Enkephalin effects on blood pressure, heart rate, and baroreceptor reflex.

The cardiovascular effects of opioid peptides have been studied. Leucine-enkephalin (Leu-ENK) produced blood pressure (BP) increases following administration into the lateral brain ventricles (i.v.t.), into the cisterna magna (i.c.i.), and following intravenous (i.v.) administration. Heart rate (HR) increases were observed following all routes of administration (threshold for BP and HR effects at 0.3 nmole, maximum at 360 nmoles). The cardiovascular effects were independent of generalized seizures, which may occur at higher doses of enkephalins (ENK). D-alanine-enkephalin (D-Ala-ENK) attenuate…

Blood pressure and heart rate and withdrawal of antihypertensive drugs.

The immediate effects on heart rate and blood pressure of withdrawing antihypertensive drugs were studied over three-day periods in 26 patients. Four groups of drugs were studied. After withdrawal all patients taking clonidine showed a considerable increase in heart rate and blood pressure with intense ectopic activity. Patients taking postganglionic neurone-blocking drugs showed a similar but less pronounced reaction with increased ventricular ectopic activity. No alarming reactions were seen after withdrawal of methyldopa or beta-blocking drugs. Methyldopa and, especially, beta-blocking drug…

Elevated heart rate is associated with increased mortality and rejection in pediatric heart transplant recipients.

PURPOSE: Elevated heart rate is associated with adverse outcomes in patients with heart failure. Heart transplant recipients often have elevated heart rate; however, the long-term associations of elevated heart rate with clinical outcomes are unknown. METHODS: The records of all pediatric (≤21 years) heart transplant recipients at our institution were reviewed. Patients who died or lacked a clinic visit within 1 year following heart transplant were excluded, as were those with a permanent pacemaker. Heart rate was collected from outpatient electrocardiograms and converted to Z-scores using ag…

Heart Rate Variability and Heart Failure with Reduced Ejection Fraction: A Systematic Review of Literature.

INTRODUCTION: Autonomic impairment is a hallmark of heart failure with reduced ejection fraction (HFrEF). While there have been studies on general values for each index of heart rate variability (HRV) analysis in HFrEF, a systematic review comprehensively examining representative values in HFrEF is lacking. METHODS: We searched PubMed, Embase, and Cochrane databases to extract studies reporting representative values of HRV metrics in HFrEF. RESULTS: A total of 470 HFrEF patients from 6 studies were included in the review. In general, time and frequency domains were abnormally lower in HFrEF,…

Discharge heart rate and 1-year clinical outcomes in heart failure patients with atrial fibrillation.

BACKGROUND: The association between heart rate and 1-year clinical outcomes in heart failure (HF) patients with atrial fibrillation (AF), and whether this association depends on left ventricular ejection fraction (LVEF), are unclear. We investigated the relationship between discharge heart rate and 1-year clinical outcomes after discharge among hospitalized HF patients with AF, and further explored this association that differ by LVEF level. METHODS: In this analysis, we enrolled 1760 hospitalized HF patients with AF from the China Patient-centered Evaluative Assessment of Cardiac Events Pros…

Resting heart rate in ambulatory heart failure with reduced ejection fraction treated with beta-blockers.

AIMS: Current guidelines recommend beta-blocker therapy in chronic heart failure with reduced ejection fraction (HFrEF) titrated according to tolerated target dose. The efficiency of this strategy to obtain adequate heart rate (HR) control remains unclear in clinical practice. The aim of this study was to determine, in a real-world setting, the proportion of HFrEF patients who fail to achieve beta-blocker target doses, whether target doses of beta-blockers have a relationship with the adequacy in reducing resting HR over time. METHODS AND RESULTS: Beta-blocker dose and resting HR of consecuti…

Skeletal muscle electrical stimulation improves baroreflex sensitivity and heart rate variability in heart failure rats.

The goal of the current study was to evaluate the effects of electrical stimulation (ES) on the arterial baroreflex sensitivity (BRS) and ca … pared to the P-Sham group and the ES-CHF group compared to the P-CHF group. ES was able to decrease heart rate sympatho-vagal modulation and peripheral sympathetic modulation in ES-CHF compared to P-CHF group. Interestingly, heart rate sympatho-vagal modulation was similar between ES-CHF and P-Sham groups. Thus, ES enhances heart rate parasympathetic modulat …

Resting heart rate and the risk of heart failure in healthy adults: the Rotterdam Study.

BACKGROUND: An elevated resting heart rate is associated with rehospitalization for heart failure and is a modifiable risk factor in heart failure patients. We aimed to examine the association between resting heart rate and incident heart failure in a population-based cohort study of healthy adults without pre-existing overt heart disease. METHODS AND RESULTS: We studied 4768 men and women aged ≥55 years from the population-based Rotterdam Study. We excluded participants with prevalent heart failure, coronary heart disease, pacemaker, atrial fibrillation, atrioventricular block, and those usi…

Impaired heart rate responses and exercise capacity in heart failure patients with paced baseline rhythms.

BACKGROUND: Conduction system disease and beta-blocker therapy are both common among heart failure (HF) patients and contribute to increasing reliance on paced rhythms. We hypothesized that many HF patients dependent on pacing have suboptimal heart rate responses and associated limitations in exercise capacity. METHODS AND RESULTS: We studied 122 HF patients (left ventricular ejection fractions ≤40%) referred for cardiopulmonary exercise testing, comparing those with pacing at baseline with those with native rhythms. The paced group (PG) had lower resting (71 ± 9 vs 75 ± 15 beats/min; P = .04…

Influence of extent of muscle shortening and heart rate on work from frog heart trabeculae.

Shortening, lengthening, and net work done by frog (Rana pipiens) heart trabeculae were measured over a range of strain amplitudes (length change) and cycle frequencies. Net work, the product of muscle strain and force over a full lengthening/shortening cycle, increased with strain to strains well over 25% of the muscle's rest length, a value greater than optimum strains reported for most skeletal muscles. A distinct optimum strain for net work was not found. Maximum net work per cycle averaged 7.5 J/kg for ventricular muscle and 2.0 J/kg for atrial muscle. Isometric twitch stress was maximal

Sweat lactate threshold heart rate monitoring based on multi-parameter electrochemical sensing and lactate excretion rate analysis.

Lactate is an important biomarker for assessing anaerobic metabolism and exercise intensity, and its continuous non-invasive monitoring is v … the limitation of using concentration alone. Human studies showed that the sweat lactate threshold heart rate (SLTHR), corrected by sweat rate, agreed closely with the blood lactate threshold, with a maximum deviation of 3 bpm. The system also tracked changes in lactate threshold over multiple days of training and captured an increase in threshold heart rat …

Combined Impact of Heart Rate Sensor Placements with Respiratory Rate and Minute Ventilation on Oxygen Uptake Prediction.

Oxygen uptake (V˙O2) is an essential metric for evaluating cardiopulmonary health and athletic performance, which can barely be directly measured. Heart rate (HR) is a prominent physiological indicator correlated with V˙O2 and is often used for indirect V˙O2 prediction. This study investigates the impact of HR placement on V˙O2 prediction accuracy by analyzing HR data combined with the respiratory rate (RESP) and minute ventilation (V˙E) from three anatomical locations: the chest; arm; and wrist. Twenty-eight healthy adults participated in incremental and constant workload cycling tests at var…
