Beyond the funding gap: Somalia's aid shock is becoming a primary health-care crisis.

Somalia's escalating humanitarian financing crisis is exposing a neglected dimension of health-system vulnerability: aid volatility can itself become a determinant of primary health-care (PHC) failure. Recent funding contractions have disrupted hospitals, primary facilities, mobile teams, and nutrition services while drought, displacement, infectious-disease outbreaks, and undernutrition continue to intensify health needs. In a system heavily reliant on external financing, abrupt donor withdrawal can destabilise frontline staffing, essential medicines, referral pathways, surveillance, and cont

Procedure-specific surgical site infection incidence varies widely within certain National Healthcare Safety Network surgery groups.

BACKGROUND: The National Healthcare Safety Network (NHSN) classifies surgical procedures into 40 categories. The objective of this study was to determine surgical site infection (SSI) incidence for clinically defined subgroups within 5 heterogeneous NHSN surgery categories. METHODS: This is a retrospective cohort study using the longitudinal State Inpatient Database. We identified 5 groups of surgical procedures (amputation; bile duct, liver or pancreas [BILI]; breast; colon; and hernia) using ICD-9-CM procedure codes in community hospitals in California, Florida, and New York from January 20

Owning the Trauma Bay: Teaching Trauma Resuscitation to Emergency Medicine Residents and Nurses through In-situ Simulation.

AUDIENCE: The following two cases were designed to address learning objectives specific to interns, junior residents, and senior residents in emergency medicine, as well as trauma-certified emergency nurses. INTRODUCTION: Traumatic and unintentional injuries account for 5.8 million deaths across the globe each year, with a high proportion of those deaths occurring within the initial hour from the time of injury. This "golden hour" begins in the pre-hospital setting yet is predominantly spent in the emergency department (ED).1 Being able to effectively manage the multidisciplinary team require

One positive impact of health care reform to physicians: the computer-based patient record.

The health care industry is an information-dependent business that will require a new generation of health information systems if successful health care reform is to occur. We critically need integrated clinical management information systems to support the physician and related clinicians at the direct care level, which in turn will have linkages with secondary users of health information such as health payors, regulators, and researchers. The economic dependence of health care industry on the CPR cannot be underestimated, says Jeffrey Ritter. He sees the U.S. health industry as about to ente

Prevalence, molecular characteristics and antimicrobial susceptibility patterns of MRSA in hospitalized and nonhospitalized patients in Barbados.

The epidemiology and clonality of methicillin-resistant Staphylococcus aureus (MRSA) has not been investigated, as not much research or surveillance has been undertaken to identify and characterize the circulating MRSA strains in Barbados. Prevalence rates, molecular characteristics and antimicrobial susceptibility pattern of MRSA infections in hospitalized and nonhospitalized patients were investigated. A total of 293 isolates were included in the study, with 100 from the hospital and 193 from the public health laboratory. Isolates were collected over a period of 1 (2015-2016) and 3 years (20

Bioterrorism vs. health security--crafting a plan of preparedness.

Bioterrorism, once a subject of fantasy and speculation, has become all too real in a world turned upside down by the September 11, 2001. series of events. An essential, but as yet unanswered, question has become a crucial topic for discussion on the nightly news and in living rooms across the United States: How much of a terrorist threat do we face, and what must be done to control its potential for mass destruction? This article seeks to both answer this question and explore proper plans of preparedness for the eventuality of the unthinkable.

Perceptions of physiotherapists regarding challenges experienced by patients with Deafness in accessing rehabilitative care.

BACKGROUND: Hearing impairment, including deafness, affects approximately 5% of the global population, with the majority residing in low- and middle-income countries (LMICs) such as South Africa. Despite the constitutional right to healthcare for all, patients with deafness face persistent health inequalities because of communication barriers with healthcare professionals. Physiotherapists are often unprepared to provide care for patients with deafness, leading to a breakdown in communication and poor healthcare outcomes. OBJECTIVES: This study explored physiotherapists' perceptions regarding

Disruptive Innovations to Achieve Health Equity Through Healthcare and Research Transformation.

In order to achieve health equity, we must implement innovative health system, public health, and policy-level interventions to address the historical root causes of structural and institutional racism embedded in our medical and social systems. A history of unconsented medical and research experimentation on vulnerable groups and residual healthcare provider biases toward minoritized patients has led to patient distrust of medical systems and poor quality of care. Historical discriminatory housing and lending policies resulted in racial residential segregation and neighborhoods with inadequat

Boosting antenatal care attendance and number of hospital deliveries among pregnant women in rural communities: a community initiative in Ghana based on mobile phones applications and portable ultrasound scans.

BACKGROUND: The World Health Organization has recommended at least four antenatal care (ANC) visits and skilled attendants at birth. Most pregnant women in rural communities in low-income countries do not achieve the minimum recommended visits and deliver without skilled attendants. With the aim of increasing number of ANC visits, reducing home deliveries, and supplementing care given by ANC clinics, a proposed system based on low-cost mobile phones and portable ultrasound scan machines was piloted. METHODS: A sample of 323 pregnant women from four rural communities in the Central Region of G

Stakeholder experiences and expectations of transition of care services to reduce medication-related hospital readmission for cardiology patients.

BACKGROUND: Transition of care (ToC) for patients with cardiovascular diseases is a complex, high-risk period often leading to medication-related harm (MRH) and hospital readmissions. While specialised programs exist, a gap in individualised medication management services exist for specific cardiology patient populations. Pharmacist-led, interdisciplinary ToC services have demonstrated a positive impact on patient outcomes. The REducing hospital re-admission for high-risk CARDiology patients (RECARD) program aims to address this by co-designing, implementing, and evaluating a new pharmacist-le

Computerized definitions showed high positive predictive values for identifying hospitalizations for congestive heart failure and selected infections in Medicaid enrollees with rheumatoid arthritis.

PURPOSE: Computerized definitions are used to identify serious infections and congestive heart failure leading to hospitalizations in studies of medication safety. However, information on their accuracy is limited. We evaluated the ability of computerized definitions to identify these conditions as the reason for admission among patients diagnosed with rheumatoid arthritis (RA). METHODS: Medical charts were randomly selected from a systematic sample of hospitalizations for selected conditions in a cohort of Medicaid patients with RA. We calculated positive predictive values (PPVs) for compute

The use of vacuum-assisted wound closure therapy in thoracic operations.

BACKGROUND: Conventional treatment of complex, nonresolving empyemas after an episode of pneumonia or a chest operation often requires an open-window thoracostomy. This necessitates frequent, often painful dressing changes and is associated with prolonged hospitalization. The wound is often malodorous, causing significant social distress to patients and unquestionably affects their quality of life. We assessed the value of using vacuum-assisted closure (VAC) therapy in managing patients with a persistent infected pleural space. METHODS: The study included 10 patients. All patients signed an i

The Healer's Heartache: Reforming Medicine from the Rough Ground.

Current and future clinicians are experiencing a malaise that is at once deeply felt yet difficult to articulate. While "burnout" and "moral injury" have come to dominate many accounts, these terms do not fully capture the experience of clinicians who struggle to name a sense of loss, lament, and love for a beloved profession. Therefore, we propose "heartache" as a more fitting description. While burnout comes from the world of industry and moral injury from the world of war, heartache comes from the world of love. Burnout depends on individuals to cultivate resilience, and moral injury encour

A qualitative study of patient choices between herbal and hospital care for infectious diseases in Somalia.

Somalia's healthcare system is marked by a reliance on traditional medicine and under-resourced modern facilities. This study explores how communities navigate treatment choices for infectious diseases, such as malaria and cholera, amid competing trust in herbal remedies and biomedical care. We conducted in-depth interviews with 30 participants, including 15 patients, 10 traditional healers, and 5 healthcare providers, alongside 3 focus group discussions (FGDs). Data were analyzed using thematic analysis. Three key themes emerged: (1) the cultural legitimacy of traditional medicine, (2) the pe

Chinese Bioethics of Trust and Autonomy: How Doctors Make Patient Referrals After the Introduction of a Two-Way Referral System.

Based on 24 semi-structured interviews with secondary hospital doctors, we empirically investigate how a new two-way referral system in China influences the alignment between referring doctors, patients, and doctors who receive referrals. In particular, we focus on how this system addresses the challenges of trust, autonomy, and bioethics during the shared decision-making process. Our research shows that the two-way referral system has spawned new acquaintance networks, therefore improving communication and collaboration between doctors. Especially during upward referrals, benefits associated
