The modes of spread of fungal infection on the nails are discussed stressing the essential importance of the knowledge of this mode for finding of effective treatment of ungual trichophytosis. On the basis of own observations and reports of other authors combined operative and pharmacological treatment of trichophytosis is discussed and its usefulness is confirmed by the results of treatment and investigations. Fungal nail infections in children and adolescents are on the rise worldwide. Diagnosis and, above all, treatment is a challenge. A 15-year-old girl had been suffering from onychomycosis of the toenails for more than 6 years, and later also of a fingernail on her right hand. Skin reactions related to secukinumab are uncommon. Although the initial phase 3 studies of this medication reported infection and urticaria as adverse cutaneous events, other cases of unique adverse events have since been reported. We are presenting a patient who developed an exuberant hand and foot reaction after starting secukinumab.

Both the metabolism and dietary intake of vitamins and minerals are essential to homeostatic function in the body. Dietary excess or deficiency, as well as genetic and acquired disorders in metabolism, can present dermatologically and systemically. More specifically, disorders in metabolism of zinc, biotin, essential fatty acids, and vitamin B, can appear with acrally distributed dermatoses. Recognition of the dermatologic manifestations associated with nutritional disorders is important for early diagnosis and management.
