Acetaminophen toxicosis in the cat.

Administration by the owner of three 325-mg (5-gr) tablets of acetaminophen (N-acetyl-p-aminophenol) to each of 2 adult Burmese cats was associated with severe illness of both cats and death of one. Administration of two 325-mg tablets to each of 2 experimental adult cats resulted in severe illness. Marked cyanosis was observed in experimental cats within 4 hours after administration of one 325-mg tablet. Cyanosis was apparently due to anoxia associated with conversion of hemoglobin to methemoglobin by acetaminophen or its metabolites. Anemia, hemoglobinuria, and icterus were subsequently obse…

Methemoglobinemia Secondary to a Traditional Healing Practice Using Mothballs: A Need of Pediatric Vigilance.

Acute-onset unexplained hypoxemia persisting despite 100% oxygen has a limited differential diagnosis but poses a challenging diagnostic dilemma. Methemoglobinemia, a hemolytic condition, may lead to significant complications if it goes undiagnosed during the critical golden hour of an emergency department (ED) presentation. This case report presents the clinical details of a 30-month-old child with acute intravascular hemolysis evident by severe pallor and hemoglobinuria and severe hypoxia documented on pulse oximetry. During the ABCDE (Airway, Breathing, Circulation, Disability, Exposure) of…

Abdominal Surgical Emergencies in Patients with Hematological Disorders: An Exacting Experience for Surgeons.

Management of surgical emergencies in patients with underlying hematological disorder is challenging due to increased incidence of peri-operative morbidity. We report two cases of abdominal surgical emergencies with pre-existing hematological conditions. The first case report is that of a patient diagnosed with pelvic abscess in a previously diagnosed case of Glanzmann's thrombasthenia and the second is a case of intestinal infarction previously diagnosed with paroxysmal nocturnal hemoglobinuria (PNH).

Long-term survival with tumor regression in androgen-induced liver tumors.

Two patients with androgen-induced liver tumors, one of whom had been partially treated by a liver resection, are reported. Hepatocellular carcinoma was diagnosed on histologic grounds. The patients had been receiving androgen therapy for primary diagnoses of either hypopituitarism or paroxysmal nocturnal hemoglobinuria. After androgen withdrawal, both are alive and well with no evidence of residual tumor 10 and 14 years after diagnosis, respectively.

Leptospirosis in lambs.

Two major episodes of acute hemolytic disease in lambs were associated with high mortality. Clinical signs were severe depression, dyspnea, and tachycardia. Gross postmortem findings included hemoglobinuria and icterus. Histologic examination revealed tubular necrosis and periacinar hepatocellular necrosis. Leptospira interrogans serotype pomona was proposed as the causative agent because high serum antibody titers for this bacterium were found in lambs and ewes in the affected flocks.

PNH clones prevalence study in ph-negative myeloproliferative neoplasms: a multicenter Italian study.

The prevalence of paroxysmal nocturnal hemoglobinuria (PNH) clones is little investigated in myeloproliferative neoplasms (MPN) patients. The aim of this multicenter study was to evaluate the prevalence of PNH clones (glycosyl-phosphatidyl-inositol lacking) in 119 Ph- negative MPN patients having anemia, LDH elevation, asthenia and history of thrombosis. All the participating centers performed the standardized diagnostic test by using a single lyophilized template for granulocytes, monocytes, and erythrocytes. Next generation sequencing (NGS) was performed in 2 PNH-positive MPN cases and 13 PN…

Reappraisal of the etiology of extracorpuscular non-autoimmune acquired hemolytic anemia in 2657 hospitalized patients with non-neoplastic disease.

INTRODUCTION: Unlike autoimmune hemolytic anemia (AIHA), literature on the etiological study of non-autoimmune hemolytic anemia (non-AIHA) i … nical or microangiopathic mechanism accounted for 19% of cases; hemolytic-uremic syndrome (HUS) 4%, hemoglobinuria because of hemolysis from external causes such as paroxysmal nocturnal hemoglobinuria (PNH) and march hemoglobinuria 7%, and chronic idiopathic hemolytic anemia or other unspecified non-AIHA 69%. We looked further for specific etiology or mechanism …

[Thrombocytopenia and thromboses in a young woman].

This article reports the case of a 34-year-old female patient with multiple thrombotic events (cerebral venous sinus thrombosis, ischemic duodenitis) with simultaneous thrombocytopenia (ca. 70/nl). A paroxysmal nocturnal hemoglobinuria without detectable hemolysis and with simultaneous myelodysplastic syndrome was causative for these symptoms. This rare disease is important to consider in the differential diagnosis of chronic inflammatory rheumatic diseases, such as systemic lupus erythematosus, antiphospholipid antibody syndrome and vasculitis.

Favism in a female newborn infant whose mother ingested fava beans before delivery.

We describe a case of favism in a female newborn infant with glucose-6-phosphate dehydrogenase (G6PD) deficiency whose mother had ingested fava beans 5 days before delivery. At birth there were clinical and hematologic signs of hemolytic anemia, hemoglobinuria, and no blood group immunization. Study of the G6PD activity and 2-deoxy-glucose-6-phosphate utilization rate revealed that the infant and the mother were heterozygous for G6PD deficiency.

[Abdominal Pain as the First Manifestation of Rare Hematological Diseases: A Case Series].

Abdominal pain frequently poses a significant diagnostic challenge because of its myriad etiologies. In this case series, we describe four patients initially presenting with abdominal pain who were subsequently diagnosed with rare hematological disorders: namely, paroxysmal nocturnal hemoglobinuria (PNH), myeloproliferative neoplasm (MPN), sickle cell Disease (SCD), and aggressive systemic mastocytosis (ASM). We discuss the unique pathophysiological mechanisms underlying the abdominal pain in these cases and emphasize the importance of considering hematological disorders in the differential di…

Pharmacology, Pharmacokinetics and Pharmacodynamics of Eculizumab, and Possibilities for an Individualized Approach to Eculizumab.

Eculizumab is the first drug approved for the treatment of complement-mediated diseases, and current dosage schedules result in large interindividual drug concentrations. This review provides insight into the pharmacokinetic and pharmacodynamic properties of eculizumab, both for reported on-label (paroxysmal nocturnal hemoglobinuria, atypical hemolytic uremic syndrome, generalized myasthenia gravis) and off-label (hematopoietic stem cell transplantation-associated thrombotic microangiopathy) indications. Furthermore, we discuss the potential of therapeutic drug monitoring to individualize trea…

Hemoglobin A1 in subjects with G-6-PD deficiency during and after hemolytic crises due to favism.

HbA1 was determined in 15 G-6-PD-deficient subjects during a hemolytic crisis with hemoglobinuria due to ingestion of fresh fava beans. The same G-6-PD-deficient subjects were studied again 4 months after the crisis, when they were asymptomatic. 15 normal healthy children served as controls. A statistically significant decrease in HbA1 was observed in the favic subjects during hemolytic crises compared with their values 4 months later and those of the control group.

Babesiosis and hemophagocytic syndrome in an asplenic renal transplant recipient.

Babesiosis is a malaria-like illness transmitted by the tick Ixodes dammini. The disease is endemic to the Northeast coastal region and parts of the Midwest. Symptoms-which include fever, anemia, elevated liver function tests, and hemoglobinuria-may be especially severe in asplenic or immunocompromised patients. In rare cases, infection with Babesia may be associated with marked pancytopenia. Bone marrow biopsy may reveal hemophagocytosis and marrow histiocytosis. We report a severe case of babesiosis and hemophagocytic syndrome in an asplenic renal transplant patient.
