Sequential treatment of chronic constipation in paraplegic subjects.

Chronic constipation is the main gastrointestinal complaint of spinal cord injury (SCI) patients, and has a significant effect on patients' … treatment the patients showed an increased frequency of bowel movements per week, a decreased total gastrointestinal transit time, and a decreased need for oral and rectal laxatives. This treatment seems to be effective in modifying patients' bowel habits, and therefore could be considered as a standardised protocol for the management of severe constipation in those who a …

The International Network on Oesophageal Atresia (INoEA) consensus guidelines on the transition of patients with oesophageal atresia-tracheoesophageal fistula.

Oesophageal atresia-tracheoesophageal fistula (EA-TEF) is a common congenital digestive disease. Patients with EA-TEF face gastrointestinal, surgical, respiratory, otolaryngological, nutritional, psychological and quality of life issues in childhood, adolescence and adulthood. Although consensus guidelines exist for the management of gastrointestinal, nutritional, surgical and respiratory problems in childhood, a systematic approach to the care of these patients in adolescence, during transition to adulthood and in adulthood is currently lacking. The Transition Working Group of the Internation

Inhibitory effect of Ipomoea aquatica extracts on glucose absorption using a perfused rat intestinal preparation.

Investigations were carried out to evaluate the effect of Ipomoea aquatica aqueous and dichloromethane/methanol extracts on the glucose absorption using a rat intestinal preparation in situ. Extracts orally tested at the dose of 160 mg/kg exerted a significant inhibitory effect on glucose absorption when compared with control animals. The most pronounced effect was observed with the aqueous extract. Ouabain used as reference inhibitor strongly inhibited glucose absorption. On the other hand both plant extracts inhibited the gastrointestinal motility suggesting that the inhibition of glucose ab

The effect of passive immunization against somatostatin on marker retention time in lambs.

The effects of passive immunization against somatostatin on retention time of marker have been investigated in sheep using Cr2O3 as a marker. The mean transit time for clearance of 50% of the marker was significantly retarded in all the anti-somatostatin-treated animals by approximately 19.0 h, as compared with control lambs given a nonspecific antiserum. The findings suggest an important role for circulating somatostatin in the regulation of the rate of flow of digesta through the gastrointestinal tract in lambs. Moreover, it enhances the claim of the immunological approach as a new technique

Silk-based double-sided magnetic microneedles for enhanced targeted retention in the gastrointestinal tract.

Gastrointestinal-retentive drug delivery devices represent a novel oral drug delivery approach capable of significantly enhancing patient compliance and improving local drug delivery efficiency compared to conventional oral formulations. However, developing gastrointestinal-retentive drug delivery devices with high retention capability based on compatible material systems presents significant challenges. Here, we introduce silk-based double-sided magnetic microneedles (SDMMs) designed to facilitate targeted oral drug delivery with precise local retention in the gastrointestinal tract. The SDMM

Gastrointestinal manifestations of systemic vasculitis.

Systemic vasculitis is known to affect the gastrointestinal tract but the nature of the complication is poorly characterized. Out of 65 patients with systemic vasculitis, the majority of whom had renal disease, the intestine was found to be affected in 18. These comprised four of eight patients with polyarteritis nodosa, nine of seventeen with microscopic polyarteritis, four of thirty-six with Wegener's granulomatosis and one of four with Churg-Strauss syndrome. The features included abdominal pain (85 per cent), diarrhoea (50 per cent), gut haemorrhage (44 per cent) and abnormal liver functio

Microencapsulation Strategies in Veterinary Medicine: Overcoming Gastrointestinal Barriers in Monogastric and Ruminant Species.

Oral delivery of bioactive compounds in veterinary medicine offers important opportunities to improve animal health, productivity, and welfa … performance of orally administered compounds. In monogastrics, gastric acidity, digestive enzymes, gastrointestinal transit, and microbiota-mediated interactions represent major barriers, whereas in ruminants, ruminal fermentation, prolonged retention, and physicochemical conditions may cause premature degradation of bioactive compounds. These barriers significantly hind …

Soft hybrid intrinsically motile robot for wireless small bowel enteroscopy.

BACKGROUND: Difficulties in establishing diagnosis of small bowel (SB) disorders, prevented their effective treatment. This problem was largely resolved by wireless capsule endoscopy (WCE), which has since become the first line investigation for suspected SB disorders. Several types of WCE pills are now used in clinical practice, despite their limitations and complications. WCE pills are large, rigid and immotile capsules. When swallowed, they provide SB enteroscopy downloaded to a data logger carried by the patient. Most of the complications of WCEs result from lack of intrinsic locomotion: i

Complementary and antagonistic effects of combined glucagon-like peptide-2 and glucagon-like peptide-1 receptor agonist administration on parameters relevant to short bowel syndrome.

BACKGROUND: Short bowel syndrome (SBS) is characterized by malabsorption requiring parenteral nutrition. The intestinotrophic glucagon-like peptide (GLP)-2 receptor agonist, h[Gly2]GLP2, is used to treat patients with SBS. Evidence suggests that GLP-1 receptor agonists such as exendin-4 (Ex4) may be beneficial in SBS given their ability to increase intestinal growth and delay gastric emptying (GE). METHODS: Intestinal growth, body weight (BW), food intake (FI), GE, gastrointestinal (GI) transit, intestinal permeability, and glucose tolerance were investigated in male and female C57/BL6 mice f

Ameliorating effects of sacral neuromodulation on gastric and small intestinal dysmotility mediated via a sacral afferent-vagal efferent pathway.

BACKGROUND/AIMS: In a recent study of sacral nerve stimulation (SNS) for colonic inflammation, a possible spinal-vagal pathway was implicated. The aim of this study was to provide evidence for such a pathway by investigating the effects of SNS on dysmotility of the stomach and duodenum that are not directly innervated by the sacral efferents. METHODS: Twenty-seven rats were chronically implanted with wire electrodes for SNS and gastrointestinal slow waves. SNS was performed in several acute sessions to investigate its effects on gastric/duodenal slow waves and emptying/transit impaired by glu

The Effect of Trimebutine on the Overlap Syndrome Model of Guinea Pigs.

BACKGROUND/AIMS: Functional dyspepsia (FD) and irritable bowel syndrome (IBS) are common gastrointestinal (GI) disorders and these patients frequently overlap. Trimebutine has been known to be effective in controlling FD co-existing diarrhea-dominant IBS, however its effect on overlap syndrome (OS) patients has not been reported. Therefore, we investigated the effect of trimebutine on the model of OS in guinea pigs. METHODS: Male guinea pigs were used to evaluate the effects of trimebutine in corticotropin-releasing factor (CRF) induced OS model. Different doses (3, 10, and 30 mg/kg) of trime

Small Intestinal Bacterial Overgrowth Following Gastrointestinal Cancer Surgery: Current Evidence and Future Perspectives.

Small intestinal bacterial overgrowth (SIBO) has recently gained increasing attention as a potential cause of postoperative gastrointestinal symptoms following gastrointestinal cancer surgery. Surgical procedures can disrupt normal gastrointestinal physiology through anatomical reconstruction, impaired motility, reduced gastric acid secretion, vagal nerve injury, and alterations in the gut microbiota, thereby predisposing patients to bacterial overgrowth. Although positive breath tests for SIBO have frequently been reported after esophageal, gastric, and colorectal cancer surgery, the reported

Gastrointestinal radiographic features of human graft-vs.-host disease.

Following allogeneic bone marrow transplantation, graft-versus-host disease (GVHD) develops when the foreign donor lymphoid graft mounts an immunologic reaction against the skin, liver, and gastrointestinal tract of the host. The GVHD syndrome is characterized by skin rash, hepatocellular dysfunction, and secretory diarrhea with destruction of intestinal mucosa. The gastrointestinal radiographic manifestations of proven GVHD were studied in 16 allogeneic marrow recipients transplanted for aplasia (nine) or leukemia (seven). The radiographic findings revealed three distinct phases that correlat

Chicago Classification update (V4.0): Technical review on diagnostic criteria for ineffective esophageal motility and absent contractility.

Esophageal hypomotility disorders manifest with abnormal esophageal body contraction vigor, breaks in peristaltic integrity, or failure of peristalsis in the context of normal lower esophageal sphincter relaxation on esophageal high-resolution manometry (HRM). The Chicago Classification version 4.0 recognizes two hypomotility disorders, ineffective esophageal motility (IEM) and absent contractility, while fragmented peristalsis has been incorporated into the IEM definition. Updated criteria for ineffective swallows consist of weak esophageal body contraction vigor measured using distal contrac

[An experimental study of the rate and dynamics of the progression of different foods through the digestive tract of the greater gerbil].

The rate and dynamics of food movement over the digestive tract was studied with the use of solid markers and small fragments of colored plastic film, in five experimental groups of great gerbils kept on different diets (various combinations of concentrated and cellulose food). The concentrated food moves relatively uniformly over the digestive tract and is fully evacuated within 24-25 h. Addition of various plant objects rich in cellulose to the grain food and feeding of the gerbil on the cellulose food alone lengthened the period of food evacuation up to 50 h. This is due to food differentia

Large Peritoneal Macrophages Play No Role in the Pathogenesis of Postoperative Ileus Induced by Intestinal Manipulation.

INTRODUCTION: Postoperative ileus (POI) is an iatrogenic disorder marked by temporary impaired gastrointestinal (GI) motility post-abdominal surgery. Surgical handling of the intestine activates resident macrophages (Mfs), leading to inflammatory cytokine release and leukocyte recruitment into the muscularis, which compromises intestinal contractility. The mechanisms behind this activation are unclear. Recent studies suggest peritoneal Mfs, particularly large peritoneal macrophages (LPMs), might play a role in sterile intestinal inflammation by rapidly recruiting to the serosal layer of the gu

The Use of a PEG/Ascorbate Booster Following Standard Bowel Preparation Improves Visualization for Capsule Endoscopy in a Randomized, Controlled Study.

BACKGROUND/AIMS: The increasing use of capsule endoscopy (CE) to examine the gastrointestinal tract highlights the need to establish intestinal preparations that ensure optimal visualization while maximizing patient adherence. Thus, we assessed whether bowel preparation involving dietary restriction and a booster regimen produces adequate CE visualization in a real-world clinical setting. METHODS: We conducted a randomized, double-blind, prospective study of CE procedures at 2 tertiary-care centers. Patients were allocated to 3 groups: group 1 followed a clear liquid diet and fasting-based bo
