Intermediate-term experience of Pipkin fracture-dislocations of the hip.

The purpose of this study was to review our intermediate-term experience with posterior fracture-dislocations of the hip. We evaluated the accuracy of the Pipkin and Brumback classification schemes as predictors of outcome, assessed the importance of certain treatment variables, and determined the complication rates of this injury in a group of 33 patients followed for an average of 49 months. The overall results were 67% good, 18% fair, and 15% poor, with no excellent results. The Pipkin classification scheme was a useful predictor of outcome because patients with less severe Pipkin 1 or 2 in

Accidental falls and injuries among seniors.

This study analyzes data on accidental falls for those aged 65 and older. The data are based on mortality statistics from 1980 to 1989 and morbidity statistics from 1985 to 1989. These statistics are provided to the Canadian Centre for Health Information by the provincial governments. Mortality rates and hospital separation rates for accidental falls are highest for those aged 65 and older. Accidents are one of the leading causes of death and hospitalization among seniors. In 1989, for those aged 65 and older, accidental falls accounted for 56% of accidental deaths and 65% of accident-related

[Hip dislocation with femur head fracture].

It is talked about the classification, the therapy and the late results of the traumatic dislocation of the hip joint with a fracture of the femoral head. Twelve patients with an average age of 36 years (from 18 to 77 years) were treated between the years 1972 and 1982. The rarer anterior dislocation of the hip including a fracture of the femoral head should be specifically classified, in order not to cause any errors for the therapeutic and prognostic assessment following the Pipkin classification. Type I and II with posterior dislocation justify a closed trial of reposition. The miscarried t

Management of a massive stage IV sacral decubitus ulcer with anterior flap hip disarticulation and myocutaneous gastrocnemius fillet flap in-continuity.

Severe sacral decubitus ulcers represent a common and often recalcitrant clinical problem in paralyzed or bedridden patients and require treatment strategies ranging from specialty beds and local wound care to aggressive debridement and local or free tissue flap coverage. This article presents the case of a young soldier who sustained severe injuries as a result of an improvised explosive device blast, including bilateral sciatic nerve injuries, sacrogluteal degloving, and severe left acetabular and femoral head fractures. The patient subsequently developed a recalcitrant stage IV sacral decub

Ballet injuries: the Australian experience.

There is a distinct difference between ballet injuries and sports injuries in general, and the sports medicine physician needs to study the technique of dance and the specific injuries that it may produce in order to treat dancers effectively. In Australia, which is typical of other countries where ballet is performed, ballet injuries include strained lumbar muscles, sprained ankle, Achilles tendinitis, clicking hip, jumper's knee, chondromalacia, stress fractures, patellar subluxation, and other knee and tendon problems.

Traumatic anterior dislocation of hip in a teenager: an open unusual type.

Traumatic anterior open dislocation of hip is rare in children and prone to be associated with injuries, extensive soft tissue damage and avascular necrosis of the femoral head. We report a hitherto undescribed anterior open dislocation of the hip joint in a 14-year-old boy. The dislocated head finished up in the contralateral obturator foramen. The management of the case and its relation to published classifications of anterior hip dislocations in children are discussed.

Relationships between core endurance, hip strength, and balance in collegiate female athletes.

PURPOSE/BACKGROUND: Lower extremity injuries such as Anterior Cruciate Ligament (ACL) tears remain a concern in collegiate female athletes. Core endurance and hip strength reportedly influence ACL and lower extremity injury risk. Good neuromuscular control, as measured by the Star Excursion Balance Test (SEBT) test is associated with decreased lower extremity injuries. The exact relationships between core endurance, hip strength, and balance (SEBT scores), and how they impact one another in the female collegiate athlete remain unclear. Thus, the purpose of this study was to investigate relatio

Traumatic and postoperative lesions of the lumbosacral plexus.

In 53 cases of injury of the lumbosacral plexus, 31 were due to trauma and 22 followed operations on the hip joint. Post-traumatic lesions occur mostly in conjunction with severe bony injuries, especially fractures of the acetabulum and of the pelvic ring. Nearly always, it is the sacral portion of the plexus that is involved, either predominantly or exclusively. Seventeen of the 22 postoperative pareses followed complete hip-joint replacement. In the postoperative lesions the lumbar plexus portion is most frequently involved. Ninety-one percent of all of our cases were misdiagnosed in previou

Posterior Hip Pointer: Subperiosteal Detachment of the Gluteal Muscles at the Posterior Iliac Crest in Two Elite Athletes.

Hip injuries are common in contact sports, particularly in high-impact activities. A well-known type of hip trauma is the hip pointer, which is a contusion of the iliac crest caused by a direct blow. Typically, hip pointers involve the lateral aspect of the iliac crest. In this case report, we present an unusual variation of this injury affecting the posterior iliac crest near the posterior superior iliac spine (PSIS). We describe two cases of elite athletes who sustained posterior iliac crest trauma, a condition we propose naming the "posterior hip pointer". This report highlights the clinica…

The Development and Validation of a Subjective Assessment Tool for the Hip in the Athletic Population.

BACKGROUND: No validated functional assessments are available that are designed specifically to evaluate the performance and function of the athletic hip. Subsections of some validated outcome assessments address recreation, but a full assessment dedicated to athletic hip function does not exist. Current hip scoring systems may not be sensitive to subtle changes in performance and function in an athletic, younger population. HYPOTHESIS: The patient-athlete subjective scoring system developed in this study will be validated, reliable, and responsive in the evaluation of hip function in the ath

Sciatic neuropathy.

Injuries to the sciatic nerve cause neurologic deficits in the peroneal and tibial nerve distributions. Interestingly, most injuries result in more severe deficits to the peroneal division compared to the tibial division. Thus, it can sometimes be difficult to distinguish sciatic neuropathy from peroneal neuropathy. The long course of the sciatic nerve leaves it vulnerable to nerve injury from a variety of causes. Most sciatic neuropathies are acute in onset, such as from hip arthroplasty and hip fracture or dislocation, but some occur from prolonged compression, such as during coma. Entrapmen

Rehabilitation of the pelvis, hip, and thigh.

Low back pain, hip pain, and muscle strain of the thigh are common in athletes. Proper rehabilitation requires proper identification. Once identified, rehabilitation must include restoration of function as well as prevention of recurrence. This article discusses methods of identifying, rehabilitating, and preventing injuries to the pelvis, hip, and thigh.

Triathlon: running injuries.

The running portion of the triathlon represents the final leg of the competition and, by some reports, the most important part in determining a triathlete's overall success. Although most triathletes spend most of their training time on cycling, running injuries are the most common injuries encountered. Common causes of running injuries include overuse, lack of rest, and activities that aggravate biomechanical predisposers of specific injuries. We discuss the running-associated injuries in the hip, knee, lower leg, ankle, and foot of the triathlete, and the causes, presentation, evaluation, an

Femoral head fractures: anatomy, diagnosis and management.

Femoral head fractures are complex and severe injuries, usually associated with hip dislocation. They typically result from high-energy trauma. Therefore, a low index of suspicion is required for diagnosis in these contexts. Initial presentation can vary depending on coexisting injuries but is typically an emergency and requires immediate reduction of the joint. Delays result in worse outcomes for patients. Pelvic radiographs are recommended before and after joint reduction, with Judet, inlet and outlet views to identify any associated acetabular fracture and pelvic ring injury. Computed tomog

Adaptor protein HIP-55-mediated signalosome protects against ferroptosis in myocardial infarction.

Ischemic heart disease is a leading cause of death worldwide. Myocardial infarction (MI) results in cardiac damage due to cell death and insufficient cardiomyocyte self-renewal. Ferroptosis, a novel type of cell death, has recently been shown as a key cause of cardiomyocyte death after MI. However, the complicated regulation mechanisms involved in ferroptosis, especially how ferroptosis is integrated into classical cell survival/death pathways, are still unclear. Here, we discovered that HIP-55, a novel adaptor protein, acts as a hub protein for the integration of the ferroptosis mechanism int

Injury toll following the 1997 Maccabiah Games bridge collapse: implications of major disasters for sports medicine teams.

A major disaster was encountered at the 1997 Maccabiah Games in Israel. As the Australian team was about to enter the main stadium for the opening ceremony, a pedestrian bridge they were crossing collapsed, killing 4 athletes and injuring many others. The aim of this paper is to establish the rates, types and anatomical locations of musculoskeletal injuries incurred by members of the Australian Maccabiah Games team, with particular reference to the impact of the bridge collapse. In total, the 410 members of the team (360 athletes and coaches and 50 team officials) reported 166 injuries from th

[Transposition of the cotyloid cavity in treatment of iatrogenic deformities of the hip joint].

The method of transposition of the cotyloid cavity after triple pelvic osteotomy was used in operative treatment of 97 patients (105 joints) aged from 7 through 19 years with iatrogenic injuries of the hip joint after conservative and operative treatment of congenital hip dislocation and dysplasia of the hip joints. All the patients had positive clinical and X-ray effects. The results obtained were analyzed from the viewpoint of endoprosthesis and compared with usually used palliative operations.

Hip Pain in Ballet Dancers: Evaluation and Management.

Hip and groin injuries are common in ballet dancers, who often begin sport-specific training at a young age. The unique demands of ballet include extreme range of motion, with an emphasis on external rotation and abduction. This creates a distinctive constellation of hip symptoms and pathology in this cohort, which may differ from other flexibility sports. When managing hip symptoms in this cohort, orthopaedic surgeons should consider the unique factors associated with ballet, including ballet-specific movements, morphologic adaptations of the hip, and the culture of the sport. Three common et

Patterns of lower-extremity injury in 224 runners.

Knee problems are the most common injury sustained in both male and female runners. Other problems include toe and forefoot injuries, inferior heel and arch pain, shin splints, ankle pain, calf and Achilles tendon pain, groin and hip pain, and stress fractures. The average total treatment for all injuries required 4.2 physician visits. Orthotics were used for more than 60% of the injuries, with successful results. A thorough understanding of running injuries should assist the physician in the treatment of these problems.

Hip pain in dancers.

Dancers require extreme ranges of motion in their hips. They require this for many styles and performances. Hip pain and hip injury in dancers can lead to lost work and performance time. There are many potential causes for hip pain in the dancer, including dysplasia, hyperlaxity, both intra- and extra-articular impingement, and soft tissue injuries. This article will review the current literature on these topics in dancers and how they can be applied to the dancer patient.
