Management of soft-tissue injuries in distal radius fractures.

Distal radius fractures account for approximately 15% of all fractures in adults, and are the most common fractures seen in the emergency department. Soft-tissue injuries associated with distal radius fractures may influence strategies for the acute management of the fracture, but also may be a source of persisting pain and/or disability despite fracture healing. This article describes soft-tissue injuries and considerations for treatment associated with distal radius fractures, including injuries to the skin, tendon and muscle, ligaments, the triangular fibrocartilage complex, neurovascular s…

Radioulnar laxity and clinical outcome do not correlate after a distal radius fracture.

Injury to the triangular fibrocartilage complex associated with distal radius fracture may cause symptoms of ulnar instability. Assessed by a radioulnar stress test, increased laxity of the distal radioulnar joint has in two previous studies been depicted to be associated with poorer outcome. This prospective study of 40 adults investigates the correlation of this test with functional outcome as measured by DASH. No clinically significant difference was found in relation to this test at two and five years after injury. Therefore using this test alone to decide whether or not to perform an acut…

Ultrastructure of the articular cartilage of the mandibular condyle: aging and degeneration.

To obtain more insight into the pathogenesis of osteoarthrosis of the temporomandibular joint, we examined the ultrastructure of articular cartilage of six healthy and sixteen osteoarthrotic human mandibular condyles. Ultrastructural changes due to aging and osteoarthrosis are described and compared with the findings of other ultrastructural studies of articular cartilage of synovial joints. Aging was accompanied by some slight degenerative signs. Osteoarthrotic hyaline cartilage and fibrocartilage showed a striking similarity. The only ultrastructural difference was the presence of elastic fi…

bFGF-genetically modified bone marrow-derived mesenchymal stem cells promote tendon-to-bone interface healing by promoting chondrogenesis and regulating macrophage polarization.

BACKGROUND: Rapid regeneration and functional recovery of the enthesis following tendon-to-bone interface (TBI) injury remains a significant … injury. Accelerated regeneration of TBI injury was evidently observed in vivo, and promoted natural fibrocartilage differentiation was intimately correlated with the therapy of bFGF-transfected BMSCs. CONCLUSION: This study demonstrated that the employment of bFGF-transfected BMSCs accelerated tendon-to-bone regeneration by regulation of immune responses and pr …

Didymin may Enhance Tendon-Bone Healing Partly by Promoting PPAR-Mediated Fatty Acid Oxidation and Macrophage M2 Polarization.

OBJECTIVE: Achieving rapid and high-quality regeneration after injury at the tendon-bone interface remains an urgent challenge in the fields … ophage polarization were investigated by evaluating various parameters such as tendon-bone healing, fibrocartilage formation at the tendon-bone interface, and macrophage polarization. In addition, in vitro experiments were used to verify whether the PPAR-mediated fatty acid oxidation (FAO) pathway was involved in the induction of macrophage polarization by DID. …

Rotator cuff and biceps tendons that appeared grossly normal were procured from adult cadavers without a history of shoulder problems. These … ongitudinal collagen fiber bundles. The proteoglycan content of rotator cuff tendons was similar to fibrocartilage in tendons that have been subjected to compressive loads in situ. This suggests that cells of normal adult rotator cuff tendons have adapted to loads distinct from pure tension. However, the histological organization did not resemble mature fibrocar …

Gitelman syndrome disclosed by calcium pyrophosphate deposition disease: early diagnosis by ultrasonographic study.

Gitelman's syndrome is a rare autosomal-recessive tubular disorder characterized by hypomagnesemia and hypocalciuria associated to hypokalemia. The clinical spectrum is wide and usually characterized by chronic fatigue, cramps, muscle weakness and paresthesiae. We describe a case of a 43 year-old male patient with early onset of knee arthritis and no other symptoms. Ultrasound revealed diffuse and confluent hyperechoic deposits in cartilage, fibrocartilage of the menisci and synovium and calcium pyrophosphate crystals were observed in the synovial fluid of the knee. The concomitant presence of…

Change perspective to increase diagnostic accuracy of ultrasonography in calcium pyrophosphate dehydrate deposition disease! A new approach: the axial scan of the meniscus.

Ultrasonography (US) is a relevant tool in the study of calcium pyrophosphate dihydrate (CPP) deposition disease. However, differential diagnosis of hyperechoic deposits within the fibrocartilage can be difficult; moreover, US study is limited by the need of an adequate acoustic window. We describe a US scanning technique that offers a new viewpoint in the study of knee meniscal structure: a longitudinal scan performed according to the long axis of meniscus. This technique proves to be particularly useful for the identification of CPP deposition, but could also improve the US diagnostic utilit…

Musculoskeletal Ultrasound Diagnosis in Calcium Pyrophosphate Dihydrate Crystal Deposition Disease.

Calcium pyrophosphate dihydrate (CPPD) crystal deposition disease is extremely versatile. We present the case of a 52 years old patient referred for inflammatory pain of the left knee and ankle of recent onset with intense inflammatory syndrome that mimics rheumatoid arthritis. Musculoskeletal ultrasound was able to detect effusion, hyperechoic crystals within the cartilage and fibrocartilage, inside tendons, suggestive for CPPD disease and to guide fluid aspiration as well as in sustaining a rapid diagnosis corroborating images suggestive for CPPD disease to biological data - hypercalcemia an…

Distal ulna fractures.

In isolation, distal ulna fractures are rare. They are often found in conjunction with distal radius fractures, and the complexity of the interaction of the distal ulna with the radioulnar joint and triangular fibrocartilage complex makes understanding and treatment of distal ulna fractures challenging. Fixation of distal ulna fractures can be problematic owing to comminution making reduction challenging. A thin soft tissue can lead to hardware prominence and necessitate implant removal. In this Current Concepts article, we review the anatomy, pathology, and treatment of distal ulna fractures …

Foveal TFCC tear classification and treatment.

During the last two decades, increased knowledge of functional anatomy and pathophysiology of the triangular fibrocartilage complex (TFCC) have contributed to a change in surgeons' perspective toward it. The earlier concept of the TFCC as the "hammock" structure of the ulnar carpus has updated to the "iceberg" concept, whereby the much larger "submerged" part represents the foveal insertions of the TFCC and functions as the stabilizer of the distal radioulnar joint and the ulnar carpus, thus lending it greater functional importance. This article presents an algorithm of the treatment of trauma…

Sports-related injuries of the wrist: an approach to MRI interpretation.

Sports-related injuries to the wrist range from minor sprains to severe soft tissue disruption that can pose a risk to the normal function of the upper extremity. It is important to identify the specific nature of such injuries so as to establish an accurate diagnosis and deliver appropriate treatment. MRI of the wrist has greatly benefited from the use of dedicated surface coils, which allow fine depiction of soft tissue and cartilaginous structures. A review of the normal anatomy, MR interpretation pitfalls, and most common abnormalities of the tendons, ligaments, triangular fibrocartilage c…

Results of the wafer procedure for ulnar impaction syndrome in the ulnar negative and neutral wrist.

Thirteen wrists with ulnar neutral or negative variance were treated by open distal ulna excision (the wafer procedure). The mean follow-up was 25 months (range, 12-38). At final follow-up grip strength had increased a mean of 14 kgf and 12 of the 13 patients were very satisfied with the functional outcome and pain relief. In treatment of the ulnar impaction syndrome, the wafer procedure provides excellent pain relief and functional restoration particularly in patients with ulnar neutral or negative wrists in whom triangular fibrocartilage tears have not yet developed.

The Galeazzi-equivalent lesion in children revisited.

A fracture at the medial end of the distal third of the radius with an epiphyseal separation of the distal ulna in a 16-year-old boy is described. This injury, known as the Galeazzi-equivalent lesion in children, is characterized by complete distal ulnar epiphyseal separation without rupture of the distal ligamentous stabilizing system between the radius and ulna, which includes the triangular fibrocartilage complex, interosseous ligaments and periosteal tube of the ulnar. The Galeazzi fracture-dislocation and the Galeazzi-equivalent lesion appear to be completely dissimilar in their pathologi…

Matched distal ulna resection for posttraumatic disorders of the distal radioulnar joint.

In 1985 we published the results of the matched ulna resection in 44 patients, the majority of whom had rheumatoid arthritis. The matched ulna resection maintains the continuity of the distal ulna to the ulnar sling mechanism, including the triangular fibrocartilage complex (TFCC), and resects the distal ulna in a smooth, curved, convex fashion to match the contour of the radius throughout forearm rotation. This article presents the results of the procedure in patients with posttraumatic and mechanical disorders of the distal radioulnar joint. Good to excellent results were noted in 24 of 32 p…

A histological study of the development of the penis of wild-type and androgen-insensitive mice.

Development of the penis of wild-type and androgen-insensitive (Tfm) mice was compared histologically to demonstrate possible androgen-dependent histogenesis in this organ. The os penis of the normal males consists of a hyaline cartilage and a membrane bone in the proximal segment and a fibrocartilage in the distal segment. Only the membrane bone of the proximal segment developed in the Tfm mice. The corpus cavernosum penis, corpus cavernosum glandis, and corpus cavernosum urethrae developed only to a small degree in the penis of the Tfm mice. It is also suggested that the morphogenesis of the…
