Bone mineral density in the chronic patellofemoral pain syndrome.

Bone mineral density (BMD) and clinical status of 40 patients with a chronic, unilateral patellofemoral pain syndrome (PFPS) were determinated. The mean duration of the disease at the time of the follow-up was 7.6 +/- 1.8 (SD) years. The BMD was measured at the spine (L2-L4), and the femoral neck, trochanter area of the femur, distal femur, patella, proximal tibia, and calcaneus of both lower extremities using a dual-energy X-ray absorptiometric (DXA) scanner. The mean BMD of the affected limb (compared with the unaffected side) was significantly lower in the distal femur (-3.3%; P = 0.002), p

New quantitative ultrasound techniques for bone analysis at the distal radius in hip fracture cases: differences between femoral neck and trochanteric fractures.

BACKGROUND: Ample evidence on etiological and pathological differences between femoral neck and trochanteric fracture cases suggests the possibility of individualized treatment. There are many issues related to areal bone mineral density and other quantitative computed tomography parameters of the proximal femur. Although osteoporosis is a systemic problem, little has been reported regarding differences in bone structural parameters, including bone mineral density, between them in regions other than the proximal femur. METHODS: Participants were consecutive female patients >50 years of age ad

Primary hyperparathyroidism presenting as atraumatic bilateral hip fractures: a case report.

BACKGROUND: Primary hyperparathyroidism (PHPT) affects up to 1.5% of older adults, causes bone loss at cortical sites and increases fracture risk. However, atraumatic hip fractures due to PHPT are rare. We report the case of an older female diagnosed with PHPT after presenting with atraumatic bilateral neck of femur fractures. CASE PRESENTATION: An 82-year-old white woman was referred to the emergency department with progressive left hip pain over two months and was unable to bear weight on presentation. There was no history of trauma, falls, prior fracture or diagnosis of osteoporosis. X-ray

Biomechanical study of a biplanar double support screw (BDSF) technique based on Pauwels angle in femoral neck fractures: finite element analysis.

OBJECTIVE: The objective of the present study is to conduct a comparative analysis of the biomechanical advantages and disadvantages associated with a biplanar double support screw (BDSF) internal fixation device. METHODS: Two distinct femoral neck fracture models, one with a 30° angle and the other with a 70° angle, were created using a verified and effective finite element model. Accordingly, a total of eight groups of finite element models were utilized, each implanted with different configurations of fixation devices, including distal screw 150° BDSF, distal screw 165° BDSF, 3 CLS arrange

Deyerle fixation for intracapsular fractures of the femoral neck.

The results of the Deyerle method of pin plate fixation for intracapsular fracture of the neck of the femur are less satisfactory than the results in the original literature. We treated 143 intrascapsular neck fractures of the femur by the Deyerle method during the period from 1969 to 1975. Seventy-four patients had follow-ups of one year or had a major complication leading to failure prior to one year for evaluation. The failures were the result of avascular necrosis in 17.6% of the patients, non-union in 27% of the patients, and pin breakage in 1.35% of patients. Eight cases with combined av

Finite element analysis of necessity of reduction and selection of internal fixation for valgus-impacted femoral neck fracture.

This study compared the biomechanical characteristics of different treatment strategies based on finite element analysis. Posterior tilt and valgus angle were measured on X-ray from ten valgus-impacted femoral neck fractures, and 7 finite element models that were generated to compare the stress and displacement. The results showed that in the intact femur, von Mises stress was concentrated at the medial and inferior sides of the femoral neck. In valgus-impacted femoral neck fractures, von Mises stress was at the same locations but was 5.66 times higher than that in the intact femur. When 3 can

Intramedullary nailing of femoral diaphyseal metastases: is it necessary to protect the femoral neck?

BACKGROUND: Intramedullary nailing is the accepted form of treatment for impending or pathologic fractures of the femoral diaphysis. Traditional teaching promotes the use of a cephalomedullary nail so that stabilization is provided for the femoral neck in the event that a future femoral neck metastasis develops. However, that approach may add cost, surgical time, blood loss, and added radiation exposure to staff members, and there is limited evidence in the literature that supports this practice. QUESTIONS/PURPOSES: The purpose of our study was to evaluate the incidence of femoral neck metast

Bone metastasis of glandular cardiac myxoma mimicking a metastatic carcinoma.

Skeletal metastasis from a cardiac myxoma is rare. We describe an extremely unusual case of a cardiac myxoma metastasing to the femur in a 46-year-old female presenting with pain in the right hip. Radiographs showed an expansile lytic lesion with pathological fracture involving the neck and proximal shaft of the right femur. Histology revealed features of cardiac myxoma with heterologous glandular elements, which was initially mistaken for a metastatic mucin-secreting adenocarcinoma.

Is FEV(1) an indicator of low bone mineral density in adults? The Fifth Korea National Health and Nutrition Examination Survey.

The association between forced expiratory volume in 1 s (FEV1) and bone mineral density (BMD) is heterogeneous in population-based studies. This study aimed to investigate this relationship when sex and menopausal status in women, BMD sites, and anthropometric measures were taken into account. In 3,975 Koreans (1,783 men, 662 premenopausal women, 1,530 postmenopausal women, aged 40-91 years) participating in the Fifth Korea National Health and Nutrition Examination Survey in 2010-2011, this relationship for BMD at femur (total and neck) and lumbar spine was examined in each subgroup after adju

[Proximal femur remodeling after proximal femur varus-derotational osteotomy in treatment of developmental dysplasia of the hip].

The aim of this paper was to assess proximal femur remodelling after proximal femur varus-derotational osteotomy in treatment of developmental dysplasia of the hip up to skeletal maturity. The analysis included 48 osteotomies performed in 33 children between 1st and 10th year of life. The follow-up period was on average 12 years. Supported by statistical evaluation the analysis showed persistent increase of the neck-shaft angle after proximal femur varus-derotational osteotomy, especially in the younger age groups. It also correlated strongly with the amount of varus fixed at the time of opera

Evolutionary roots of the risk of hip fracture in humans.

The transition to bipedal locomotion was a fundamental milestone in human evolution. Consequently, the human skeleton underwent substantial morphological adaptations. These adaptations are responsible for many of today's common physical impairments, including hip fractures. This study aims to reveal the morphological changes in the proximal femur, which increase the risk of intracapsular hip fractures in present-day populations. Our sample includes chimpanzees, early hominins, early Homo Neanderthals, as well as prehistoric and recent humans. Using Geometric Morphometric methods, we demonstrat

Chondroblastoma of the femoral neck as a cause of hip synovitis.

Chondroblastoma is a rare benign chondroid bone tumor, which typically arises from the epiphysis of a long tubular bone. We present a 13-year-old girl with persistent hip synovitis caused by a chondroblastoma that arose from the femoral neck (metaphysis) and uniquely extended extraosseously into the hip joint. Computed tomography scan clearly demonstrated a tumoral lesion of the left femoral neck and a periosteal reaction along the anterior cortex of the femur, distal to the legion. Magnetic resonance imaging showed reactive synovitis with marked joint effusion of the hip and an extensive edem

Subcapital fracture of a congenitally dislocated hip: revival of Girdlestone resection arthroplasty.

A twenty-year-old, active male with a high congenital dislocation of the hip sustained a displaced subcapital fracture (accompanied by a comminuted transarticular fracture of the ipsilateral distal femur). Such a combination of injuries poses a challenging problem; the English literature does not cite any report describing the occurrence of subcapital fractures in congenitally dislocated hips. This article reviews the treatment options of the fractured neck of the femur, and the reasons for electing to perform a Girdlestone resection arthroplasty.

[Effect of the metaphyseal involvement in Perthes' disease and its importance for the definitive development of the head of the femur].

The article describes the effect of the metaphysical involvement in Perthes' disease on the definitive development of the head of the femur. The classification according to Eyring et al. was used as evaluation criterion. Assessment of the results was affected according to the final deformities seen in the head of the femur, as well as according to the epiphyseal quotient and the quotient of the femoral head/neck, and the degree of shortening of the leg. It was found that the severe types of metaphyseal involvement in Perthes' disease have an extremely unfavourable prognosis.

Risk Factors for Avascular Necrosis of the Femoral Head in Pediatric Femoral Neck Fractures.

Introduction A neck of femur fracture is a rare injury in the pediatric population and is of foremost importance, as it is associated with a high rate of complications. It usually occurs due to high-velocity trauma or a fall from a height. There is a scarcity of data on risk factors and their role in the prognostication of avascular necrosis. The purpose of the study was to retrospectively analyze the association of various risk factors with avascular necrosis (AVN) of the femoral head in patients with a neck of femur fracture in the pediatric age group. Material and methods The study included

Fine mapping of bone structure and strength QTLs in heterogeneous stock rat.

We previously demonstrated that skeletal structure and strength phenotypes vary considerably in heterogeneous stock (HS) rats. These phenotypes were found to be strongly heritable, suggesting that the HS rat model represents a unique genetic resource for dissecting the complex genetic etiology underlying bone fragility. The purpose of this study was to identify and localize genes associated with bone structure and strength phenotypes using 1524 adult male and female HS rats between 17 to 20 weeks of age. Structure measures included femur length, neck width, head width; femur and lumbar spine (

Does the timing of treatment affect complications of pediatric femoral neck fractures?

OBJECTIVES: Pediatric femoral neck fractures were reviewed to compare complications with the time of presentation. METHODS: 34 pediatric femoral neck fractures were studied to detect union time, avascular necrosis, premature physis fusion, and the neck-shaft angle. All complications were comparable between both groups. RESULTS: The mean union time was more in late presented group. Four children developed AVN. The neck shaft angle and pre mature fusion of growth plate were same in both groups. CONCLUSIONS: The frequency of the complications except union time in pediatric neck femur fracture

Unusual Combination of Hip Dislocation with Ipsilateral Femoral Neck and Acetabulum Fracture: A Rare Case Report.

INTRODUCTION: Traumatic dislocation of hip associated with ipsilateral femur neck and acetabulum fracture is a rare and devastating injury as a result of high-energy trauma, with potential complications of femur head aseptic necrosis and hip joint degenerative arthritis. Patients, postoperatively, may encounter decreased functional outcomes and serious challenge, during activities of daily living. Management guidelines lack clarity, with possible options including fracture fragments fixation or hip joint replacement. CASE REPORT: A 45-year-old patient was diagnosed with posterior dislocation …

[Bone mineral density in idiopathic inflammatory myopathies].

The study was undertaken to examine the impact of glucocorticoid therapy (GCT) on mineral bone density (MBD) in patients with idiopathic inflammatory myopathies. Double-energy X-ray absorptiometry was used to determine MBD of the vertebral column and proximal femur in 42 females (30 with the preserved menstrual cycle (PMC), 12 in postmenopause (PMP) with polymyositis/dermatomyositis (PM/DM). All the patients received GCT. A random population sample including 106 females aged 20 to 69 years formed a control group. MBD of the neck of the femur was significantly lower in patients from all age gro
