A new suction cannula is described that is able to dissect and aspirate breast tissue. The cannula has proven effective in treating gynecomastia without surgical resection of breast tissue.

The article reports on operative plastic and reconstructive surgery in the management of gynaecomastia in adolescents. The disadvantages of an unsatisfactory early result after circumareolar approach and the uncertainty with regard to the possible late result are avoided by the approach presented here. The surgical management is described in detail.

Gynecomastia is a common deformity encountered among male patients. Surgery is primarily aimed at reducing the size of the breast mound and therefore should be considered aesthetic in nature. In an attempt to minimize the problems of hematoma formation, nipple necrosis, and unsightly scarring around the upper torso after the surgery, we have had the opportunity to utilize a circumareolar approach for breast tissue removal in 33 breasts (24 patients). A 70-year-old man had a breast tumor for 50 years. The tumor, which weighed 8.6 kg, proved to be a malignant cystosarcoma phyllodes. It was successfully removed by a simple mastectomy with low axillary dissection, and a skin graft was performed. The lesion was associated with gynecomastia, and it is believed to have represented a cystosarcomatous transformation of a giant fibroadenoma.
