Joint degeneration following closed intraarticular fracture in the mouse knee: a model of posttraumatic arthritis.

Posttraumatic arthritis is one of the most frequent causes of disability following joint trauma. The objective of this study was to develop a model of a closed articular fracture in the mouse knee joint to quantify the temporal sequence of joint degeneration in a model of posttraumatic arthritis. Closed intraarticular fractures were created in the tibial plateau of adult mice (C57BL/6) using a computer-controlled materials testing system and a custom-built indenter tip. Tibial plateau fractures were classified and imaged over time using high-resolution digital radiography. Animals were sacrifi

Closed reduction with CT-guided screw fixation for unstable sacroiliac joint fracture-dislocation.

BACKGROUND: Unstable posterior pelvic ring fractures and dislocations are uncommon but potentially life-threatening injuries in children. Early definitive management reduces risk of immediate complications as well as chronic pain and gait dysfunction. Conventional operative therapy carries substantial risk of extensive blood loss and iatrogenic neurological and vascular injury. Minimally invasive image-guided intervention may further reduce immediate risk and improve long-term outcome. OBJECTIVE: To describe CT-guided closed reduction and internal fixation (CRIF) and review outcomes of unstab

Effect of recombinant human osteogenic protein-1 on the healing of a freshly closed diaphyseal fracture.

Osteogenic protein-1 (OP-1), or bone morphogenetic protein-7, is an osteoinductive morphogen that is involved in embryonic skeletogenesis and in bone repair. In bone defect models without spontaneous healing, local administration of recombinant human OP-1 (rhOP-1) induces complete healing. To investigate the ability of rhOP-1 to accelerate normal physiologic fracture healing, an experimental study was performed. In 40 adult female goats a closed tibial fracture was made, stabilized with an external fixator, and treated as follows: (1) no injection; (2) injection of 1 mg rhOP-1 dissolved in aqu

[Mandibular fracture caused by absolute close-range gunshot with a blank cartridge fright weapon].

Bony injuries caused by starter's pistols were repeatedly observed in the form of fractures of the temporal squama and other thin osseous lamellae; in experiments rib fractures could also be produced. In the presented case a contact shot from a blank cartridge pistol caused a fracture of the lower jaw (transverse fracture with a bone fragment split off in reg. 46). The friend of the 17-year-old victim had fired a shot to the right mandibular region during an argument. In the depth of the gunshot entrance wound, which communicated with the oral cavity, intense soot deposits were found as signs

Blunt vascular injury associated with closed mid-shaft femur fracture: a plea for concern.

During a period of six years, 765 consecutive patients were treated by the trauma service at Allegheny General Hospital for closed mid-shaft femur fractures that were a component of their injury complex. Thirty-one patients underwent angiography of the involved extremity for indications including loss of pulses in eleven and large hematomas or deformities of the thigh in the remainder. Ten patients (1.3%) were found to have acute vascular injuries. In nine patients there was an intimal flap of the superficial femoral artery (SFA), and in one, a pseudoaneurysm. Two patients had injuries of the

[Necrotizing fasciitis after closed pelvic ring fracture. Case report and review of the literature].

We are reporting a case of necrotizing fasciitis occurring on closed pelvis fracture and vulva injury, striking not only the soft tissue, but spreading all over the bone and the hip joint, too, and inducing a septic shock syndrome. The very atypical course of disease is described, pathogenesis, diagnosis and therapy are discussed and literature is reviewed.

Functional and Radiological Outcome of Unstable Distal End Radius Fractures After Management With Closed Reduction and Fixation With Percutaneous Pinning Versus Closed Reduction and Colles' Cast Application: A Comparative Study.

BACKGROUND: Distal end radius fractures are a common orthopaedic presentation and among the most frequent fractures in adults. Management options include closed reduction with Colles' cast immobilisation (CRCI) and closed reduction with percutaneous pinning (CRPP). This study aimed to evaluate and compare the functional and radiological outcomes of two treatment modalities for unstable distal radius fractures. METHODS: A prospective comparative study was conducted in the Department of Orthopaedics of Dr. Baba Saheb Ambedkar Medical College and Hospital, a tertiary care centre in New Delhi, In

Closed Reduction Nailing or Open Reduction Plating in Unstable Paediatric Forearm Fractures: A Case of Paediatric Forearm Fracture Implant Failure.

A significant amount of all paediatric fractures are forearm fractures involving the radius, ulnar shaft, or both. As surgical stabilisation lowers the likelihood of re-displacement, surgical intervention is currently recommended over conservative treatment of such fractures involving significant displacement and angulation. Open reduction and plating can better anatomically repair the majority of fractures. Bracing is necessary for the first six to eight weeks after nailing since nailing does not give a rigid fixation. External bracing is generally not necessary for plating. In our facility,

Subcapital femoral neck fracture after closed reduction and internal fixation of an intertrochanteric hip fracture: a case report and review of the literature.

A subcapital femoral neck fracture in a healed intertrochanteric fracture treated by an open reduction and internal fixation is a rare, but catastrophic, event. We present the case of an 86-year-old woman, a community ambulator, who sustained a displaced right intertrochanteric hip fracture during a fall. She was treated with closed reduction and internal fixation with a dynamic compression hip screw and side plate. Four months later, she was noted to have a displaced subcapital femoral neck fracture and underwent hip screw and side plate hardware removal and cemented bipolar hemiarthroplasty.

Unstable vertebral fractures in the lower third of the spine treated with closed reduction and transpedicular posterior fixation: a retrospective analysis of 82 fractures in 78 patients.

Seventy-eight patients were treated with closed reduction and transpedicular fixation for 82 spine fractures. The fractures were localised in the lower third of the spine and were all, according to the Denis classification, considered unstable. Eighteen patients had neurological deficiencies. One patient with a fracture in T11 was completely paraplegic, four patients had a cauda equina syndrome while the rest had radicular symptoms only. Primary reconstruction of the vertebral height and the physiological curves of the spine was satisfactorily obtained. An improvement in the neurological sympt

Emergency Department Procedural Sedation Versus Operating-Theater General Anesthesia for Closed Reduction of Pediatric Forearm Fractures: A Systematic Review.

Displaced pediatric forearm fractures requiring closed reduction are traditionally managed in the operating theater under general anesthesia (GA). However, emergency department (ED)-based procedural sedation may reduce treatment delays, admissions, and resource utilization. This systematic review evaluated clinical outcomes, safety, repeat intervention, and resource implications of ED procedural sedation compared with theater-based GA and identified factors associated with unsuccessful ED management. A systematic review was conducted in accordance with the Preferred Reporting Items for Systema

A novel closed reduction technique for treating femoral shaft fractures with intramedullary nails: Kirschner wire and the fulcrum technique.

Femoral shaft fractures (FSF) are usually treated with closed reduction and intramedullary nailing. In this study, we report a novel technique with Kirschner wire to achieve closed reduction. From January 2020 to January 2023, this technique was used on 32 patients with FSF (AO 32 A and 32 B). The fracture was reduced with a radiolucent traction table, 3.0 mm Kirschner wire and the fulcrum technique was conducted. The operative effect was evaluated on the basis of the operative time, reduction time, fluoroscopy time, and intraoperative blood loss. All 32 patients were treated by closed reducti

Delayed fixation of distal radial fractures beyond three weeks after initial failed closed reduction increases the odds of reoperation.

AIMS: We aimed to compare reoperations following distal radial fractures (DRFs) managed with early fixation versus delayed fixation following initial closed reduction (CR). METHODS: We used administrative databases in Ontario, Canada, to identify DRF patients aged 18 years or older from 2003 to 2016. We used procedural and fee codes within 30 days to determine which patients underwent early fixation (≤ seven days) or delayed fixation following CR. We grouped patients in the delayed group by their time to definitive fixation (eight to 14 days, 15 to 21 days, and 22 to 30 days). We used interve
