Necrotizing fasciitis (NF), also known as "flesh-eating disease," is a rapid-acting, potentially deadly infection. This condition is devastating in its ability to spread through the body at an alarming rate. It can be caused by almost any type of injury that introduces microorganisms into the subcutaneous tissues. The initial assessment can be deceptive, so awareness is the key to recognition. This condition often goes unrecognized until it is too late or major disfigurement results. Vital nursing strategies for managing the care of patients with NF include early recognition, accurate assessme…

The necrotizing fasciitis is a life threatening disease, since its mortality reaches 30-35%, in different studies. In diabetic patients, the incidence and severity of this illness are even higher. The authors present a series of 39 diabetic patients with necrotizing fasciitis of the leg, which were operated between 2001 and 2006 in the Surgical Department of "Dr. I. Cantacuzino" Clinical Hospital, from Bucharest. It is emphasized the importance of a complete diagnosis and the surgical treatment of all the suppurative processes. Sometimes, more than one procedure is required, in order to achiev…

Necrotizing fasciitis is a life-threatening condition in diabetic patients; its management and salvage of the patient is a formidable challenge. Diabetes mellitus is one of the serious conditions associated with necrotizing fasciitis. It is a disorder that primarily affects the microvascular circulation. We review our experience and present our approach to necrotizing fasciitis in patients with diabetes mellitus. All cases of necrotizing fasciitis treated at Inonu University, Turgut Ozal Medical Center, from January 1997 to December 2003 were reviewed. A review of 59 charts identified 11 cases…

BACKGROUND: Necrotizing fasciitis (NF) is still a source of high morbidity and mortality. These difficult cases are increasingly referred to burn centers due to special wound and critical care issues. Here we examine our institution's recent experience with a large series of NF.
METHOD: We performed a retrospective chart review of 32 consecutive patients over a 10-year period with NF who required radical surgical debridement and surface reconstruction.
RESULTS: Overall survival was 85%, with average length of stay of 74.0+/-7.5 days for survivors (S) and 68.8+/-6.3 days for nonsurvivors (NS)…

The fibrosing disorders represent a diverse group of uncommon chronic diseases that include systemic sclerosis, eosinophilic fasciitis, eosinophilia-myalgia syndrome, toxic oil syndrome, and localized forms of fibrosis. A rare case of eosinophilic fasciitis is reported. The patient was a 61-year-old female who presented with generalized massive edema and eosinophilia. Signs of common edema-producing diseases, such as heart failure, were absent. Corticosteroids were initiated with slow improvement in edema. Over the ensuing months, miliary tuberculosis and tenosynovitis of her left hand and lef…

Fibrosarcoma is an uncommon, malignant soft-tissue tumor that is rarely found as a primary neoplasm in the foot. A case report is presented that demonstrates a large, locally invasive fibrosarcoma of the plantar aspect of the foot with initial symptoms consistent with plantar fasciitis. Below-the-knee amputation was performed as curative treatment.

OBJECTIVE: To describe the management of patients with necrotizing fasciitis of the eyelids.
DESIGN: Retrospective, noncomparative interventional case series.
PARTICIPANTS: Seven patients with necrotizing fasciitis limited to the eyelids.
METHODS: Retrospective review of the charts and photographs of seven patients with necrotizing fasciitis limited to the eyelids.
MAIN OUTCOME MEASURES: Eyelid function and appearance, mortality, and morbidity.
RESULTS: Seven of seven patients had good eyelid function and adequate appearance without reconstruction after healing. No deaths occurred.

The first case of the confirmed necrotizing fasciitis caused by Group A Streptococcus in Yugoslavia was presented. Male patient, aged 28, in good health, suddenly developed symptoms and signs of severe infective syndrome and intensive pain in the axillary region. Parenteral antibiotic, substitutional and supportive therapy was conducted along with the radical surgical excision of the necrotizing tissue. The patient did not develop streptococcal toxic shock syndrome thanks to the early established diagnosis and timely applied aggressive treatment. He was released from the hospital as completely…

Visceral involvement in eosinophilic fasciitis (EF) is a well known but rare event. With regard to neurological manifestations, both carpal tunnel syndrome and peripheral neuropathy have been described. We report the case of a 40-year-old woman with EF who had a major motor seizure. This association might not be fortuitous and be related to the known neurotoxin of eosinophils.

Necrotizing fasciitis (NF) is a severe often life threatening bacterial infection. There are 2 main reasons to use hyperbaric oxygen (HBO2): the polymorphism of the bacterial flora with a predominance of anaerobes, either strict or aerotolerant; and the tissular necrosis due to an extensive disseminated microvascular obstruction within the infected area. Association of HBO2 to antibiotics and surgery is based on strong pathophysiological findings as well as on evidences from animal studies. Clinical evidence in human is still lacking even if published data supports its use in severe cases. Con…

We report a case of eosinophilic fasciitis, with the unusual features of pulmonary and pleural involvement. Similar cases which involve the lungs have been reported after exposure to L-tryptophan, but there is no relevant drug history in this case.

A case of necrotizing fasciitis of the upper limb caused by group A streptococcal infection and a case with an abscess of the hand caused by staphylococcus aureus, are presented. The anatomo-pathological and clinical differences between the two different types of infection are underlined and stress is laid on the excruciating pain, the high temperature and the rapidly spreading inflammatory-necrotizing evolution of the first case. More than the anatomo-pathological findings of a necrotic area the authors consider more important, the rapidity and the severity of the infection, that can evolve i…

A case of fatal craniofacial necrotizing fasciitis is described in a 72-year-old diabetic woman and management is discussed. Progressive infection of the eyelids occurred with involvement of the right side of the face. Computed tomography revealed soft tissue swelling. Antibiotic treatment was started and debridement performed; histopathology showed acute inflammation and thrombosis of the epidermis and dermis. Despite treatment, scepticemia occurred, resulting in death less than 48 h after presentation. At this time extensive necrosis had developed in the superficial fascia with undermining a…

We report a 13-day-old female neonate with necrotizing fasciitis, who presented with fever and erythema on her back. Rapidly progressive extension of the erythema to the abdominal wall occurred within 12 hours. Sonography for the lesion showed extensive fascial thickening with fluid accumulation. An abscess was also demonstrated. Necrotizing fasciitis was highly suspected. Under sonographic guidance, pus was aspirated from the abscess. A smear for the aspirated pus revealed a large number of neutrophils and abundant gram-positive cocci. Immediate surgical debridement was done. Cultures from th…
