Fracture of the ulnar sesamoid of the metacarpophalangeal joint of the thumb: a case report.

Fracture of the thumb sesamoid bones is rare. We report here a fracture of the ulnar sesamoid in a 25-year-old man caused by a fall on his outstretched hand. He complained of pain and loss of thumb motion. Physical examination revealed tenderness, swelling, and a hematoma over the first metacarpal joint of the right thumb. Radiographs showed a fracture of the ulnar sesamoid which should not be confused with normal variants that occasionally arise from ossifications centers that fail to unite.

The Mitek Mini GII anchor introduced for tendon reinsertion in the hand.

The Mitek Mini GII anchor is showcased as a relatively new device that provides a rapid, simple method for bony reattachment; here, it is introduced for the fixation of flexor or extensor tendons at their insertion. Transosseous tunnels with limited visualization, pull-out sutures, and external devices can now be avoided if desired. Applications for soft-tissue fixation should in no way be limited to the small bones of the hand. Expense of the apparatus remains its only major drawback.

Case report: Erdheim-Chester disease: polyostotic sclerosing histiocytosis.

Erdheim-Chester disease is characterised by diffuse metaphyseal and diaphyseal sclerosis of long tubular bones of the appendicular skeleton. In addition, there is an inconstant tendency to visceral involvement by lipo-granulomatous infiltration. Histologically, this condition has marked similarities to Hand-Schuller-Christian disease. A further case is presented which supports the view that Erdheim-Chester disease is part of the spectrum of histiocytoses and not a distinct pathological entity. Polyostotic sclerosing histiocytosis would be an appropriate alternative label for this condition.

[Diaphyseal and articular fractures of the growing skeleton of the hand].

The closed as well as the operative treatment of fractures of the skeleton of the hand during growth must take into consideration the growth potential of the bones. It is particularly important in its potential to correct shortening as well as axial deviation and angulation (except rotational deformity). There are, however, limits to this potential as shown in epiphyseal fractures. Modern internal fixation has, therefore, an important -- though very restricted -- place in the treatment of these epiphyseal fractures. Other rare indications are mentioned.

Curettage without bone grafting for a simple bone cyst in the capitate.

PURPOSE: To review the outcome after intralesional curettage without bone grafting for simple bone cysts in the capitate. METHODS: Records of 12 hands in 10 consecutive females aged 14 to 21 (mean, 17.5) years who underwent intralesional curettage without bone grafting for a simple bone cyst in the capitate were reviewed. Clinical outcome was evaluated using the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire. Wrist/hand pain was assessed using the modified Mayo wrist score. Patient satisfaction, range of motion, and grip strength were self-evaluated as improved, unchanged, or

Lower hand grip in rheumatoid arthritis patients is associated with low finite element analysis using high resolution peripheral quantitative computed tomography scan of the 2nd metacarpophalangeal joint.

AIM: To evaluate hand function by hand grip test in rheumatoid arthritis (RA) patients, and its association with bone erosions and the estimated bone strength (finite element - FE analysis) through the analysis of the 2nd metacarpal head of the dominant hand using high resolution peripheral quantitative computed tomography (HR-pQCT). METHOD: Eighty-two female RA patients between 18-50 years old were selected. Demographic data, Health Questionnaire Assessment Disability Index (HAQ), Disease Activity Score of 28 joints (DAS)-28, simplified disease activity index (SDAI) and the hand grip test we

Synpolydactyly of the foot in homozygotes.

In 2002, we reinvestigated a large synpolydactyly kindred first described in 1995. It was found to have expanded with an increase in number of homozygous offspring. These homozygotes had severe hypoplasia, with synpolydactyly of their hands and feet. We present the clinical, genetic, and surgical findings of this deformity and the histologic findings of the removed bones of the heterozygous and homozygous members. There were 125 affected individuals (113 heterozygotes and 12 homozygotes) of 245 members of the past five generations. We identified seven marriages in which both spouses were affec

Bizarre parosteal osteochondromatous proliferation (Nora's lesion): a retrospective study of 12 cases, 2 arising in long bones.

Twelve cases of bizarre parosteal osteochondromatous proliferation (BPOP), also known as Nora's lesion, are reported. Ten lesions were located in the small bones of the hands, and 2 were located in long bones (femur and proximal tibia). Patient age ranged from 12 to 63 years (average, 30.3 years). Radiography of the lesions in the hand bones showed calcific masses attached to the underlying cortex, without interruption of the latter. The long bone lesions revealed unusual findings. In the femur, BPOP presented with extensive cortical destruction and was suggestive of a malignant lesion. This p

Clinico-radiological Correlation of Bone Changes in Leprosy Patients Presenting with Disabilities/Deformities.

Leprosy is a medical - social disease, it is associated with stigma in the society due to the resulting deformities in some persons. Although stigma has decreased after the widespread use of MDT, some disabilities do occur which are mostly due to late initiation of treatment and inappropriate care. Besides the nerve and skin involvement bone changes have been reported to be common in leprosy. These bony changes need to be understood in the present MDT era specially in the context of clinical spectrum and duration of disease/ deformities. Fifty clinically diagnosed and histologically classified

Hand images: normal and abnormal.

Supplemental hand scintigrams with abnormal features were obtained from 29% of patients (134 of 463) who were referred for routine minified bone imaging with 99mTc-Sn-polyphosphate. A wide spectrum of normal activity distribution ranging from well-defined to "wash-out" images is described in 329 cases (71%). In the abnormal images of the joints and individual bones, the changes--although not always characteristic of some particular disease--may often suggest a diagnosis and/or its pathophysiologic status. The joints with heavy uptake correlate well with the presence of active clinical findings

Correlation between hand/wrist and panoramic radiographs in severe secondary hyperparathyroidism.

OBJECTIVES: Hand/wrist and dental radiographs are important for osteoporosis analysis in secondary hyperparathyroidism (SHPT). This study evaluated whether a correlation exists between the effects of the disease on the hands and jaws, and investigated the association between osteoporosis progression in the hands and parathyroid hormone (PTH) levels in chronic kidney disease (CKD) patients. MATERIALS AND METHODS: Four panoramic radiographic parameters (mental index, mandibular cortical index, trabecular bone pattern, and calcification/resorption) and four corresponding hand/wrist radiographic

Magnification immersion radiography: better soft tissue visualization in the hands.

We have combined microfocal magnification with immersion radiography and mammographic technique to produce superb soft tissue and bone detail in the hand using a single PA exposure. The lateral projections provide excellent demonstration of the flexor and extensor tendons and joint capsules. This method has proved valuable as the primary radiologic examination in the evaluation of soft tissue lesions in the hands, particularly in patients suspected of having rheumatoid arthritis.

Three different methods for treating multiple enchondromatosis in one hand.

Ollier's disease remains comparatively rare, and is a non-hereditary cartilage dysplasia of bone. It is usually associated with problems such as deformity and fracture. Three different methods were used in a one-hand of 15-year-old boy reporting his pain in the left hand and swellings. After the curettage of tumor, regarding as the differences of all parts of the bone structure reconstruction in the patient's hand, we chose three following methods for this boy, i.e. fixed by the locking plate with calcium phosphate cement, filled with allograft bone, curetted the tumor without any bone graft.

The management of dog bites and dog bite infections to the hand.

Dog bites account for approximately one of 200 emergency room visits. The majority of bites are from dogs known to the victim. Due to the numerous small compartments and the thin covering of soft tissue over the bones and joints, bite wounds to the hand are more likely to become infected that are bites to the arm, leg, or face. Improper management can result in significant morbidity and prolonged treatment. This article discusses the microbiology of dog bite wounds, antibiotic selection, and proper wound management.

[Fresh fractures! Classification, management, results and problems: the wrist joint].

Carpal bone fractures are rare, with an incidence of 13% compared to that of fractures and luxations of the hand bones. Generally they are intraarticular fractures. Fracture of the scaphoid bone occurs most frequently, with an incidence of 75%. The greater number of fractures is found in the middle-third of the wrist bordering on the os lunatum. They are usually treated conservatively by splinting with a cast. Surgery is indicated only when the fracture is open, instable, dislocated or combined with carpal instabilities.

[Pachydermoperiostosis (Touraine-Solente-Golé syndrome). Clinical and radiological differential diagnosis (author's transl)].

This report describes a patient with pachydermoperiostosis (Touraine-Solente-Golé-syndrome). This disorder is characterized by clubbing of the terminal phalanges with watchglass-like nails, enlargement of hands and feet, pachydermia and periostotic skeletal changes. The signs of pachydermia are cutis verticis et frontis gyrata, ptosis and increased skin-thickness of the palms of the hand and soles of the feet. Frequently, a hyperhidrosis of the hands and feet can be found. The differential diagnosis of clinical symptoms and changes in the bones is discussed in detail.

[Instability of the wrist joint--diagnosis and therapy].

Wrist-joint instability can be caused by injuries to ligaments or bones, it can be intermittent or permanent. Clinical symptoms and pain sensations are non-characteristic. The X-ray findings are the clue to the diagnosis. In this article recommendations are given for exact X-ray positions of the hand and their evaluation. The classification of instability is corresponding to the X-ray findings. Further investigations like radiographs under stress, dynamic investigations or the arthrography of the wrist-joint can be helpful to find out the exact pathologic changes. They are variable and are ran

[Hand for the dermatologist].

Through its anatomical, topographical, and functional distinctiveness, more than all other parts of the body, the hand is the interface between humans and their environment. All types of stimulus produce cutaneous signs in the hands, notably cold, light, pressure, contact, etc. Exposure to light makes it the region where photodermatosis is expressed, but also a particular site for cutaneous carcinogenesis. The countless chemical substances that the hand encounters can create specific or particular diseases in this location (irritative dermitis, contact eczema, hyperkeratosis, atrophy, etc.). T
