Endoprosthesis is a method allowing a valuable substitution of bone defects (articular ends of long tubular bones, both articular ends of the bones making the articulation, whole long tubular bones with articulations). The employment of endoprostheses is expedient for the substitution of defects of the breast bone, pelvis, hand bones as well. The authors have treated 16 cases of tumours of the upper end of the femur by resection over at least 12 centimetres and insertion of a massive prosthesis. The tumour was secondary in 9 cases and primary in 7. This treatment was used for metastases where conservative surgery was impossible or had failed. In primary tumours, when the excision was very extensive, the main problem was of stability of the prosthesis and its long-term future. A new type of total massive prosthesis, designed by the authors after biomechanical studies, is described.

Four patients showed signs of femoral nerve compression with subsequent paresis of the quadriceps muscle, after radiation therapy of malignant tumours. The compression was caused by scar tissue due to radiation treatment of the inguinal region. The first symptom was radiating pain in the front of the thigh and lower leg which appeared 12-16 months after X-ray treatment. Two patients with metastatic angiosarcoma of the brain are described. In one, a 17-year-old man, the tumor was located at the pineal region and exhibited significant vascularity. The diagnostic value of CT examination in 21 children with 14 malignant and 7 benign bone neoplasias was evaluated. CT contributed in 72%; mainly in malignant conditions it was superior to other diagnostic techniques and demonstrated the exact intra- as well as the extraosseous extension. The cross sectional display facilitated evaluation of complex anatomic structures. Monitoring the treatment was easily performed by CT follow-up studies. However, 28% of all the examinations were not contributary, this group mainly including benign lesions.

The notes and radiographs of 19 patients who had pathological fractures of long bones due to secondary neoplastic deposits were reviewed. Involvement of over half of the cortex is recommended as an indication for prophylactic internal fixation to prevent the development of a pathological fracture. Eight patients were treated successfully in this way. A notable secondary effect of prophylactic internal fixation was the considerable reduction in pain at the site of the lesion.

Forty-two arteriovenous grafts were placed to provide vascular access in 40 patients with poor or sclerosed superficial veins.
