Tackling physical inactivity and inequalities: implementing a whole systems approach to transform community provision for disabled people and people with long-term health conditions.

BACKGROUND: Physical inactivity is a global public health priority. There are known health and well-being consequences of being inactive, and the benefits of being physically active are well established. However, there are persistent inequalities when it comes to how physically active people are, with disabled people, people living with long-term health conditions, and people residing in areas of socio-economic deprivation being particularly affected. Methods such as whole system approaches (WSAs), which are dynamic, multifaceted, and engage all relevant stakeholders, have gained momentum as a

Life at the State summit: views and experiences of 18 psychiatric leaders.

In 1981 the American Psychiatric Association appointed an ad hoc committee to investigate the decline of psychiatrist leadership of public mental health programs. As part of the committee's research, the 18 psychiatrists who were then state commissioners or directors of mental health were surveyed on how they got their jobs and on their backgrounds, present responsibilities, support systems, and future plans. The results of the survey are discussed and presented in tabular format. The psychiatrists surveyed recommended that future commissioners have backgrounds similar to their own, which cons

Help-Seeking Among Women Survivors of Intimate Partner Violence in Thailand: A Qualitative Research Study.

Limited evidence exists on help-seeking, access, and use of support services and resources by survivors of intimate partner violence (IPV) in Thailand. This study uses qualitative descriptive methods to explore help-seeking behaviors of adult (18 years and older) Thai women who reported IPV in the past 6 months. Twenty-seven women were recruited through social media and consented to participate in a confidential interview conducted via telephone. The interview data was analyzed using content analysis. Findings found that survivors' perceptions of help-seeking varied, with participants recogniz

A qualitative exploration of contraceptive use and discontinuation among women with an unmet need for modern contraception in Kenya.

BACKGROUND: Addressing the unmet need for modern contraception underpins the goal of all family planning and contraception programs. Contraceptive discontinuation among those in need of a method hinders the attainment of the fertility desires of women, which may result in unintended pregnancies. This paper presents experiences of contraceptive use, reasons for discontinuation, and future intentions to use modern contraceptives. METHODS: Qualitative data were collected in two rural counties in Kenya in 2019 from women with unmet need for contraception who were former modern contraceptive users

Controlling hepatitis C in Rwanda: a framework for a national response.

With the introduction of direct-acting antiviral drugs, treatment of hepatitis C is both highly effective and tolerable. Access to treatment for patients, however, remains limited in low- and middle-income countries due to the lack of supportive health infrastructure and the high cost of treatment. Poorer countries are being encouraged by international bodies to organize public health responses that would facilitate the roll-out of care and treatment on a national scale. Yet few countries have documented formal plans and policies. Here, we outline the approach taken in Rwanda to a public healt

Blue-green spaces, heat stress, and health-supportive public-space use under urban stress: machine learning and SEM evidence from jogging flow in Fuzhou, China.

Urban blue-green spaces (BGS) are increasingly discussed in relation to health-oriented and resilience-oriented urban planning, particularly in high-density cities exposed to heat-related environmental stress. However, empirical evidence remains limited on how BGS and heat stress are associated with everyday public-space use. Using Fuzhou, China, as a case study, we integrated crowdsourced jogging data, BGS indicators, land surface temperature (LST), and urban development variables to analyze jogging flow as a behavioral proxy for health-supportive public-space use. Machine learning (ML) and s

Gold standards framework implementation in an acute hospital: end-of-life outcomes.

OBJECTIVES: Hospitals remain the most common place of death in the UK, with 30% of inpatients in their last year of life. National policies emphasise early identification and proactive planning to support patients to die in their preferred place. The Dudley Group National Health Service (NHS) Foundation Trust implemented the Gold Standards Framework (GSF) to identify patients nearing the end of life and guide proactive care. This study evaluated whether GSF coding influenced outcomes, specifically advance care planning (ACP) and place of death. METHODS: A retrospective review was conducted of

Which Behavior Change Techniques May Help Waterpipe Smokers to Quit? An Expert Consensus Using a Modified Delphi Technique.

INTRODUCTION: Waterpipe smoking is addictive and harmful. The determinants of waterpipe smoking may differ from those of cigarette smoking; therefore, behavioral approaches to support quitting may also differ between these two tobacco products. While some evidence exists on effective behavioral change techniques (BCTs) to facilitate cigarette smoking cessation, there is little research on waterpipe smoking cessation. METHODS: Twenty-four experts were selected from the author lists of peer-reviewed, randomized controlled trials on waterpipe smoking cessation. They were invited to two rounds of

Unraveling reproductive and maternal health challenges of women living with HIV/AIDS in Vietnam: a qualitative study.

BACKGROUND: Human Immunodeficiency Virus (HIV) remains a significant public health concern worldwide. Women living with HIV/AIDS (WLHA) have the additional and unique need to seek sexual and reproductive health services. WLHA's maternal health journeys can be shaped by the cultural norms and resources that exist in their society. This study sought to understand if and how WLHA's family planning, pregnancy, and motherhood experiences could be influenced by the patriarchal culture, gender roles, and HIV stigma in Vietnam, specifically. METHODS: Between December 2021 and March 2022, 30 WLHA with

The World Health Organization's global strategy for prevention and assessment of HIV drug resistance.

Antiretroviral treatment (ART) for HIV is being scaled up rapidly in resource-limited countries. Treatment options are simplified and standardized, generally with one potent first-line regimen and one potent alternate first-line regimen recommended. Widespread HIV drug resistance (HIVDR) was initially feared, but reports from resource-limited countries suggest that initial ART programmes are as effective as in resource-rich countries, which should limit HIV drug resistance if programme effectiveness continues during scale-up. ART interruptions must be minimized to maintain viral suppression on

A Survey of State Correctional Health Care Providers on Advance Care Planning: Opportunity for Collaboration With Corrections.

Prison populations are rapidly aging. Persons in prison age quicker and suffer more chronic illness and disability than their nonincarcerated peers, posing challenges to caring for prisoners who are chronically ill and dying. The goal of our study was to describe state prisons' practices and policies addressing persons in prison with advanced chronic and life limiting illness through a national web-based survey of state-level prison health care professionals. In particular, we focused on advanced care planning, use of health care directives, decision-making about goals of care, including life
