Enhanced cardiac sympathetic afferent reflex involved in sympathetic overactivity in renovascular hypertensive rats.

Sympathetic outflow is increased in hypertension. The aim of the present study was to investigate whether the cardiac sympathetic afferent reflex (CSAR) is enhanced in two-kidney one-clip (2K1C) renovascular hypertensive rats, and whether the enhanced CSAR contributes, in part, to the increased sympathetic outflow. Furthermore, the role of central angiotensin II type 1 (AT(1)) receptors in mediating the CSAR was determined. Under urethane and alpha-chloralose anaesthesia, the renal sympathetic nerve activity (RSNA) and mean arterial pressure (MAP) were recorded in sinoaortic denervated and cer

Unmasking Coronary Microvascular Dysfunction in Hypertrophic Cardiomyopathy: Multimodality Imaging With Stress Cardiac MRI and Coronary CT Angiography.

Coronary microvascular dysfunction (CMD) is a hallmark feature in hypertrophic cardiomyopathy (HCM), linked to significant clinical challenges including myocardial ischemia, fibrosis, and an elevated risk of heart failure and sudden cardiac death. In this case, a 21-year-old female presented with intermittent lower extremity edema, exertional dyspnea, and persistent fatigue. Imaging studies were pivotal in identifying the underlying CMD and confirming the HCM diagnosis. Transthoracic echocardiography revealed mild left ventricular hypertrophy with hyperdynamic function, reduced global longitud

Impact of the inflammation on the outcomes of catheter ablation of drug-refractory ventricular tachycardia in cardiac sarcoidosis.

INTRODUCTION: Catheter ablation (CA) of ventricular tachycardia (VT) in cardiac sarcoidosis (CS) has been reported with varying success. However, there is a scarcity of data on the outcomes of CA based on ongoing inflammation. OBJECTIVE: We hypothesized that the response to VT ablation depends upon the stage of the disease. METHODS: Between July 2004 and December 2018, 24 patients of CS presented with drug-refractory VT at CARE Hospital (Hyderabad) and the University of Minnesota (Minneapolis, MN). Patients were classified into two groups based on cardiac magnetic resonance imaging and posit

Spatial characterization of electrogram morphology from transmural recordings in the intact normal heart.

PURPOSE: Unipolar (UE) and bipolar electrograms (BE) are utilized to identify arrhythmogenic substrate. We quantified the effect of increasing distance from the source of propagation on local electrogram amplitude; and determined if transmural electrophysiological gradients exist with respect to propagation and stimulation depth. METHODS: Mapping was performed on 5 sheep. Deployment of >50 quadripolar transmural needles in the LV were located in Cartesian space using Ensite. Contact electrograms from all needles were recorded during multisite bipolar pacing from epicardial then endocardial el

Pulsed Field Ablation for Perimitral Atrial Flutter With Involvement of the Vein of Marshall: A Case Report.

BACKGROUND: Perimitral atrial flutter (PMF) involving the vein of Marshall (VOM) remains a therapeutic challenge. The role of pulsed field ablation (PFA) in the treatment of PMF is unclear. CASE PRESENTATION: A 65-year-old male was admitted with a 7-year history of palpitations. He had undergone three previous radiofrequency ablation procedures for atrial fibrillation and atrial flutter (AFL). He was readmitted due to recurrent AFL, with an electrocardiogram indicating atypical AFL. A three-dimensional electrophysiological activation map of the left atrium revealed a macro-reentrant circuit c

Sub-Zero: A Review of the Current Evidence and Evolving Technologies Enabling Zero‑Fluoroscopy Procedures.

Fluoroscopy has long been the cornerstone imaging modality in cardiac electrophysiology (EP), enabling real-time catheter visualization but exposing patients and healthcare providers to ionizing radiation and significant occupational hazards. With the emergence of three-dimensional electroanatomic mapping (EAM) systems and intracardiac echocardiography (ICE), zero- and near-zero fluoroscopy (ZF) approaches have become increasingly feasible across a wide spectrum of EP and structural interventions. This review synthesizes current evidence supporting ZF workflows, highlighting their efficacy, sa

Multipolar radiofrequency ablation: Early experience with a novel approach to target intramural ventricular arrhythmias.

BACKGROUND: Conventional radiofrequency ablation (RFA) may fail to treat ventricular arrhythmias (VAs) if the target is located deep within the myocardium. We have previously demonstrated successful treatment of intramural VAs using multipolar ablation (MPA). OBJECTIVE: To evaluate the efficacy of MPA as a means to treat refractory VAs. Accordingly, we herein report a series of patients with refractory VAs treated with MPA. METHODS: This observational retrospective case series included 31 consecutive patients who underwent 36 MPA procedures for premature ventricular contractions (n = 20) and

[Anatomy for ablation of ventricular extrasystoles and tachycardia in the right ventricle].

BACKGROUND: Catheter ablation of ventricular tachycardias (VTs) is one of the most complex tasks in interventional electrophysiology. It is complicated by the fact that VT can recur during treatment which can affect the hemodynamic stability of the patient. In addition, navigation with the ablation or mapping catheter through the valvular apparatus and the trabecularization of the ventricle can be challenging. MATERIALS AND METHODS: In most cases, a three-dimensional mapping system is used to facilitate orientation and the search for the site where the tachycardia originates. Access to the ri

Freehand allograft aortic valve replacement and aortic root replacement. Utility of intraoperative echocardiography and Doppler color flow mapping.

UNLABELLED: Seventeen consecutive patients undergoing 20 planned aortic valve replacements with allograft valves at Stanford University Medical Center were studied with intraoperative epicardial echocardiography and Doppler color flow mapping before and after cardiopulmonary bypass. Native aortic valves were replaced in 12 of the 20 patients, and eight patients underwent second aortic valve procedures. In 17 of 20 patients allograft selection was guided by prebypass echocardiographic estimates of annular diameter and/or length of allograft aortic root required. Other prebypass findings include

Atrial fibrillation ablation in patients with pulmonary lobectomy or pneumectomy: Procedural challenges and efficacy.

BACKGROUND: Catheter ablation of atrial fibrillation (AF) in patients with pulmonary lobectomy or pneumectomy is challenging due to anatomical alterations. After lung resection, electrically active pulmonary vein (PV) stumps remain and need to be localized for PV isolation (PVI). The present study aims to describe clinical challenges of PVI in patients with pulmonary lobectomy or pneumectomy. METHODS: We performed a retrospective study on 19 patients with previous pulmonary lobectomy or pneumectomy undergoing catheter ablation for AF in three German hospitals. RESULTS: Nineteen patients with

Left atrial voltage mapping using a new impedance-based algorithm in patients with paroxysmal atrial fibrillation.

AIMS: Atrial fibrosis is associated with the pathogenesis and progression of atrial fibrillation (AF). We sought to evaluate the extent of left atrial (LA) scarring in patients with paroxysmal AF (PAF) undergoing catheter ablation using a new impedance-based algorithm. METHODS: We prospectively enrolled 73 consecutive patients (43 males, 58 years) with PAF who underwent pulmonary vein antral isolation. We first performed high-density bipolar voltage mapping during sinus rhythm using Tissue Proximity Indicator (TPI), one of the features of the ConfiDense mapping module integrated in the electr

A Novel Workflow for Electrophysiology Studies in Patients with Brugada Syndrome.

Brugada Syndrome (BrS) is a primary electrical epicardial disease characterized by ST-segment elevation followed by a negative T-wave in the right precordial leads on the surface electrocardiogram (ECG), also known as the 'type 1' ECG pattern. The risk stratification of asymptomatic individuals with spontaneous type 1 ECG pattern remains challenging. Clinical and electrocardiographic prognostic markers are known. As none of these predictors alone is highly reliable in terms of arrhythmic prognosis, several multi-factor risk scores have been proposed for this purpose. This article presents a ne

An approach to the three-dimensional display of left ventricular function and viability using MRI.

Cardiac MRI was performed in human volunteers to determine the magnitude of the misregistration (MSR) of cardiac landmarks due to variability in the diaphragm position for repeated breath-holds. Seven normal volunteers underwent MR imaging of the left ventricle (LV) to evaluate the magnitude of the endocardial centroid MSR. The MSR for a mid-ventricle short-axis image was 3.01 +/- 1.68 mm through-plane and 4.16 +/- 1.62 mm in-plane. A second order polynomial fit through the LV centroid coordinates minimized the in-plane component of the MSR error. Short-axis cine images, corrected for MSR, pro

Mitral valve prolapse and ventricular arrhythmias: observations in a patient with a 20-year history.

INTRODUCTION: Ventricular arrhythmias are a common feature in patients with mitral valve prolapse. In an attempt to determine the origin and underlying electrophysiologic mechanism, we describe a patient with ventricular fibrillation, exercise-induced ventricular tachycardia (VT), and, at the time of diagnosis, prolapse of the posterior mitral valve leaflet without mitral regurgitation. METHODS AND RESULTS: Treatment with beta-blockade and diphenylhydantoin prevented the occurrence of malignant ventricular arrhythmias for more than 17 years. Discontinuation of the therapy resulted in an immed

Arrhythmogenic Remodeling of the Left Ventricle in a Porcine Model of Repaired Tetralogy of Fallot.

BACKGROUND: Ventricular arrhythmias are frequent in patients with repaired tetralogy of Fallot (rTOF), but their origin and underlying mechanisms remain unclear. In this study, the involvement of left ventricular (LV) electrical and structural remodeling was assessed in an animal model mimicking rTOF sequelae. METHODS: Piglets underwent a tetralogy of Fallot repair-like surgery (n=6) or were sham operated (Sham, n=5). Twenty-three weeks post-surgery, cardiac function was assessed in vivo by magnetic resonance imaging. Electrophysiological properties were characterized by optical mapping. LV f

Characterizing the transient electrocardiographic signature of ischemic stress using Laplacian Eigenmaps for dimensionality reduction.

OBJECTIVE: Despite a long history of ECG-based monitoring of acute ischemia quantified by several widely used clinical markers, the diagnostic performance of these metrics is not yet satisfactory, motivating a data-driven approach to leverage underutilized information in the electrograms. This study introduces a novel metric for acute ischemia, created using a machine learning technique known as Laplacian eigenmaps (LE), and compares the diagnostic and temporal performance of the LE metric against traditional metrics. METHODS: The LE technique uses dimensionality reduction of simultaneously r

Augmented input from cardiac sympathetic afferents inhibits baroreflex in rats with heart failure.

It has been established that the baroreflex is markedly decreased in chronic heart failure (CHF). Our recent study has indicated that activation of the cardiac sympathetic afferent reflex (CSAR) inhibits the baroreflex in normal rats, and in the rats with CHF the CSAR is significantly enhanced, which is related to augmented central angiotensin II (Ang II) mechanism. Therefore, the hypothesis is that the augmented CSAR in the CHF state tonically inhibits the baroreflex via central AT1 receptor. To test the hypothesis, the rats with myocardial infarction-induced CHF or sham surgery were anesthet

Novel CineECG Derived From Standard 12-Lead ECG Enables Right Ventricle Outflow Tract Localization of Electrical Substrate in Patients With Brugada Syndrome.

BACKGROUND: In Brugada syndrome (BrS), diagnosed in presence of a spontaneous or ajmaline-induced type-1 pattern, ventricular arrhythmias originate from the right ventricle outflow tract (RVOT). We developed a novel CineECG method, obtained by inverse electrocardiogram (ECG) from standard 12-lead ECG, to localize the electrical activity pathway in patients with BrS. METHODS: The CineECG enabled the temporospatial localization of the ECG waveforms, deriving the mean temporospatial isochrone from standard 12-lead ECG. The study sample included (1) 15 patients with spontaneous type-1 Brugada pat
