This case report describes for the first time a manifestation of thrombotic thrombocytopenic purpura (TTP, Moschcowitz's disease) during pregnancy. The characteristic neurologic symptoms of TTP were missing, while the typical clinical signs of the HELLP syndrome could be observed. We report on a case of an HELLP syndrome in the 36th week of gestation in a primigravida with known uterus duplex cum vagina duplice, which under intensive medical control showed a spontaneous decrease of the elevated liver enzymes and normalisation of platelet count. 12 days after the occurrence of the HELLP syndrome, a Caesarean section was performed because of PROM and breech presentation of the foetus.

A case report is given on a 24 years old primipara who developed a progredient HELLP-syndrome (H = hemolysis, EL = elevated liver enzymes, LP = low platelet counts) in the 23rd week of gestation two days after first diagnosis of pregnancy induced hypertension. As the situation became increasingly critical, the pregnancy had to be terminated in the 23rd week of gestation by sectio parva. To our knowledge the development of a HELLP-syndrome at this early gestational age has not yet been described.

We describe the case of a 31-year-old woman who underwent a section caesarean and 24 hours later a laparotomy for treatment of a ruptured subcapsular liver hematoma due to a HELLP syndrome.
