Vaccine preventable diseases surveillance in Nepal: How much does it cost?

Assessing the cost of vaccine preventable diseases (VPD) surveillance is becoming more important in the context of the Global Polio Eradication Initiative (GPEI) funding transition, since GPEI support to polio surveillance helped the incremental building of VPD surveillance systems in many countries, including low income countries such as Nepal. However, there is limited knowledge on the cost of conducting VPD surveillance, especially the national cost for surveillance of multiple vaccine-preventable diseases. The current study sought to calculate the economic and financial costs of Nepal's co

Reproductive health, and child health and nutrition in India: meeting the challenge.

India, with a population of more than 1 billion people, has many challenges in improving the health and nutrition of its citizens. Steady declines have been noted in fertility, maternal, infant and child mortalities, and the prevalence of severe manifestations of nutritional deficiencies, but the pace has been slow and falls short of national and Millennium Development Goal targets. The likely explanations include social inequities, disparities in health systems between and within states, and consequences of urbanisation and demographic transition. In 2005, India embarked on the National Rural

Community self-help and the homeless poor in Latin America.

Finding realistic housing solutions that are able to respond to the realities of poverty in the developing, or newly industrializing, world are frequently distinct from those suited to the developed world due to levels of poverty and differing welfare regimes. This requires a different understanding of the concept of housing and shelter for developing and developed countries. Population increase and emerging habitation patterns in parts of Latin America have required that policy-makers review traditional 'top down' approaches to the way the homeless poor are treated and how self-help or 'botto

Assessing the integration of social protection into infectious disease policies: a multi-country analysis of TB, HIV, and malaria strategies in Sub-Saharan Africa.

BACKGROUND: Tuberculosis (TB), HIV, and malaria continue to impose major economic hardship on households in Sub-Saharan Africa. While global frameworks such as the WHO End TB Strategy and Universal Health Coverage (UHC) emphasize social protection, evidence remains limited on how national policies translate these commitments into practice. OBJECTIVES: This study assessed how national TB, HIV, and malaria policies and strategic plans in Kenya, Malawi, Mozambique, Nigeria, and Zambia define and operationalize social-protection mechanisms to reduce household economic burden. It also examined ali

Leveraging geo-referenced malaria information to increase domestic financial support for malaria elimination in Thailand.

Thailand's National Malaria Elimination Strategy 2017-2026 seeks to increase domestic support and financing for malaria elimination. During 2018-2020, through a series of training sessions, public health officials in Thailand utilized foci-level malaria data to engage subdistrict-level government units known as Local Administrative Organizations (LAOs) with the aim of increasing their understanding of their local malaria situation, collaboration with public health networks, and advocacy for financial support of targeted interventions in villages within their jurisdictions. As a result of these

Addressing key gaps in implementation of mosquito larviciding to accelerate malaria vector control in southern Tanzania: results of a stakeholder engagement process in local district councils.

BACKGROUND: Larval source management was historically one of the most effective malaria control methods but is now widely deprioritized in Africa, where insecticide-treated nets (ITNs) and indoor residual spraying (IRS) are preferred. However, in Tanzania, following initial successes in urban Dar-es-Salaam starting early-2000s, the government now encourages larviciding in both rural and urban councils nationwide to complement other efforts; and a biolarvicide production-plant has been established outside the commercial capital. This study investigated key obstacles and opportunities relevant t

Enabling climate action: A nexus approach for delivering co-benefits across biodiversity, water, food, and health.

Climate response options rarely affect biodiversity, water, food, and health in uniform ways. This comparative analysis examines why technically viable response options generate uneven cross-nexus outcomes, and how governance and justice conditions shape whether these outcomes become durable co-benefits. Anchored in the IPBES Nexus Assessment, the analysis synthesizes evidence from fifteen response options spanning land, ocean, energy, urban, industrial, and societal transitions. Ecosystem-centered and demand-side pathways more consistently produce synergies because they work through ecologica

Antimicrobial Resistance in Somalia: Policy Without Implementation in a Fragile Health System.

Antimicrobial resistance (AMR) is quietly becoming Somalia's next health catastrophe. While the world focuses on immediate crises, an estimated 8,400 deaths annually are directly attributed to AMR, with another 32,700 linked to AMR complications. These numbers place Somalia among the top 10 countries globally for AMR-related mortality, surpassing deaths from maternal disease, cardiovascular conditions, and infectious disease combined. Yet Somalia has a national action plan. The problem is not policy; it is implementation. Despite developing an AMR National Action Plan aligned with global strat

Strengthening Prehospital Emergency Care in Somalia: Current Barriers and Priorities for System Reform.

Effective prehospital emergency care is a core component of an emergency care system because assessment, initial stabilization, communication, and transport begin before definitive hospital treatment. In Somalia, prehospital services remain fragmented, with substantial reliance on informal transport and limited access to trained responders, standardized protocols, dispatch, and coordinated referral. Somalia-specific reports have documented major gaps in organized prehospital care, including limited ambulance capacity and a lack of a unified national system. This Perspective examines these barr

The role of digital health and artificial intelligence in improving the reach and effectiveness of HIV prevention in Africa.

As global HIV prevention efforts face mounting fiscal pressures and persistent coverage gaps, digital health interventions (DHIs), including artificial intelligence, promise improved effectiveness, equity, and reach. Across several countries in sub-Saharan Africa, digital health tools, including mobile applications and health information systems, are extending service delivery, optimising resource allocation, improving client engagement, and strengthening HIV prevention service delivery. Artificial intelligence applications are further enhancing data analytics, epidemic modelling, hotspot pred

Rights-Based Legal Considerations for Tuberculosis Isolation Practices in Community Settings in the Postpandemic Era.

State and local governments are free to enact infectious disease control laws to protect the public health but must conform those laws to right-based limitations imposed by the United States Constitution. Tuberculosis (TB) control laws that empower public health agencies and their representatives to institute restrictions on movement and activities for persons with TB have remained largely unchanged for decades and warrant review to ensure consistency with modern legal principles. During the coronavirus disease 2019 pandemic, there was increased attention to the tension between nonpharmaceutic

Necropolitics as accumulation: Enforcement and enclosure in Brisbane during COVID-19.

This article explores the uneven impacts that Indigenous and detained migrant populations have endured in Australia in the context of the COVID-19 pandemic. Australia has one of the most restrictive immigration enforcement systems in the world. Along with imposing practices of mandatory detention in rural and remote regions, the Australian government finances the carceral systems of nearby countries and island nations. These logics of enforcement are embedded within histories and techniques of Indigenous quarantine, incarceration, and colonial erasure. Following Achille Mbembe (2019), I advanc

Uneven paths: Soft Policy's benefits to recovery in Louisiana Parishes after Hurricanes Katrina and Rita.

Factors driving community recovery trajectories after disaster are not well understood. We assess why some communities show stronger recoveries from disaster than others, examining the role of four policy toolkits that U.S. county governments frequently adopt to recover from disaster. Using mixed methods, we examine the cases of Hurricanes Katrina and Rita with a novel dataset of recovery policies adopted within each Louisiana parish following the disasters. We typologize recovery strategies and analyze policy adoption patterns after crises. To compare which policy toolkit leads to the best re

Overview of Slovenian Control Programmes for Cattle Diseases Not Regulated by the European Union.

The European Union (EU) regulates the control of cattle diseases listed in categories A and B of the Animal Health Law (AHL). However, the control of other cattle diseases that have no, or limited EU regulation, is left to each member state. Slovenia has five control programmes (CPs) for non-EU regulated cattle diseases: bovine viral diarrhoea (BVD), infectious bovine rhinotracheitis (IBR), enzootic bovine leukosis (EBL), bluetongue and anthrax. Two (IBR and BVD) are voluntary and the others (EBL, anthrax and bluetongue) are compulsory. The three compulsory CPs are funded by the government. Al

The Impact of the COVID-19 Pandemic on Community Health Centers in Canada and the United States: A Scoping Review.

Community health centers (CHCs) play a critical role in addressing the health needs of diverse and underserved communities. However, there remains a significant gap in the literature examining their resilience, particularly in crises such as COVID-19. This scoping review analyzed 68 articles to identify which performance indicators have been used to assess the impact of the pandemic on CHCs, and to understand what these indicators reveal about the CHCs' performance and resilience. The findings revealed that most performance indicators focused on access, availability, and quality of care, highl

Applying principles of health system strengthening to eye care.

Understanding health systems have now become the priority focus of researchers and policy makers, who have progressively moved away from a project-centred perspectives. The new tendency is to facilitate a convergence between health system developers and disease-specific programme managers in terms of both thinking and action, and to reconcile both approaches: one focusing on integrated health systems and improving the health status of the population and the other aiming at improving access to health care. Eye care interventions particularly in developing countries have generally been verticall

Spatial and temporal distribution of infectious disease epidemics, disasters and other potential public health emergencies in the World Health Organisation Africa region, 2016-2018.

BACKGROUND: Emerging and re-emerging diseases with pandemic potential continue to challenge fragile health systems in Africa, creating enormous human and economic toll. To provide evidence for the investment case for public health emergency preparedness, we analysed the spatial and temporal distribution of epidemics, disasters and other potential public health emergencies in the WHO African region between 2016 and 2018. METHODS: We abstracted data from several sources, including: the WHO African Region's weekly bulletins on epidemics and emergencies, the WHO-Disease Outbreak News (DON) and th

Findings from the First Year of a Federally Funded, Direct-to-Consumer HIV Self-Test Distribution Program - United States, March 2023-March 2024.

In September 2022, CDC funded a nationwide program, Together TakeMeHome (TTMH), to expand distribution of HIV self-tests (HIVSTs) directly to consumers by mail through an online ordering portal. To publicize the availability of HIVSTs to priority audiences, particularly those disproportionately affected by HIV, CDC promoted this program through established partnerships and tailored resources from its Let's Stop HIV Together social marketing campaign. The online portal launched March 14, 2023, and through March 13, 2024, distributed 443,813 tests to 219,360 persons. Among 169,623 persons who an

Association of the Overall Well-being of a Population With Health Care Spending for People 65 Years of Age or Older.

IMPORTANCE: New US health care payment models have increasingly incentivized health care systems to promote health and reduce health care spending at the population level, with Medicare beneficiaries representing one of the largest populations affected by new payment models. Identifying novel strategies to promote health and reduce health care spending is necessary. OBJECTIVE: To assess whether the overall well-being of a population is associated with health care spending for people 65 years of age or older. DESIGN, SETTING, AND PARTICIPANTS: This US national, population-based cross-sectiona

Protocol for the evaluation of a free health insurance card scheme for poor pregnant women in Mbeya region in Tanzania: a controlled-before and after study.

BACKGROUND: The use of demand-side financing mechanisms to increase health service utilisation among target groups and enhance service quality is gaining momentum in many low- and middle-income countries. However, there is limited evidence on the effects of such schemes on equity, financial protection, quality of care, and cost-effectiveness. A scheme providing free health insurance cards to poor pregnant women and their households was first introduced in two regions of Tanzania in 2011 and gradually expanded in 2012. METHODS: A controlled before and after study will examine in one district t
