The Usefulness of Surgical Titanium Microclips for Mucosal Repair in the Frontal Sinus Using ORBEYE: A Technical Note.

Bifrontal craniotomy frequently involves opening the frontal sinus and mucosal injury. We report a new technique for mucosal repair in the frontal sinus using surgical titanium microclips. Six consecutive patients who underwent bifrontal craniotomy with frontal sinus exposure and mucosal injury underwent mucosal repair using surgical titanium microclips between April 2019 and August 2022. In all cases, the frontal sinus mucosa was peeled from the inner walls of the frontal sinus to ensure sufficient mucosal margin for clipping using ORBEYE. The repair was accomplished with the microclips in al

Epidermoid cysts of the cranial bones.

Ectodermally derived tissue in ectopic sites is seen in the head and neck. Extracranial dermoid and epidermoid tumors are relatively rare. While most otolaryngologists are familiar with the cholesteatoma of the otic area, not very many are exposed to this lesion in other sites. Those of the cranial bones are even rarer still. These tumors may expand (1) laterally in the cranial bones, (2) externally to present as masses in the scalp or facial region, or (3) internally to involve the intracranial contents. A patient with a frontal bone epidermoid, whose tumor had expanded in all three direction…

Aesthetic correction of a hypertrophic sinus frontalis: a long-term follow-up evaluation and refinement of the method.

Two cases treated for hypertrophy of the frontal sinus are presented. In both patients, the excised frontal bone was inverted, and the resultant cavity was filled with either bone dust from the parietal region or Medpor. The first patient is shown with an excellent result after a follow-up period of 16 years. The second patient, who also achieved an excellent result, is presented after a follow-up period of 5 years. Using Medpor instead of bone dust resulted in the advantage that the operation could be performed with the patient under local anesthesia with sedation and prevention of an additio…

Rotating distraction osteogenesis in a child with secondary craniosyntostosis.

Distraction osteogenesis has been acknowledged as a most effective surgical method to maximize intracranial space in patients with craniosynostosis by progressive displacement of the skull flap. However, it is difficult to control the growth direction of the skull flap even with application of four distraction bars. The authors of this study performed a cranioplasty procedure in a four-year-old girl with brachicephalic microcephaly, who showed a high lumbar puncture pressure of 21 cm H2O. After bilateral frontal craniectomy, the frontal bone and the midline of the superior orbital bone were fi…

Multi-fragment Onlay Reconstruction in Forehead Contouring Surgery: A Novel Hardware-Free Technique for Frontal Sinus Remodeling.

BACKGROUND: Forehead contouring plays a pivotal role in craniofacial and aesthetic plastic surgery, particularly in patients undergoing facial feminization or seeking correction of frontal bossing. Traditional methods often rely on anterior frontal sinus wall osteotomy with titanium plate fixation, which, while effective, carries risks, such as foreign body reaction, infection, hardware palpability, and interference with natural bone healing. To address these limitations, we propose a hardware-free technique utilizing multi-fragment onlay reconstruction following sinus obliteration. METHODS:

Giant Frontal Sinus Mucocele With Headache and Vision Changes: Case Report.

Giant frontal sinus mucoceles, caused by ostial obstruction, can lead to severe complications such as headaches, vision impairment, and brain compression. Early diagnosis through imaging and prompt surgical intervention, such as craniotomy, is critical to remove mucopurulent debris and necrotic bone, preventing vision loss and intracranial complications. For giant frontal sinus mucoceles with extensive bony necrosis and intracranial extension, open craniotomy for complete debridement of mucopurulent debris and necrotic bone may be necessary over endoscopic marsupialization to prevent recurrenc

[Personal modification of the Codubix prosthesis in craniofacial plastic surgery].

Typical "Codubix" prosthesis usually applied to cover cranial defect was used in craniofacial reconstruction. In this case the defect resulted from total removal of cranial basis neoplasm infiltrating orbital roof and surrounding structures. The cranial defect included right frontal bone--superciliary arch, orbital roof and margin; zygomatic bone frontal part junction and the lesser and greater wings of the sphenoid. Cranial reconstruction in this spatially differentiated area is technically difficult. The authors discuss the way of standard prosthesis modification in order to apply it in cran…

Bilateral Through-and-Through Trajectory of a Low-Velocity Transcranial Penetrating Foreign Object in a Twelve-Month-Old.

A 12-month-old girl sustained a penetrating intracranial trauma of a thin aluminum rod traversing from the left frontal bone and exiting the right occipital bone. The rod entered the left anterior frontal lobe, traveled through the ventricular system, narrowly missed the right posterior cerebral artery by less than 1 mm and exited through the right cerebellum. The rod was surgically extracted, and the child remained neurologically intact. Pre- and postoperative vascular imaging, antibiotics, seizure prophylaxis and surgical planning are paramount to successful care of these delicate cases.

The craniofacial skeleton in anencephalic human fetuses. II. Calvarium.

A detailed study of the calvarium of twelve anencephalic and four normal human fetuses 26 to 40 weeks gestational age using gross dissection, alizarin red S staining, silver nitrate radiography and histology revealed dramatic alterations in the presence, form, location and relationship of the individual bones. In the larger dorsal cranial defects the interparietal portions of the occipital bone were relocated anteriorly to approximate the frontal bone. The occipital components were rotated anterolaterally and inferiorly with lack of fusion of the chondrocranium posterior to the foramen magnum.…

Minimally Invasive Treatment With a Patient Specific Implant in Reconstruction of Isolated Anterior Wall Fracture of the Frontal Sinus.

Isolated fractures of the anterior wall of the frontal sinus are most often treated through a coronal approach. Although the coronal approach is a relatively easy procedure, the size of the incision is causing more problems related to patient morbidity and recovery time than smaller approaches. A novel, minimal invasive procedure for reconstruction of the anterior wall fracture of the frontal sinus is presented in this article. An endoscopic assisted approach to camouflage the defect of the anterior wall and restore the contour of the frontal bone with a titanium patient specific implant is de…

The repair of frontal encephaloceles in Papua New Guinea.

Frontal encephaloceles are common in Papua New Guinea (PNG). Seventeen cases collected over 7 years are reported. Eleven frontal (sincipital) encephaloceles were repaired successfully via an extracranial approach. One of these patients with a frontonasal encephalocele developed a recurrence following the extracranial approach, which was subsequently repaired intracranially. Three patients with small naso-ethmoidal encephaloceles were repaired intracranially via an extradural approach. The other three cases have not yet had surgical correction. Complex craniofacial surgery which corrects hypert

Possible therapeutic trepanation on a frontal sinus of a skull from the Old Kingdom period of Egypt.

Trepanation has been reported on skulls from major ancient civilizations since the Neolithic period. In this study, on a skull excavated from the Qau village site during the Old Kingdom period of ancient Egypt (7th to 8th dynasties; 2181-2160 BCE), we found a highly probable sign of therapeutic trepanation in the frontal bone. The practice of trepanation was suggested by the healing sign observed at the site on the frontal bone of case QAU-26. Based on our radiological findings, we presume that this ancient Egyptian male likely suffered from frontal sinus pathology, which may have led to the a…

[Effects of trauma to the facial bones on the anterior shelf of the base of the skull (author's transl)].

The different types of fractures of the upper jaw and facial bones and their extension to the anterior wall of the frontal sinus are discussed. Possible effects on the bony floor of the anterior shelf of the base of the skull and subjacent meninges are outlined. These injuries are then reviewed as part of the larger picture of lesions of the middle third of the craniofacial region. Several types can be distinguished:I.--Lesions in the anterior wall of the frontal sinus only, II.--Lesions affecting the base of the facial bones, III.--Lesions in the frontal vault radiating to the base, IV.--Dire

Use of a new mucoperiosteal flap from the upper lateral nasal wall in frontal sinus surgery.

Various mucoperiosteal flaps from the nose (nasal walls) are in use today in frontal sinus surgery in cases where a wide opening between the frontal sinus and the nasal cavity is created. These flaps are transferred to cover denuded bone in the area of opening and thus prevent the opening from narrowing or closing, which would result in the reappearance of frontal sinus problems. In this paper, a new mucoperiosteal flap taken from the upper extension of middle turbinate is described.

Frontal sinus approach to the orbit. Technical note.

The authors have previously advocated a supraorbital approach to tumors of the orbit. In this paper, they describe a technique in which they take advantage of a large frontal sinus as a means of entering the orbit without the necessity of intracranial exposure, as required by the more conventional supraorbital approach. This is achieved without frontal burr holes, allowing for a superior cosmetic result. The anterior wall of the frontal sinus is removed, and with it the roof of the orbit as a single bone flap. A case in which this technique was used is described.
