OBJECTIVE: The aim of this study was to clarify the role of ostiomeatal complex obstruction in maxillary fungus ball.
STUDY DESIGN AND SETTING: Comparative study in a hospital setting of the mean Lund-Mackay scores for the anterior ethmoid and frontal sinuses of 54 versus 48 patients with maxillary fungus ball versus chronic unilateral rhinosinusitis, respectively.
RESULTS: In cases with partial opacification in the maxillary sinus, the anterior ethmoid and frontal sinuses were diseased in the chronic unilateral rhinosinusitis group but not in the maxillary fungus ball group. In cases with t

With continued surgical instrument advancement, endoscopic sinus surgery techniques continue to evolve with an overall improvement of our ability to offer surgical treatment for a variety of surgical disorders of the sinuses and anterior skull base. Frontal sinusitis and the extent of surgery performed in the frontal recess once medical management fails are constantly debated in the literature. It seems that even though instrumentation has greatly improved, surgical management of frontal sinusitis remains a frustrating endeavor for otolaryngologists. Open approaches to the frontal sinus, inclu…

Transcranial extension of frontal sinus infection is a rare, but not eradicated entity. We present a 21-year-old male, in whom a persistent scalp abscess heralded the discovery of skull vault osteomyelitis and extradural abscesses secondary to frontal sinusitis. Patients with prolonged or unusual symptoms with a history of sinusitis or trauma warrant further investigation as they may have developed serious intracranial complications. Urgent management, both surgical and antimicrobial, is indicated in such scenarios.

Over a 2-year period, 21 patients with clinical and radiologic evidence of persistent or recurrent frontal sinusitis who had a prior ethmoidectomy and/or frontal sinusotomy underwent an endoscopic Lothrop procedure. The patients' chief complaints were headaches (13), nasal obstruction and/or purulent rhinorrhea (4), orbital abscess/cellulitis (2), anosmia (1), and cough (1). Preoperative frontal headaches were present in 19 patients. The common frontal ostium remained patent (> 50% of intraoperative size) by flexible fiberoptic examination and transillumination 2-24 months postoperatively in 1…

From a study of nitric oxide (NO) output in the nose and sinuses it seems that: (i) the results obtained regarding the regulation of NO output in the nose do not necessarily apply to the sinuses; (ii) the results obtained for one group of sinuses may not apply to another; and (iii) NO output in the sinuses does not behave as one would expect if it serves to protect against infection.A pilot study was undertaken in one subject to determine whether the control of NO output in the nose differs from that in the sinuses.NO output was measured by aspirating different gaseous concentrations of oxygen

BackgroundThe frontal sinus drainage pathway (FSDP) includes the frontal recess as its most anatomically complex segment, where surrounding cell pneumatization can influence drainage patterns. Despite this variability, large-scale 3-dimensional characterization of the drainage pathway in both opacified and nonopacified sinuses remains limited, particularly with operative validation.ObjectiveTo perform 3-dimensional mapping of the FSDP and evaluate its relationship to adjacent cells, focusing on drainage patterns associated with frontal sinus opacification.MethodsThis single-center cross-sectio

Seventy-four verified cases of cerebral abscess seen in the Regional Neurosurgical Centre at Addenbrookes Hospital, Cambridge between 1965 and 1987 were reviewed. During that time no decline in incidence or change in the associated mortality was found to occur. ENT disease, taken as a whole, represented the most common source (40%) while acute frontal sinusitis (23%) proved to be the most common single underlying cause. Cerebral abscesses of sinogenic origin in particular were diagnosed late, with a deteriorating conscious level being the precipitating event in 94% of cases. Over 80% of such p…

Many surgical techniques have been described for the treatment of recurrent or persistent frontal sinusitis. The major difficulty of all techniques is to avoid recurrent stenosis of the frontonasal duct. Our endoscopic surgical technique without an external approach is described, which allows the creation of a bony and rigid frontonasal duct. Our first results seem to confirm the hypothesis that the creation of such a duct reduces the frequency of secondary stenosis and therefore the recurrencies of frontal sinusitis.

A 48-year-old man visited the emergency department of our hospital with swelling of the left upper and lower eyelids from the day before. On the first examination, he had severe swelling of the left upper and lower eyelids, proptosis, and chemosis. Left intraocular pressure was 33 mmHg. Computed tomographic images showed an orbital abscess in the anterosuperolateral orbital space, maxillary and ethmoidal sinusitis, and dacryocystitis. The orbital abscess was not contiguous to maxillary and ethmoidal sinusitis and dacryocystitis. Ground-glass appearance was seen in the frontal, maxillary, and e

Intractable frontal sinus infection after obliteration of the injured sinus and nasofrontal duct has become a less common injury nowadays. Adequate debridement complemented with a viable peri-cranial flap would usually prevent the troublesome frontal sinusitis. Nonetheless if recurrent infection involves both the frontal and ethmoid sinuses, local tissues may not suffice for obliteration of the dead space. We developed the use of a free vascularised muscle flap that may be the best option in treating osteomyelitic sinusitis. Three cases of intractable fronto-ethmoid osteomyeltic sinusitis trea…

Frontal sinusitis and its treatment remain controversial. Although several authors have evaluated the results of traditional endoscopic sinus surgery, few have assessed patient outcomes and quality of life after osteoplastic frontal sinus obliteration. This retrospective study evaluates these outcomes in a postoperative review and questionnaire of 39 patients treated at the University of Washington from 1991 to 1994. Most patients were satisfied with the results of this procedure, had significant improvements in their Chronic Sinusitis Survey scores, and reported decreased clinic visits and an…

An 8-year old boy with a history of trauma, sinusitis and a swelling of the frontal bone with somnolence was diagnosed with a Pott's puffy tumor (PPT). Minimal invasive surgical intervention was performed together with a strict regimen of antibiotic therapy. In this case debridement of the frontal bone was not necessary. Serial X-ray imaging of the skull showed complete ossification of the frontal bone lesion. Early diagnosis using thorough radiological evaluation is necessary to effectuate the proper therapeutic approach. For this reason, a patient with a forehead swelling and a history of tr

Six cats with chronic recurrent rhinitis and frontal sinusitis were treated by surgical ablation of their frontal sinuses by the implantation of autogenous fat grafts and curettage of the remaining ethmoid conchae. Deep nasal bacterial cultures and skull radiographs were performed before surgery. Excellent results were achieved in four of six cats and there was significant improvement in two of six cats. Median follow-up time was 15 months. Subcutaneous emphysema over the temporal area was managed by needle aspiration and bandaging. Frontal sinus ablation with autogenous fat graft in combinati…

INTRODUCTION: Although encephalocele is a rare congenital abnormality, secondary encephalocele is extremely rare and can cause fatal complications. Here, we report a case of secondary encephalocele caused by frontal sinus wall defect due to chronic sinusitis, which was completely removed by cranialization with autologous bone graft. A 50-year-old man with a 10-year history of chronic sinusitis visited our hospital due to suddenly altered mentality characterized by stupor. Computerized tomography scanning and magnetic resonance imaging revealed an enlarged left frontal sinus with sinusitis. The

Pott's puffy tumor is a rare and serious complication of frontal sinusitis, characterized by the development of osteomyelitis and subperiostal abscess in the frontal bone. Paranasal sinus osteomas are benign osteoblastic tumors, usually seen in the 3rd and 4th decades of life. In this report, we present a case of Pott's puffy tumor due to frontal sinus osteoma in an adolescent male patient. In the literature, we found no similar case in the pediatric age group and we wanted to emphasize the development of Pott's tumor as a rare complication of chronic or recurrent sinusitis and draw attention …

The paranasal or accessory nasal sinuses begin their development as evaginations of the mucosa during the 3rd and 4th fetal months, but undergo their major expansion after birth according to the development of the facial cranium and the teeth. While in the newborn the sphenoidal and frontal sinuses are still only predispositioned, the ethmoidal labyrinth and particularly the maxillary sinus are of appreciable size. In the 4-year-old the frontal and sphenoidal sinuses are already developed and in the 12-year-old the paranasal sinuses reach their final form and a size equivalent to the adult.
