The aging cardiovascular and renal systems put the elderly patient at increased risk of end-organ damage from marked hypertension. Thus, the office-based physician needs to be skilled in making the diagnosis of a hypertensive urgency or emergency based on accurate blood pressure readings and an assessment of the heart, brain, retina, and kidney. Hypertension urgency and emergency are distinguished from each other by the clinical decision of how quickly the blood pressure must be lowered. The clinician has a wide variety of agents from which to choose for pharmacologic treatment, with the goal

The average age of patients with endocarditis is increasing. It is becoming a disease of the elderly, especially related to use of prosthetic heart valves and IV lines or catheters associated with bacteremias. Recognition is the major problem in the elderly patient since many features of the disorder are often attributed to aging. The diagnosis should be considered in the elderly patient with fever or other unexplained findings. Blood cultures are indicated to prevent misdiagnosis.

Most urodynamic tests currently in use in the evaluation of female urinary incontinence have not been applied to a community-based sample to determine their specificity. In this study of a random sample of noninstitutionalized elderly, 258 self-reported continent and 198 self-reported incontinent women sixty years and older, who participated in a household survey, underwent a clinic evaluation (history, physical examination, and urinalysis); of these 67 continent and 100 incontinent female respondents underwent urodynamic testing. The uroflowmetry, cystometry, and supine static urethral pressu

The authors review problems associated with prolonged intubation. Laryngeal stenosis and injuries caused by the endotracheal tube itself are described. Postintubation laryngotracheal pathology, including signs and symptoms and the management of specific lesions, is described in detail. Other injuries associated with prolonged intubation, such as nasal ulceration, sinusitis, and oral ulcers, also are reviewed.

The impact of chronic venous insufficiency on the activity and well-being of the elderly patient can be devastating. Acute complications such as cellulitis and venous ulcers often require extended hospitalization at a great cost to both patient and society. However, it is the long-term loss of ambulation that has the most profound effect on the elderly. It is clear that despite hundreds of medical papers the optimal management still has not been delineated. In general, early intervention to improve the hemodynamics before irreversible damage occurs is most beneficial. It is the responsibility

The American Board of Medical Specialties recently approved the concept of certificates of added qualifications in geriatrics within both internal medicine and family practice. Certification requirements have been worked out for each training model, and questions have quite naturally arisen addressing whether the developing family practice model is substantially different from the longer established internal medicine model. A survey was made of 12 family practice based geriatric fellowship programs. Program directors were asked how they felt family practice geriatrics differed from internal me…

Atherosclerotic occlusion of the renal arteries is the predominant cause of renovascular hypertension in the elderly. Medical therapy is appropriate when blood pressure is easily controlled and renal function remains stable. Percutaneous transluminal renal angioplasty is suitable for selected patients with non-ostial, unilateral lesions, but surgical revascularization currently offers the best interventive results for both hypertension and preservation of renal function in elderly patients with atherosclerotic renovascular disease.

Geriatric patients constitute a significant proportion of referrals to a psychiatric consultation-liaison service, accounting for 29% of referrals in the present study. Organic mental disorders, the most common psychiatric diagnosis, were identified in 51% of geriatric referrals. The next most common psychiatric conditions were affective disorders and adjustment disorders, each of which were diagnosed in 17% of geriatric referrals. Transfers to in-patient psychiatry were unusual and the most common recommendations were for psychotropic medications, further medical investigations, and for psych

Shoulder pain in the geriatric patient is a common presentation encountered by primary care physicians. Proper evaluation requires an understanding of the pertinent anatomy, a thorough physical examination, and a knowledge of common shoulder disorders that occur in this population. This article provides information needed to evaluate the geriatric patient with shoulder pain. Common causes of shoulder pain--both intrinsic and extrinsic--and their management will be reviewed.

The use of hypnotics in the elderly must be approached with special care in view of possible changes in pharmacokinetic behaviour and drug interactions in older patients. In a randomized, double-blind study in a large geriatric department, midazolam was compared with the barbiturate combination Vesparax with regard to efficacy and safety. The comparison of the efficacy of the 2 hypnotics showed them to be almost identical in this respect. Side-effects, however, were more frequent with Vesparax. Furthermore, in the Vesparax group, unlike in the midazolam group, a troublesome cumulation of effec

Little is known about the factors responsible for the institutionalization of the elderly. This study's objective was to discover markers associated with nursing home placement. Fifty-six white ambulatory women over 63 yrs of age were selected from 3 settings encompassing a range of dependency: 20 women from nursing homes; 16 women who lived in their own homes but requiring social assistance and 20 totally independent women. Each woman was examined for factors that might be associated with her state of dependency: age, socioeconomic status, morale, education, strength of social network, number

The high average life expectation in the civilized and industrial countries can be attributed to a considerable extent to the achievements of the medical sciences. However, medical research also has the task of providing the physiological basis which will ensure that the extra years do not mean just an extension of the period of senility but instead form a period valuable for the individual and society. In accordance with the common definitions, one expects a "geriatric agent" (geriatricum) to slow down the aging process or to diminish its effects. Such influences are usually referred to as re

When an elderly patient has an abnormal electrocardiogram but no other objective evidence of heart disease, the physician may question whether the ECG is really a reliable index of heart disease in old people or whether aging has some influence on which findings are normal and which are abnormal. The study reported here plus data gathered from an extensive review of the literature indicate the ECG criteria do not have to be changed for geriatric patients. Regardless of age, the prognosis of a specific abnormality remains that of the underlying disease. With age, both heart disease and abnormal

Geriatric care is a complex and challenging aspect of health care. The United Kingdom, with 13.6% of its population over 65 years of age, compared to 10.1% in the United States, has given special attention to this field. Characteristics of the British system include financing through the National Health Service, existence of a specialty of geriatrics, absence of "nursing homes," emphasis on home care, and avoidance of "heroics" for the terminally ill. While British life expectancy at birth is longer than that for Americans, the opposite is now true for those over 65. The British have made effo…
