Anorectal abscesses are a common presentation to the Emergency Department, but are frequently misdiagnosed. We report a patient in whom penile discharge was the presenting symptom of an ischiorectal abscess. A 42-year-old man presented with scrotal pain, swelling, and penile discharge. The genitourinary examination revealed a tender scrotum, and a fluctuant mass was identified on digital rectal examination. A 70-year-old woman was admitted for surgical treatment of a thymoma. She underwent thymectomy with left brachiocephalic vein resection because of invasion. Pleural involvement is a rare presentation of brucellosis. We report a patient referred for the evaluation of fever, right-sided pleural effusion and hilar lymph node enlargement. The pleural fluid revealed exudative characteristics with the predominance of neutrophils. Brucella melitensis was isolated from bone marrow, pleural effusion and blood cultures. The diagnosis was also confirmed by demonstration of a high Brucella agglutinin titre. The patient was successfully treated with rifampicin and doxicycline for three months.

The objective of this study was to estimate the interest and the limits of skin exudate microscopy for patients with chronic ulceration with clinically suspected Buruli's ulcer and living in an endemic area in Côte d'Ivoire. Two stained smears, one with Ziehl-Neelsen and the other Dugomier staining, were produced from 140 samples obtained after a swab of skin lesions. The positive smear rate for the acid fast bacilli (AFB) was respectively 16.4% and 12.9% for Dugomier and Ziehl-Neelsen staining. BACKGROUND: Choroidal hemangioma associated with bullous retinal detachment may be difficult to treat, due to varying results with conventional laser photocoagulation, radiotherapy, or surgical drainage. MRI is generally regarded as being sensitive but non-specific for the detection of insufficiency fractures affecting the sacrum and pelvic ring. The finding of intrafracture fluid is described in two elderly patients with insufficiency fractures. BACKGROUND: Preoperative lymphoscintigraphy (LS) with 99mTc antimony sulphide colloid is now part of the routine management of patients with intermediate thickness melanoma at the Sydney Melanoma Unit. Over a 13-year period, 1375 patients have been examined using LS, and we have observed many unusual lymphatic drainage pathways, including direct drainage through the body wall to retroperitoneal and paravertebral lymph nodes from the skin of the back. The aim of this study was to determine the incidence of such drainage in the 542 patients who had primary melanoma sites on the posterior trunk.

Exudate is often found in the root canal of teeth requiring endodontic therapy. The aim of this study was to evaluate the relationship of sequential changes of different host mediators in endodontic exudates to clinical and radiographic findings. Few anatomopathological criteria for distinguishing vocal nodules, polyps or REINKE's edema have been established in previously published studies. In order to study this issue, 163 samples (from 119 patients) were examined by an experienced anatomopathologist, without her prior knowledge of the macroscopic or microscopic diagnosis. BACKGROUND: Diabetic macular oedema is the leading cause of vision deterioration in diabetic retinopathy. Extracellular fluid within the retina, which distorts the retinal architecture, was assumed to be strictly of retinal vasculature origin. However, there is some experimental evidence supporting clinical observations suggesting a possible role of the retinal pigment epithelium (RPE). We report a case of intracystic carcinoma of the breast in which a correct preoperative diagnosis was achieved on the basis of an increased level of carcinoembryonic antigen (CEA) in the aspirated cystic fluid. Cyst fluids from 55 premenopausal women with gross cystic breast disease were classified by K+/Na+ ratio: 19 with ratio over 1 (type I) and 36 with ratio less than 1 (type II). Immunoradiometric assay of cathepsin D in both types of cyst fluids revealed the presence of large amounts of this proteinase. To investigate the pathological changes of hyperphonated vocal folds 15 cats were used and over-cry was induced by mechanical stimuli. The longest experiment time was 15 weeks. All of the larynges were taken and the vocal folds were examined grossly and microscopically. The results were as follows: 1) liquid exudate accumulation beneath the epithelium developed in all of the experimental animals; 2) thickening of the epithelium was noticed in cats which had hyperphonated for more than 4 weeks; 3) acute laryngitis occurred in three cats.
