Endoscopic plantar fascia release, calcaneal drilling and calcaneal spur removal for management of painful heel syndrome.

BACKGROUND: The pathogenesis of painful heel syndrome is multifactorial including plantar fasciitis, increased intra-osseous pressure of the os calcis, calcaneal periostitis and presence of calcaneal spur. The currently used endoscopic treatment of painful heel syndromes involves endoscopic plantar fascia release alone without addressing other pathological changes. OBJECTIVES: To evaluate the clinical outcome of endoscopic plantar fascia release, calcaneal drilling and calcaneal spur removal. METHODS: The study was conducted on 22 cases/24 feet with idiopathic painful heel syndrome resistant…

Gait improvement of hemiplegic patients using an ankle-foot orthosis with assistance of heel rocker function.

The ankle-foot orthosis (AFO) with oil damper, GaitSolution (GS), was previously developed to make heel rocker function possible during the gait of hemiplegic patients. To clarify the characteristics of patients who show adaptation to GS use, a questionnaire was completed by the clinicians responsible for 99 patients with hemiplegia who participated in this study. Clinicians completed items concerning gait data of patients without AFO use and with GS use, adaptation to the use of GS, and patient opinion of GS use. Approximately 60% of patients achieved heel rocker function during the loading r…

Magnetic resonance osteodensitometry in human heel bones: correlation with quantitative computed tomography using different measuring parameters.

RATIONALE AND OBJECTIVES: Density of trabecular bone structures in human heel bones was assessed by 3D magnetic resonance (MR) gradient echo imaging (GEI) with multiple echoes. Different spatial resolutions were applied to investigate the influence of the pixel size on signal characteristics in GEI and to find suitable measuring parameters for a maximum correlation between GEI and bone mineral density obtained by quantitative computed tomography (QCT). METHODS: Thirty-five patients aged 31 to 65 years with suspected osteoporosis underwent MR and QCT examinations of the heel bones. The MR prot…

Long-term follow-up of a patient with lateral calcaneal flaps for bilateral posterior heel necrosis.

Lateral calcaneal flaps were used to cover the skin necrosis of bilateral posterior heels, and a favorable result was obtained. During the first 5 years after surgery, the patient enjoyed hiking as before and made favorable progress, having no trouble in the region covered with the flap and attaining sensory cortical reorientation soon after surgery. Paresthesia of donor sites was reduced in its extent and caused no difficulties in everyday life, but it remained after the lapse of 5 years. The medical record of this patient during the 5 years is reviewed, and we discuss the indications of a la…

Heel movement and foot biomechanics in aluminium nail-on and indirect glue-on fabric cuff shoes in horses.

BACKGROUND: Heel movement is critical to shock absorption and vascular perfusion of the equine digit. Conventional horseshoeing techniques are suspected to restrict natural hoof deformation, affecting hoof biomechanics. Several studies have explored the effects of shoeing on hoof function, though many have been limited to artificial conditions, cadavers, or homogenous populations. OBJECTIVE: We aimed to compare the effects of two aluminium shoeing techniques, nail-on versus indirect glue-on fabric cuff shoes, on heel movement in horses. Our objective was to determine which technique allowed g…

A Traumatic Loss of the Calcaneus and Heel Pad From a Lawnmower Injury in a Pediatric Patient: A Case Report.

Traumatic calcaneal injuries with combined soft tissue loss are extremely rare in pediatric patients, posing significant reconstructive challenges due to contamination and the need to preserve growth potential. We report a case of a 10-year-old female who sustained a lawnmower-induced degloving injury with partial calcaneal loss, plantar heel pad avulsion, and posterior calf soft tissue damage. Management included emergent debridement followed by staged reconstruction aimed at preserving the distal calcaneal growth plate. At 11 months post-injury, the patient achieved complete wound closure, i…

Modelling and Simulation of Soft Tissue Loading Reduction at the Posterior Heel by a Moisture-Responsive Prophylactic Dressing.

Pressure ulcers/injuries at the posterior heel commonly develop in patients with limited mobility or sensation, where movements such as sliding in bed generate shear forces that elevate the deep soft tissue stresses. Prophylactic multilayer dressings are recommended to reduce this injury risk, yet many current products incorporate high-friction silicone adhesives for attaching the dressing to the skin, which may increase shear transfer to the skin and subdermally while a patient is moving or being moved. This study evaluated a multilayer dressing featuring an Aquacel Hydrofiber Technology skin…

Morphological characteristics of the heel fat pad in the dominant and non-dominant feet under varying loads.

[Purpose] Morphological changes in the dual-layered structure of the heel fat pad under varying loads, differences between the dominant and non-dominant feet, and changes over one month are not understood. This study aimed to examine these factors, as understanding normal conditions provides insights into identifying abnormal conditions. [Participants and Methods] Forty healthy Japanese adults (80 feet) participated in this study. The heel fat pad was divided into a macrochamber layer (extending from the calcaneal tuberosity to the fibrous septum, including the macrochambers) and a microchambe…

The CHANSE score: a novel clinical tool incorporating the heel drop test for the diagnosis of pediatric appendicitis.

BACKGROUND: Acute appendicitis is one of the most common abdominal surgical emergencies in children presenting to the emergency department. Although clinical guidelines emphasize combining symptom characteristics, physical examination findings, and inflammatory markers for accurate diagnosis, pediatric presentations are often atypical and overlap with other conditions. Classic physical signs such as the obturator, psoas, and Rovsing's signs have limited predictive value. The heel drop test has been reported to outperform rebound tenderness in detecting intraperitoneal inflammation, as it is mo…

The association of demographic, psychological, social and activity factors with foot health in people with plantar heel pain.

BACKGROUND: Plantar Heel Pain (PHP) can be a debilitating musculoskeletal condition from which only 50% recover within a year due to poor understanding of the mechanisms explaining severity and predicting outcomes specific to PHP. OBJECTIVE: To explore associations between biopsychosocial variables and the severity of people with PHP. Secondly, to determine what combination of self-reported factors distinguishes people with PHP from other foot pain (OFP). METHODS: We collected data from 235 participants, including 135 (%57) PHP (age 44 ± 12 years, 66% female) and 99 OFP (%43) (age 38 ± 11 ye…

Sinogram: A Valuable Diagnostic Tool for an Adult Male With Discharging Heel Ulcer and Suspected Calcaneal Osteomyelitis.

This report highlights the clinical significance of a sinogram in diagnosing osteomyelitis in resource-poor areas. We report a case in which a sinogram was used successfully for the diagnosis of calcaneal osteomyelitis. A 25-year-old male patient sought medical attention for persistent pain in the right ankle joint and heel, accompanied by a discharging ulcer over the right heel. He had a history of foot trauma involving vegetative matter sustained during a farming injury one month prior to the onset of symptoms. An ankle radiograph revealed an osteolytic lesion involving the calcaneum, prompt…

Midfoot and ankle motion during heel rise and gait are related in people with diabetes and peripheral neuropathy.

BACKGROUND: Midfoot and ankle movement dysfunction in people with diabetes mellitus and peripheral neuropathy (DMPN) is associated with midfoot deformity and increased plantar pressures during gait. If midfoot and ankle motion during heel rise and push-off of gait have similar mechanics, heel rise performance could be a clinically feasible way to identify abnormal midfoot and ankle function during gait. RESEARCH QUESTION: Is midfoot and ankle joint motion during a heel rise associated with midfoot and ankle motion at push-off during gait in people with DMPN? METHODS: Sixty adults with DMPN c…

The heel amelanotic melanoma, a rare subtype of skin cancer misdiagnosed as foot ulcer: A case report.

Amelanotic melanoma (AM) of the heel is a very rare subtype of malignant melanoma in which the tumour cells, unlike other types of melanoma, are characterised by little or no melanin pigmentation. AM resembles many benign dermatological complications that often lead to late diagnosis of lesions, poor prognosis, and occasionally misdiagnosis at an early stage of the disease. We report a case of a 73-year-old man with a heel ulcer who was admitted to Al-Zahra Hospital (Isfahan, Iran). Chronic osteomyelitis was considered the primary diagnosis, and several courses of antibiotics were prescribed f…

Evaluation of the effectiveness of vibroacoustic therapy treatment of patients with so-called "heel spur". A preliminary report.

BACKGROUND: The so-called "heel spur" is a radiological term referring to adaptive bone growth as a result of chronic overload enthesopathy of the proximal attachment of the plantar fascia. The main cause of the pain is continued localised pressure on the surrounding soft tissues. Vibroacoustic wave therapy is a relatively new method gaining popularity among doctors, physiotherapists and patients. The aim of this study was to confirm the clinical efficacy of vibroacoustic therapy compared to laser and ultrasound therapy. MATERIAL AND METHODS: The study enrolled 60 patients treated for plantar…

Granular cell tumor of the posterior tibial nerve as a rare cause of heel pain: a case report.

Granular cell tumor of peripheral nerves is extremely rare. We present the case of a patient with a well-capsulated intraneural granular cell tumor involving the posterior tibial nerve, who presented with chronic heel pain mimicking plantar fasciitis. Magnetic resonance imaging revealed a well-defined intraneural soft-tissue mass within the substance of the posterior tibial nerve. Histopathologic examination showed a granular cell tumor, which is extremely rare in the peripheral nerves. Heel pain is one of the common conditions handled by physicians, podiatrists, and orthopedic surgeons. Poste…
