Tissue Extraction in Gynecologic Surgery: Past, Present, and Future.

Fibroid tissue extraction during hysterectomy and myomectomy has become increasingly controversial. A wave of research has tried to clarify difficult questions around the prevalence of occult malignancies, the effect of morcellation on cancer outcomes, proper informed consent, and surgical options for tissue extraction. This review examines the history of these controversies and discusses tissue extraction techniques and continued areas of debate in the field.

Hemodynamic changes following accidental infiltration of a high dose of vasopressin.

Vasopressin local infiltration is useful in gynecological surgery because it can reduce hemorrhage. Depending on the activities of the sympathetic system and the renin-angiotensin system, reactions to vasopressin may differ and predicting its systemic effects is difficult. Because life-threatening complications can occur, infiltration with vasopressin should be administered with caution. A 42-year-old female patient was diagnosed with uterine leiomyomas. During a robot-assisted laparoscopic myomectomy, 50 U of vasopressin, which is ten-times the recommended dose, was accidentally infiltrated.

Gynecologists' perspectives on two types of uterus-preserving surgical repair of uterine descent; sacrospinous hysteropexy versus modified Manchester.

INTRODUCTION AND HYPOTHESIS: The modified Manchester (MM) and sacrospinous hysteropexy (SSH) are the most common uterus-preserving surgical procedures for uterine descent. Little is known about gynecologists' preferences regarding the two interventions. The study's aim was to identify which factors influence Dutch (uro)gynecologists when choosing one of these techniques. METHODS: This qualitative study consists of ten semi-structured interviews with Dutch (uro)gynecologists using predetermined, open explorative questions, based on a structured topic list. An inductive content analysis was per

The role of anticholinergic medication in the prevention of bradycardia during laparoscopic gynaecological surgery.

Bradycardia and cardiac arrest are recognised complications of laparoscopic gynaecological surgery. Anticholinergic agents are indicated in the treatment of perioperative bradyarrhythmia; however, their role in the prevention of bradycardia as a result of insufflation of the peritoneum is less well defined. This article systematically reviewed the literature with respect to the role of anticholinergic agents in the prevention of clinically significant bradyarrhythmia during laparoscopic gynaecological surgery. Eight studies were included for review following an extensive database search. This

Herlyn-Werner-Wunderlich syndrome with cervical atresia complicated by ovarian endometrioma: A case report.

Herlyn-Werner-Wunderlich syndrome (HWWs) is a rare congenital malformation that involves uterus dideslphys, obstructed hemivagina and ipsilateral renal agenesis. The obstructed vagina affects menstrual flow and causes related clinical features after menarche. Pelvic endometriosis is one of the common complications of HWWs. Resection of the vaginal septum can release the obstruction and result in good outcomes. However, in the case of cervical atresia, a rare variant of HWWs, ipsilateral hysterectomy is recommended because it is difficult to canalize cervical atresia surgically. Here we present

Abnormal uterine bleeding: managing endometrial dysfunction and leiomyomas.

Abnormal uterine bleeding refers to any change in the regularity, frequency, heaviness or length of menstruation. There are several potential causes for bleeding disturbance, the two most common being primary endometrial dysfunction and fibroids. Management of abnormal uterine bleeding involves both medical and surgical options and will largely depend on a patient's fertility plans. The use of levonorgestrel-releasing intrauterine devices for heavy menstrual bleeding is increasing in Australia, and they are considered first-line medical management for women accepting of hormonal therapies. Radical Trachelectomy Performed During Pregnancy: A Review of the Literature.

OBJECTIVE: Cervical cancer is one of the most frequent malignant diseases diagnosed during pregnancy. Abdominal or vaginal radical trachelectomies are fertility-preserving alternatives to radical hysterectomy for young women with early-stage cervical cancer that can be performed during ongoing pregnancy. METHODS: A literature review of articles on this subject was conducted through a Medline search for articles published in English or French. RESULTS: At this moment, 21 cervical cancer patients, including ours (4 stage IA2, 16 IB1, and 1 IB2) who underwent radical trachelectomy during pregna

Vesicouterine fistula: teaching video on diagnosis and surgical treatment.

INTRODUCTION: A 42-year-old woman presented with urinary incontinence 9 years after the last of four vaginal deliveries. She had also had one Caesarean section. Immediately after the last delivery, she presented with haematuria, which resolved within a few hours, but the drain remained prophylactically for 7 days. Nine years later, she was referred to a specialist hospital. METHOD: The patient presented with continuous urinary incontinence, and physical examination revealed a loss of urine from the vagina, the latter confirmed by a methylene blue test showing loss of urine from the uterine ce

Loop electrosurgical excision procedure: an effective, inexpensive, and durable teaching model.

The effectiveness of simulation training for enhancing operative skills is well established. Here we describe the construction of a simple, low-cost model for teaching the loop electrosurgical excision procedure. Composed of common materials such as polyvinyl chloride pipe and sausages, the simulation model, shown in the accompanying figure, can be easily reproduced by other training programs. In addition, we also present an instructional video that utilizes this model to review loop electrosurgical excision procedure techniques, highlighting important steps in the procedure and briefly addres

Prophylactic antibiotics for transcervical intrauterine procedures.

BACKGROUND: The transcervical intrauterine route is commonly used for operative gynaecological procedures in women. The vagina is an area of the body that is abundant with normal bacterial flora. An operative procedure through the vagina may, therefore, be considered to have added potential for post-procedure infection. Prophylactic antibiotics may play a role in the prevention of post-procedure transcervical intrauterine infections. OBJECTIVES: To assess the effectiveness and safety of antibiotic prophylaxis compared to placebo or no treatment in women undergoing transcervical intrauterine p

Surgical repair of bilateral levator ani muscles with ultrasound guidance.

Separation of the levator ani muscles from pubic bone is a common major levator trauma that may occur in vaginal delivery and is associated with pelvic floor dysfunctions. We describe a novel ultrasound-guided technique to repair these muscles. A 33-year-old woman presented with a history of difficult vaginal delivery and complaint of numbness and weakness of the vagina. In evaluation, bilateral levator defects were diagnosed by physical examination, three-dimensional endovaginal ultrasound, and magnetic resonance imaging. With ultrasound guidance the detached ends of muscles were tagged and s

Balloon cervicoplasty: a simplified technique for correction of isolated cervical atresia.

OBJECTIVES: To test the efficacy, safety and tolerability of a simplified surgical alternative for the formation of a new cervix in patients with isolated cervical aplasia. STUDY DESIGN: A case series of six patients with complete cervical atresia (four primary and two secondary) were included. A silicone Foley catheter sutured to the distal end of a sharp curved long inserter was inserted laparoscopically, and left in utero inflated and subject to downward traction for one menstrual cycle. Clinical and sonographic follow-up were done. The main outcome measures were operative time, complicati

Follow-up of dysfunctional bladder and rectum after surgery of a deep infiltrating rectovaginal endometriosis.

PURPOSE: The radical surgery of the deep infiltrating endometriosis of the rectovaginal septum and the uterosacral ligaments with or without bowel resection can cause a serious damage of the pelvic autonomic nerves with urinary retention and the need of self-catheterization. Major goal of this review article is to compare different surgical techniques of deep infiltrating endometriosis and their follow-up results. METHODS: The research strategy included the online search of databases [MEDLINE, EMBASE, SCOPUS] for the diagnosis of deep infiltrating endometriosis with the indication of an opera

Primary solitary fibrous tumor of the vulva: a case report.

BACKGROUND: Solitary fibrous tumor (SFT) of the female genital tract is an extremely rare neoplasm of mesenchymal origin. CASE: A case of primary SFT originated from the vulva of a 39-year-old woman. The tumor presented as a 10-cm, well-circumscribed lump and was composed of bland-looking cells admixed with thin and thick collagen fibers with the appearance of hemangiopericytoma. Immunohistochemically, the spindle cells were strongly positive for CD99 and vimentin, with focal positivity for CD34, and were negative for S100, SMA, desmin, ER, PR, etc. Microscopic evaluation and immunohistochemi
