-19 case numbers, this study investigated the consequences of the COVID-19 pandemic on surgical emergencies at the Rostock University Medical Center and compared the results to the years before. MATERIAL AND METHODS: The aim of the study was to compare the case numbers of the surgical emergency department of the Rostock University Medical Center from the period 202…

Heat, Cold, and Environmental Emergencies in Athletes.

With the increase in outdoor events, there is an inevitable rise in climate-related environmental emergencies. Heat exposure can place athletes at risk for life-threatening heatstroke which requires emergent diagnosis and rapid in-field management. Cold exposure can lead to hypothermia, frostbite, and other nonfreezing injuries that require prompt evaluation and management to minimize morbidity and mortality. Altitude exposure can lead to acute mountain sickness or other serious neurologic or pulmonary emergencies. Finally, harsh climate exposure can be life-threatening and require appropriate…

General Surgery During Pregnancy and Gynecologic Emergencies.

Nonobstetrical surgical emergencies can occur throughout pregnancy but are often difficult to diagnose due to the physiologic and anatomical changes that occur during pregnancy. Medical providers should have insight into these changes and be familiar with options for the diagnosis and management of common nonobstetrical surgical emergencies, such as appendicitis, cholecystitis, and small bowel obstruction. Surgeons should also be aware of obstetrical emergencies, such as ectopic pregnancy and severe vaginal bleeding, which may be life threatening to mother and the fetus. Intraoperatively, surg…

Oncologic emergencies and urgencies: A comprehensive review.

Patients with advanced cancer generate 4 million visits annually to emergency departments (EDs) and other dedicated, high-acuity oncology urgent care centers. Because of both the increasing complexity of systemic treatments overall and the higher rates of active therapy in the geriatric population, many patients experiencing acute decompensations are frail and acutely ill. This article comprehensively reviews the spectrum of oncologic emergencies and urgencies typically encountered in acute care settings. Presentation, underlying etiology, and up-to-date clinical pathways are discussed. Criter…
