Calcium gradients in inner ear endolymph.

Recent studies suggest that endolymphatic hydrops resulting from the ablation of the endolymphatic duct and sac in guinea pigs may be caused by a disturbance of endolymph calcium homeostasis. A similar disturbance of calcium homeostasis could represent the underlying cause of Ménière's disease. In this study, we mapped the calcium concentrations and electrical potentials throughout the endolymphatic system in normal guinea pigs. Large concentration differences exist between different compartments, including a more than twofold increase along the length of the cochlea. The electrochemical poten…

[Clinical electrocochleography in Menière's disease].

Transtympanic electrocochleography (ECochG) was performed on 137 definite Menière ears, 29 normal ears and 383 ears with other diseases. The average SP/AP amplitude ratio (SP/AP) was significantly larger than that of normal subjects in the following four disease groups whose causes are thought to be endolymphatic hydrops: 1) definite Menière's disease, 2) suspected Menière's disease, 3) delayed endolymphatic hydrops and 4) labyrinthine syphilis. The SP amplitude could not provide any information for a differential diagnosis of endolymphatic hydrops from normal subjects nor from other sensorine…

Tympanic electrocochleography: normal and abnormal patterns of response.

Electrocochleography has been widely used in human and animal studies of endolymphatic hydrops. A variety of response patterns have been reported in normal and hydropic ears. Recent clinical studies have focused almost exclusively on the amplitude ratio of the summating potential (SP) and action potential (AP) derived from alternating polarity click responses. In this report normal response patterns are described with a tympanic membrane electrode employing condensation, rarefaction and alternating polarity clicks and tone burst stimulation. A variety of response abnormalities are described in…

[Effect of furosemide on the carbonic anhydrase activity in vestibule].

In order to explore the mechanism that the furosemide dehydrates and improves the vestibular function in the endolymphatic hydrops. The effects of furosemide on the carbonic anhydrase activity in vestibule were studied by using histocytochemistry and image analysis. The results demonstrated that the furosemide can obviously inhibit the carbonic anhydrase activity. There was no significant difference between the normal ear and the ear with endolymphatic hydrops in the location of the carbonic anhydrase in the vestibule. It suggested that one of the dehydrant mechanisms is inhibition of carbonic…

Frequency analysis of evoked otoacoustic emissions in Meniére's disease.

Our study was designed to establish a diagnostic significance in Meniére's disease by means of evoked otoacoustic emissions (EOAEs). EOAEs w … re, lower frequency dominant emissions might be elicited by modification of the basilar membrane in endolymphatic hydrops and that the main frequencies might change from lower to higher with reduction of endolymphatic hydrops induced by osmotic diuretics. It was considered that the EOAE testing might be useful for a diagnosis of endolymphatic hydrops in Meniére.

[Hydropic inner ear disease of the vestibular type].

We report the case of a patient with episodic rotational vertigo for years. Ear symptoms were negated. The clinical diagnosis was not clear - Menière's disease, vestibular migraine and recurrent vertigo after vestibular neuritis all qualified for differential diagnoses. A locally enhanced inner-ear MRI established clarity by showing an endolymphatic hydrops in the vestibulum. Besides the classical triad of Menière's disease there are other clinical appearances of endolymphatic hydrops, which can be visualized with inner-ear MRI.

MRI in otology: applications in cholesteatoma and Ménière's disease.

Imaging of middle-ear cholesteatoma with diffusion-weighted imaging (DWI) magnetic resonance imaging (MRI), and inner-ear endolymphatic hydrops (in Ménière's disease) with post-gadolinium high-resolution MRI, are reviewed. DWI MRI provides for a more specific diagnosis of tympano-mastoid cholesteatoma. There is an established and increasing role of DWI MRI in detecting both primary and postoperative cholesteatoma, localising disease, and planning surgery. The contemporary diagnostic accuracy of DWI is reviewed, pitfalls in interpretation are described, and potential future developments are hig…

The "hype" of hydrops in classifying vestibular disorders: a narrative review.

BACKGROUND: Classifying and diagnosing peripheral vestibular disorders based on their symptoms is challenging due to possible symptom overlap or atypical clinical presentation. To improve the diagnostic trajectory, gadolinium-based contrast-enhanced magnetic resonance imaging of the inner ear is nowadays frequently used for the in vivo confirmation of endolymphatic hydrops in humans. However, hydrops is visualized in both healthy subjects and patients with vestibular disorders, which might make the clinical value of hydrops detection on MRI questionable. OBJECTIVE: To investigate the diagnost…

Endolymphatic subarachnoid shunt failure caused by Silastic allergy.

During the period from 1964 through 1994, the endolymphatic subarachnoid shunt operation was initially successful in eliminating endolymphatic hydrops and the symptoms and findings it produces in 76% of 645 ears of patients with Meniere's disease. After initial success, lasting from five weeks to nine years, endolymphatic hydrops suddenly returned due to obstruction of the Silastic shunt tube in 11% of patients. In these cases, prompt revision can often restore an initial good result. Histologic and immunologic examination of the material surrounding and occluding the tubes showed an allergic …

Electrocochleographic changes in the hearing loss associated with X-linked hypophosphataemic osteomalacia.

Transtympanic electrocochleography was carried out on 13 patients (21 ears) suffering from X-linked hypophosphataemic osteomalacia, and known to have cochlear hearing loss. The majority of ears exhibited a cochlear pattern of cochleogram with a recruiting input/output function and a markedly biphasic action potential. In addition 14 ears yielded a greatly enhanced negative summating potential, suggesting the presence of endolymphatic hydrops. It is proposed that the hearing loss so frequently associated with this condition is in part due to endolymphatic hydrops.

Experimental alteration of physiological state of inner ear fluids.

Mechanisms of endolymphatic hydrops formation in the inner ear are not yet fully understood. The available knowledge of the composition and the function of inner ear fluids, of mechanisms for maintenance of microhomeostasis, and the possible factors responsible for hydrops formation were reviewed. The nature of the functional barrier systems of the inner ear was investigated by measuring the amount of intravenously or intracisternally injected test substances which entered the perilymph. Based on studies of the osmotic relationship between the blood and the perilymph, an hypothesis of hydrops …

Effect of inner ear blood flow changes in Ménière's model mice.

CONCLUSIONS: The endolymphatic sac (ES) is important for inner ear fluid homeostasis. A dysfunctional ES can cause vertigo attacks following … ally by light microscopy. RESULTS: The injection of epinephrine into the model animals reduced the endolymphatic hydrops in the cochlea, but also produced mild hydrops in the vestibule, which was never observed in untreated (control) animals. The ES did not react to epinephrine in the normal way. Injection of SNP did not cause any changes.

Electron microscopic studies of the endolymphatic sac in Ménière's disease.

Five specimens of the endolymphatic sac taken from patients with Ménière's disease during epidural drainage surgery were examined under the electron microscope. The clinical history and histopathological findings are presented. The most common and prominent finding was perisaccular fibrosis of the endolymphatic sac which appears to disturb the free absorption of endolymph in the endolymph in the endolymphatic sac resulting in endolymphatic hydrops. Well-preserved epithelial cells of the endolymphatic sac could only be found in one of the specimens.

Magnetic resonance imaging of the temporal bone in patients with Ménière's disease.

Ménière's disease (MD) is still controversial in several aspects. The vestibular aqueduct, the osseous channel that carries the endolymphatic duct and sac, has previously been studied by tomography and computed tomography, with shortening and narrowing of this structure observed. These findings are apparently correlated to the development of the endolymphatic hydrops present in MD and related to its episodic symptoms. In studying the endolymphatic duct, the key structure in the pathology of this disease, magnetic resonance imaging (MRI) studies of the temporal bone were performed in 12 patient…
