The homeostasis of water in the inner ear is essential for maintaining function of hearing and equilibrium. Since the discovery of aquaporin water channels, it has become clear that these channels play a crucial role in inner ear fluid homeostasis. Indeed, proteins or mRNAs of AQP1, AQP2, AQP3, AQP4, AQP5, AQP6, AQP7 and AQP9 are expressed in the inner ear. Delayed endolymphatic hydrops (DEH) differs from Ménière's disease in that it occurs in pre-existing ear pathology in patients who have a profound unilateral or total deafness that was caused by infection, trauma, or unknown causes during childhood or adulthood. The present study reports a successful anial model of endolymphatic hydrops (EH) in guinea pigs (n = 48) through a surgical obliteration of the endolymphatic sac and duct. von Hippel-Lindau disease is a rare, inherited multicystic disorder that is characterized by several benign and malignant neoplasms (Odrzywolski, 2010). Classically, the disease manifests itself in a broad spectrum, including renal cell carcinomas, intracranial and spinal hemangioblastomas, endolymphatic sac tumors, renal and pancreatic cysts, and pheochromocytomas. Another important, but commonly forgotten manifestation is the cystadenoma of the rete testis.

Hemangiomas of the temporal bone are benign, vascular and are tumors. The diagnosis is in most cases made during the operation because the symptoms are non conclusive. Two cases were recently observed: one simulating a Meniere disease while the tumor was developed close to the jugular bulb and the endolymphatic sac; the second invaded the geniculate area and the petrous apex and was revealed by a progressive facial palsy.

Peripheral vestibular vertigo requires surgical management in 10 to 20 % of patients, who present persistent symptoms despite conservative treatment. The main indications include Ménière's disease, superior semicircular canal dehiscence, benign paroxysmal positional vertigo, and perilymphatic fistulas. Non-ablative interventions aim to control vertigo while preserving inner ear function. They include endolymphatic sac surgery, semicircular canal occlusion, and perilymphatic fistula repair.

Meniére's disease, neither spontaneous nor inducible in animals, is studied only in patients. Natural history (epidemiology) shows a triad of major symptoms: vestibular, auditory, and aural pressure. The experience gained by the authors in the treatment of Menière's disease and similar affections which are supposed to have 3 stages provided the following approach to their treatment: after examination of the cochleovestibular analyzer involvement good results may be expected of conservative treatment in stage I-II, chordoplexectomy is justified in stage I, drainage or decompression of the endolymphatic sac and vestibulotomy are indicated in stage II and III, respectively.

The original technique of the operation of cochleostomy is described. It should be considered as conservative operation, since it produces an "ostomy" of the cochlear promontory bone, joining the spiral ligament and mucosa of middle ear, without opening of the Cochlea. There is no risk of iatrogenic anacusis. The follow up of 178 cases with more than 10 years after surgery is described. The results are significantly better than endolymphatic sac surgery.
