The Italian National Surveillance System for Occupational Injuries: Conceptual Framework and Fatal Outcomes, 2002-2016.

Background: A national database of work-related injuries has been established in Italy since 2002, collecting information on the injured person, his/her work tasks, the workplace and the risk factors contributing to incident dynamics, according to a model called Infor.Mo. Methods: A descriptive study of occupational fatal injuries, excluding work-related fatal traffic injuries, that occurred in Italy from 2002 to 2016 (15 years) was performed. Results: Among 4874 victims involved, all were males, mainly >51 years of age (43.2%), predominantly self-employed (27.8%) or workers with non-standard

Within-subject blood pressure level--not variability--predicts fatal and nonfatal outcomes in a general population.

To assess the prognostic significance of blood pressure (BP) variability, we followed health outcomes in a family-based random population sample representative of the general population (n=2944; mean age: 44.9 years; 50.7% women). At baseline, BP was measured 5 times consecutively at each of 2 home visits 2 to 4 weeks apart. We assessed within-subject overall (10 readings), within- and between-visit systolic BP variability from variability independent of the mean, the difference between maximum and minimum BP, and average real variability. Over a median follow-up of 12 years, 401 deaths occurr

Excipient Lung Disease and Fatal Outcome Among Young Adults: A Case Series on the Hidden Dangers of Intravenous Drug Abuse in India.

BACKGROUND: Excipient lung disease (ELD) arises from the intravenous (IV) injection of crushed oral tablets, leading to acute or chronic pulmonary complications. Despite its significance, ELD is under-recognized, particularly in countries like India, where over-the-counter availability of pharmaceutical drugs contributes to rising abuse rates. This study investigates sudden deaths among young adults due to ELD, emphasizing the need for increased awareness and preventive measures. MATERIALS AND METHODS: This case series examined 10 cases of sudden death in individuals aged 18-25 with a history

Genome wide association study of Escherichia coli bloodstream infection isolates identifies genetic determinants for the portal of entry but not fatal outcome.

Escherichia coli is an important cause of bloodstream infections (BSI), which is of concern given its high mortality and increasing worldwide prevalence. Finding bacterial genetic variants that might contribute to patient death is of interest to better understand infection progression and implement diagnostic methods that specifically look for those factors. E. coli samples isolated from patients with BSI are an ideal dataset to systematically search for those variants, as long as the influence of host factors such as comorbidities are taken into account. Here we performed a genome-wide associ

What Can We Do When All Collapses? Fatal Outcome of Collapsing Glomerulopathy and Systemic Lupus Erythematosus With Diffuse Alveolar Hemorrhage: Case Report.

INTRODUCTION: Collapsing glomerulopathy (CG) is a rare form of glomerular injury. Although commonly associated with human immunodeficiency virus (HIV) infection, it can occur in association with systemic lupus erythematosus (SLE). CASE REPORT: We present the case of a 50-year-old man, with chronic kidney disease secondary to focal and segmental glomerulosclerosis, who received a cadaveric kidney transplant in 2007. There were no relevant intercurrences until May 2015, when he presented with nephrotic range proteinuria (± 4 g/d). A graft biopsy was performed and it did not show any significant

A Rare Case Report on Dedifferentiated Liposarcoma of the Thigh Mimicking an Abscess With Vascular Features: A Diagnostic Challenge With Fatal Outcome.

Dedifferentiated liposarcoma (DDLPS) is an aggressive malignant soft tissue tumor comprising approximately 15%-20% of all liposarcomas. When presenting in the extremities, its atypical clinical and radiological features may mimic vascular or infectious conditions, posing a serious diagnostic challenge that can delay curative intervention. A 60-year-old diabetic female presented with a 6-month progressive left thigh mass accompanied by intermittent fever, significant weight loss, and refractory anemia. Doppler ultrasonography initially suggested a hemangioma; contrast-enhanced CT was interprete

Newly emerging type B insulin resistance (TBIR) during treatment with eculizumab for AQP4-IgG-positive neuromyelitis optica spectrum disorder (NMOSD): fatal outcome.

BACKGROUND: Aquaporin-4 immunoglobulin G (AQP4-IgG) antibody-positive neuromyelitis optica spectrum disorders (NMOSD) are frequently associated with other autoimmune disorders, including systemic lupus erythematosus (SLE). Eculizumab (ECU) is a highly effective long-term treatment for NMOSD. However, ECU is known to increase significantly the risk of infection with encapsulated bacteria and sepsis. Recently, increased insulin resistance (IR) in patients with NMOSD has been suggested. Type B IR (TBIR) is a rare autoimmune condition often accompanying or preceding SLE. TBIR has not yet been repo

CTH G1208T and MTHFR A1298C polymorphisms are associated with a higher risk of a first myocardial infarction with fatal outcome among women.

OBJECTIVES: Cystathionine-gamma-lyase (CSE) in the transsulfuration pathway generates hydrogen sulfide (H2S), suggested regulating cardiovascular function. The G1208T polymorphism in the CTH gene, rs1021737, has, in addition to MTHFR, been found to increase homocysteine, related to myocardial infarction (MI) risk. This study aimed, for the first time, to investigate the associations of the polymorphisms CTH G1208T, MTHFR C677T, and A1298C with the prospective risk of developing a fatal or non-fatal first MI. METHODS: This case-referent study included 545 cases later developing a first-ever MI

Case Report: Fatal Outcome for a Preterm Newborn With Meningitis Caused by Extended-Spectrum β-Lactamase-Producing Escherichia coli Sequence Type 1193.

INTRODUCTION: In this case report, we describe an extended-spectrum beta-lactamase (ESBL) - Escherichia coli (E. coli) strain of sequence type (ST) 1193, a novel, virulent, multidrug-resistant (MDR) clone with a rapid global spread. ST 1193 has been more commonly associated with invasive disease than other ESBL-E. coli STs. To our knowledge, this is the first known case in Sweden where a newborn died of an ESBL-E. coli ST 1193 meningitis. We emphasize that the clinical knowledge about the properties of certain MDR-clones should be increased. CASE REPORT: A moderately preterm boy was born afte

Immunity to a salivary protein of a sand fly vector protects against the fatal outcome of visceral leishmaniasis in a hamster model.

Visceral leishmaniasis (VL) is a fatal disease for humans, and no vaccine is currently available. Sand fly salivary proteins have been associated with protection against cutaneous leishmaniasis. To test whether vector salivary proteins can protect against VL, a hamster model was developed involving intradermal inoculation in the ears of 100,000 Leishmania infantum chagasi parasites together with Lutzomyia longipalpis saliva to mimic natural transmission by sand flies. Hamsters developed classical signs of VL rapidly, culminating in a fatal outcome 5-6 months postinfection. Saliva had no effect…

Fatal outcome in a patient with metastatic prostate cancer and acquired severe hypogammaglobulinemia with complete absence of mature peripheral blood B-cells.

Common variable immunodeficiency (CVID) is the second most common immunodeficiency and may be classified according to the presence or absence of mature B-cells, the latter subgroup being exceedingly rare. We describe a 66-year-old patient who over six months developed profound hypogammaglobulinemia associated with a complete absence of circulating B-lymphocytes and had a fatal outcome. Immunophenotypic analysis of a bone marrow aspirate demonstrated a strongly reduced number of B-cell precursors (CD34+/CD19+) but normal numbers of CD34+/CD45+ hematopoietic progenitor cells. This case is remark…

[Acute rhabdomyolysis, myoglobinuria and renal insufficiency caused by ethanol: review of the literature and description of a clinical case with fatal outcome].

Several studies demonstrated that alcohol can induce both chronic and acute myopathy with myoglobinuria. Acute rhabdomyolysis is commonly followed by a renal failure (R.F.) of variable degree. The Authors report a case of rhabdomyolysis induced acute R.F. in a chronic alcoholic young man. The patient died in irreversible shock. Early diagnosis of the disease seems to be important, because early treatment can prevent acute myoglobinuric R.F.

Disseminated Carcinomatosis of the Bone Marrow in Colorectal Cancer: A Case Series of Two Patients with Rapidly Fatal Outcomes.

INTRODUCTION: Disseminated carcinomatosis of the bone marrow (DCBM) is a rare but highly fatal complication in colorectal cancer (CRC) patients. Although previous reports have described favorable responses to urgent chemotherapy, we encountered 2 cases of CRC-associated DCBM with a devastating clinical course. Here, we present these cases and discuss current optimal management in light of the literature. CASE PRESENTATION: The first patient was a 69-year-old man with stage IIIC rectal cancer who underwent high anterior resection with lymph node dissection and partial ileal and bladder resecti
