Clinical outcomes following open reduction and internal fixation without primary cruciate ligament reconstruction or avulsion fixation for Schatzker types IV-VI tibial plateau fractures: a retrospective cohort study.

BACKGROUND: Schatzker types IV-VI tibial plateau fractures (TPFs) are frequently complicated by soft tissue injuries. This study aimed to evaluate the clinical and functional outcomes of Schatzker types IV-VI TPFs treated with ORIF alone, and to compare outcomes between patients with and without concomitant anterior or posterior cruciate ligament (ACL or PCL) injuries. METHODS: 316 Schatzker types IV-VI TPFs patients with a mean age of 51.5 ± 13.2 years old who underwent open reduction and internal fixation (ORIF) were enrolled. There were 152, 56 and 108 patients with type IV, type V, and ty

A rare traumatic triad: Open bicondylar non-conjoint Hoffa fracture with partial tibial tubercle and medial epicondyle avulsions - A case report and biomechanical analysis.

UNLABELLED: Coronal shear fractures of the distal femoral condyles are rare intra-articular injuries, and the simultaneous occurrence of a non-conjoint bicondylar Hoffa fracture with a partial anterior tibial tubercle avulsion and a medial epicondyle avulsion constitutes an exceptionally complex traumatic triad. We report the case of a 35-year-old male motorcyclist who sustained this "floating" articular block injury (AO/OTA 33-B3.3) following a high-velocity accident. Diagnostic assessment utilizing Computed Tomography with 3D reconstruction revealed a vertical Letenneur Type II fracture of t

The Use of Suture Tape through a Single Tunnel to Fix Posterior Cruciate Ligament Avulsion Fracture of Tibia, Via Arthroscopy: A Case Series of 16 Patients.

Posterior cruciate ligament (PCL) avulsion fractures from the tibial insertion are rare but clinically notable injuries that can compromise knee stability if not treated effectively. Arthroscopic approaches have increasingly replaced open surgery for such injuries due to their less invasive nature and lower risk of postoperative issues. This retrospective case series evaluates the clinical and radiological outcomes of 16 patients treated with a single suture tape pullout approach for PCL tibial avulsion fractures. The average age of the patients was 29.6 years, and all were treated within 3 we

Catastrophic Mid-Flexion Instability After Avulsion Fractures of the Articular Capsule of the Femur and Tibia in a Patient With Posterior-Stabilized Total Knee Arthroplasty: A Case Report.

Mid-flexion instability can be caused by patient-related, implant-related, or technique-specific factors and impairs the activities of daily living after total knee arthroplasty (TKA). In this study, we report a rare case of a patient with severe mid-flexion instability following tibial and femoral avulsion fractures after posterior-stabilized (PS) TKA for knee osteoarthritis. An 82-year-old female with bilateral knee osteoarthritis underwent staged bilateral TKA with a posterior-stabilized prosthesis. The course of the early postoperative period was good, and the patient was able to walk inde

Arthroscopic reduction of posterior cruciate ligament tibial avulsion fracture using two cross-linked pull-out sutures: A surgical technique and case series.

Surgical treatment of the posterior cruciate ligament (PCL) tibial avulsion fracture is challenging due to the deep-seated location of the lesion with complex adjacent anatomy and usually with small-sized bone fragment. We introduce a novel arthroscopic reduction technique using two cross-linked pull-out sutures (2XLPOS) through triple bone tunnels in posterior cruciate ligament (PCL) tibial avulsion fracture. Posterior trans-septal portal was established following the four standard arthroscopic portals. Bilateral margins of the PCL with 1∼2 mm margin from the border were penetrated using sutu

Severe cartilage damage from a broken absorbable screw head after fixation of an avulsion fracture of the tibial attachment of the posterior cruciate ligament: A case report.

INTRODUCTION: The use of bioabsorbable interference screws has become popular for treatment of avulsion fractures of the posterior cruciate ligament (PCL). Complications are uncommon. We report a case of severe chondral damage caused by the early breakage of an absorbable screw head after fixation of an avulsion fracture of the tibial attachment of the PCL. The patient felt a sudden locking of the knee when getting off a car at 4 months after the PCL surgery. MRI revealed intraarticular migration of the head of the interference screw. During revision surgery, the broken part was removed withou

Ipsilateral Posterior Cruciate Ligament Insertion Avulsion and Femoral Head Fracture Without Dislocation: A Rarest of Rare Dashboard Injury and Principles of Management: A Case Report.

CASE: A 25-year-old man presented with posterior cruciate ligament (PCL) avulsion and ipsilateral fracture of the femoral head after sustaining a dashboard injury. The hip was not dislocated on presentation, and there were no other injuries; he was managed operatively for both. His hip and knee healed well, with good functioning at 15 months of follow-up. CONCLUSIONS: Dashboard injuries can cause simultaneous knee and hip injuries, usually PCL and posterior hip dislocation. This case report identifies a specific pattern of tibial avulsion fracture of the PCL combined with femoral head fractur

Reduced bone mineral density and computed tomography values of the proximal tibia are associated with avulsion fractures of the anterior and posterior cruciate ligaments in adults: A case-control study.

BACKGROUND: While recent studies of cruciate ligament injuries have predominantly focused on tibial and femoral morphological characteristics, the relationship between bone mass and the risk of tibial attachment avulsion fracture (TAF) remains unclear. To determine whether compromised bone quality contributes to TAF occurrence, we examined: (1) whether bone mineral density (BMD) differs between TAF patients and controls; (2) whether proximal tibial bone mass measured by three-dimensional computed tomography (3D-CT) differs between groups; and (3) which CT plane best predicts TAF risk.
