Machine Learning Model for Classifying the Results of Fetal Cardiotocography Conducted in High-Risk Pregnancies.

PURPOSE: Fetal well-being is usually assessed via fetal heart rate (FHR) monitoring during the antepartum period. However, the interpretation of FHR is a complex and subjective process with low reliability. This study developed a machine learning model that can classify fetal cardiotocography results as normal or abnormal. MATERIALS AND METHODS: In total, 17492 fetal cardiotocography results were obtained from Ajou University Hospital and 100 fetal cardiotocography results from Czech Technical University and University Hospital in Brno. Board-certified physicians then reviewed the fetal cardi

[A process audit of stillbirths in a rural family medical practice].

A process audit was undertaken of 5 stillbirths recorded in a rural family medical practice between 1985-1987. The stillbirth rate for this … urred in the first of them at 38 weeks of a pregnancy complicated by maternal viral hepatitis A (no fetal monitoring for technical reasons), in the second at 39 weeks after continual fetal monitoring for 48 hours, and in the third at 41 weeks in a pregnancy complicated by pre-eclamptic toxemia. It was concluded that probably all 5 fetal deaths could have been prev …

The effect of lumbar epidural anesthesia on fetal heart rate baseline variability.

Baseline fetal heart rate (FHR) variability has become an important parameter in the diagnosis of fetal distress when electronically monitoring the fetus. Loss of the baseline variability has been noted to be associated with fetal distress, and in association with late deceleration or severe variable deceleration patterns has been shown to be ominous. Baseline FHR variability, however, has been modified not only by fetal distress but also by prematurity and the administration of certain drugs to the mother. Because FHR is often monitored during labor in parturients given lumbar anesthesia, the

Successful expectant management of a giant chorioangioma.

Giant chorioangiomas are benign placental tumours, which can have potential severe fetal consequences. Complications in pregnancy include polyhydramnios, fetal hydrops and growth restriction. Such pregnancies can carry a significant risk of poor perinatal outcome and require close monitoring. Therapeutic options include fetoscopic or interstitial vessel ablation, chemosclerosis and embolisation. Where there is no evidence of fetal compromise, such pregnancies can successfully be managed conservatively.

Obstetric aspects of hypoxic ischemic encephalopathy.

Hypoxic ischemic encephalopathy (HIE) describes neonatal encephalopathy that is caused by intrapartum asphyxia and it can result in the long term sequelae of cerebral palsy which is a major cause of disability. The incidence of cerebral palsy has not changed over the last few decades and the challenge to obstetricians remains how best to recognise those babies at risk of this intrapartum insult both before and during labour. Many associations and risk factors are unavoidable or unrecognisable, and others are fairly common and associated with poor predictive value. Intrapartum fetal heart monit

Sonographic diagnosis of chronic abruption.

BACKGROUND: placental abruption is usually an acute event in which clinical decision-making overrides the need for ultrasound imaging. By contrast, chronic abruption may present with vague or even confusing clinical findings. We describe a case in which the diagnosis of chronic abruption was established by ultrasound and the findings directly influenced clinical care. CASE: a 30-year-old woman with asymptomatic preeclampsia was evaluated in our fetal medicine unit at 30 weeks' gestation. Despite normal fetal monitoring, a large, retroplacental sonolucent area was noted on ultrasound. A planne…

Heterogeneity in management of category II fetal tracings: data from a multihospital healthcare system.

BACKGROUND: Electronic fetal monitoring is widely used to identify and intervene in suspected fetal hypoxia and/or acidemia. Category II fetal heart rate tracings are the most common class of fetal monitoring in labor, and intrauterine resuscitation is recommended given the association of category II fetal heart rate tracings with fetal acidemia. However, limited published data are available to guide intrauterine resuscitation technique selection, leading to heterogeneity in the response to category II fetal heart rate tracings. OBJECTIVE: This study aimed to characterize approaches to intrau…

Anesthesia for in utero repair of myelomeningocele.

Recently published results suggest that prenatal repair of fetal myelomeningocele is a potentially preferable alternative when compared to postnatal repair. In this article, the pathology of myelomeningocele, unique physiologic considerations, perioperative anesthetic management, and ethical considerations of open fetal surgery for prenatal myelomeningocele repair are discussed. Open fetal surgeries have many unique anesthetic issues such as inducing profound uterine relaxation, vigilance for maternal or fetal blood loss, fetal monitoring, and possible fetal resuscitation. Postoperative manage…

Technical Update No. 429: Maternal Heart Rate Artefact During Intrapartum Fetal Health Surveillance.

OBJECTIVE: To improve perinatal outcomes and minimize provider error by increasing awareness of strategies to detect intrapartum maternal heart rate artefact and to respond when such artefact is suspected. TARGET POPULATION: All pregnant patients during labour. OPTIONS: Maternal heart rate artefact may be detected based on clinical features or through technology. Suspected maternal heart rate artefact may be assessed by applying a fetal scalp electrode (preferred) or through external fetal monitoring, augmented by point-of-care sonography (alternative). OUTCOMES: Unrecognized intrapartum ma…

[Intrauterine fetal movements and their significance for the condition of the fetus].

A literature review reports the results of investigations about fetal intrauterine activity. Different methods of observation are demonstrated and discussed. The results indicate that real-time-ultrasound is an accurate method for observation of fetal movements. Different patterns are identified: 1. Fetal body and extremity movements Development of fetal activity in early pregnancy is looked at as a process of maturation associated with fetal cerebral function. After 12 weeks of pregnancy typical patterns are established and seen also in the following weeks. Different results are reported abou

The value of Doppler ultrasound in the diagnosis and management of twin-to-twin transfusion syndrome.

To evaluate the efficiency of the Doppler examination of umbilical arterial blood flow for the antenatal diagnosis and the monitoring of fetal condition during intrauterine treatment of twin-to-twin transfusion syndrome (TTTS), we studied 33 pairs of twins including 5 TTTS cases. In all cases umbilical arterial blood flow was examined by Doppler ultrasound and pulsatility index (PI) was calculated as umbilical arterial impedance. In twins with TTTS, PI of the recipient was outside the normal range and the difference of PI was greater than +0.5. In discordant twins without TTTS and concordant t

Combined prenatal and postnatal prediction of early neonatal compromise risk.

OBJECTIVE: Electronic fetal monitoring/cardiotocography (EFM) is nearly ubiquitous, but almost everyone acknowledges there is room for improvement. We have contextualized monitoring by breaking it down into quantifiable components and adding to that, other factors that have not been formally used: i.e. the assessment of uterine contractions, and the presence of maternal, fetal, and obstetrical risk factors. We have created an algorithm, the Fetal Reserve Index (FRI) that significantly improves the detection of at-risk cases. We hypothesized that extending our approach of monitoring to include …

Placental chorioangioma: a rare cause of fetomaternal transfusion with maternal hemolysis and fetal distress.

Multiple placental chorioangiomas in a 31-year-old woman led to a severe, acute fetal-to-maternal transfusion and maternal hemoglobinuria from hemolysis of the fetal red blood cells. Fetal distress was manifest by a sine wave fetal monitor pattern. A severely asphyxiated infant was delivered by cesarean section. The infant did well after transfusion therapy.

A case of massive meconium peritonitis in utero successfully managed by planned cardiopulmonary resuscitation of the newborn.

We report a case of emergent massive meconium peritonitis due to intrauterine volvulus without malrotation. Fetal ascites was detected on a regular ultrasonographic examination, and fetal distress was found on cardiotocographic monitoring. The mother had noticed a slight decrease in fetal movements over the preceding 24 hours. Prenatal magnetic resonance imaging allowed us to distinguish the meconium from fetal peritoneal fluid and to evaluate the degree of compression of the fetal thoracic cavity. The infant was delivered by emergency cesarean section and demonstrated tense abdominal ascites

Systemic lupus erythematosus and pregnancy.

Although most women with systemic lupus erythematosus can achieve a successful pregnancy, fetal (fetal loss, preterm birth) and maternal (lupus flares, worsening renal function) morbidity remain major problems. Predictors of fetal loss and preterm birth have been identified and are reviewed. Modern management of lupus pregnancy includes identification of high-risk pregnancies with appropriate monitoring and treatment (when indicated) for anti-Ro and antiphospholipid antibodies, frequent assessment of and control of maternal lupus activity, adjustment of medications to avoid those associated wi

[Effects of maternal analgesia and anesthesia on the fetus and the newborn].

Continuous electronic fetal heart rate (FHR) monitoring is part of routine care for laboring patients under either systemic or locoregional … orrection of maternal hypotension and/or the use of tocolytics agents. Reinstallation of electronic fetal monitoring at arrival in the operating room before cesarean section for suspected fetal hypoxia may be helpful to choose better anesthetic technique and try to avoid general anesthesia associated with increased maternal morbidity and mortality.
