Comparison of Patients with Hospital-Recorded Nephrotic Syndrome and Patients with Nephrotic Proteinuria and Hypoalbuminemia: A Nationwide Study in Denmark.

BACKGROUND: Registry-based studies of nephrotic syndrome (NS) may only include a subset of patients with biochemical features of NS. To address this, we compared patients with laboratory-recorded nephrotic proteinuria and hypoalbuminemia to patients with hospital-recorded NS. METHODS: We identified adult patients with first-time hospital-recorded NS (inpatients, outpatients, or emergency-room visitors) in the Danish National Patient Registry and compared them with adults with first-time recorded nephrotic proteinuria and hypoalbuminemia in Danish laboratory databases during 2004-2018, definin

Risk of venous thromboembolism in people admitted to hospital with selected immune-mediated diseases: record-linkage study.

BACKGROUND: Venous thromboembolism (VTE) is a common complication during and after a hospital admission. Although it is mainly considered a complication of surgery, it often occurs in people who have not undergone surgery, with recent evidence suggesting that immune-mediated diseases may play a role in VTE risk. We, therefore, decided to study the risk of deep vein thrombosis (DVT) and pulmonary embolism (PE) in people admitted to hospital with a range of immune-mediated diseases. METHODS: We analysed databases of linked statistical records of hospital admissions and death certificates for th

Flavonoid-rich dietary patterns and incident hospital-recorded Parkinson's disease: a prospective UK Biobank study.

INTRODUCTION: Evidence linking flavonoid-rich dietary patterns to the risk of Parkinson's disease (PD) remains limited and inconsistent, and molecular clues to this association remain scarce. This study examined the association between a flavonoid diet score (FDS) and incident hospital-recorded PD and explored related plasma proteomic and cellular expression patterns. METHODS: In this prospective analysis of 123,655 UK Biobank participants, we used multivariable Cox proportional hazards models to estimate hazard ratios (HRs) and 95% confidence intervals (CIs) for the association between FDS a

Recognising and responding to clinical deterioration in acute hospitals. Recording patient-reported wellness after Martha's rule.

BACKGROUND: Early recognition of clinical deterioration is central to patient safety, yet preventable harm still occurs. UK policy emphasizes listening to patients and systematically capturing patient-reported wellness. AIMS: The aim of this study was to understand current practice for including patient and family voices in recognising and responding to deterioration, and to assess the feasibility of a structured approach to including patient-reported wellness as part of routine clinical monitoring. METHODS: A mixed-methods feasibility study of a structured approach to capturing patient asse

Cardiology hospital admission risk prediction: training, internal validation and technical implementation in the electronic health record.

AIMS: Rising healthcare demand is increasingly outpacing available outpatient capacity in cardiology, where follow-up is often scheduled at fixed intervals despite substantial variation in individual patient risk. This uniform follow-up approach contributes to high outpatient workload and inefficient use of clinical resources. Accurate risk estimation using routinely collected electronic health record (EHR) data may support more individualized follow-up planning by identifying patients at very low risk of mortality or unplanned hospitalization, in whom follow-up intervals could be safely exten

Treatment goal limitations in acute hospital care : A retrospective empirical ethics study of documentation and recorded reasoning.

BACKGROUND: Treatment goal limitations, such as "do not resuscitate", "do not escalate" and "comfort terminal care" are frequent in acute hospital care for patients with advanced illness, multimorbidity or impending death. Their ethical defensibility depends on transparent documentation of medical indications, proportionality and, where relevant, patient will. OBJECTIVE: To examine whether documentation made medical rationale, patient will, advance directives, relatives' involvement or ethics consultation visible. METHODS: We retrospectively analyzed documentation in an Austrian internal med

Development and Validation of an Electronic Health Record-Derived Prediction Model for Preventing COVID-19 Hospitalization and Death.

Hospitalization and death following COVID-19 infection continue to pose a major public health concern and place strain on health system resources. Outpatient antiviral medication can reduce the risk of COVID-19 hospitalization and death for those at risk of poor outcomes, but identifying high-risk populations who may benefit most from treatment is challenging. The objective of this study was to develop and validate a prediction model for the composite outcome of hospitalization or death in the 14 days following COVID-19 infection. Our sample included 67,530 COVID-19 infections documented in ou

Guideline concordance of electronic health record order sets for hospital-based treatment of alcohol withdrawal syndrome.

BACKGROUND: Treatment of alcohol withdrawal syndrome (AWS) in hospitals is inconsistent. Electronic health record (EHR) order sets protocolize care. OBJECTIVE: We examined variation in AWS order sets across hospital organizations and their concordance with AWS guidelines. METHODS: We conducted a cross-sectional study of hospital organization user-created EHR order sets for AWS extracted from the December 2021 Epic® userweb community library. Hospital organizations with an acute care hospital and ≥ 1 AWS order set were included. We measured the proportion of guideline-concordant care pract

Clinical and financial effects of smart pump-electronic medical record interoperability at a hospital in a regional health system.

PURPOSE: The pharmacist-led implementation of a smart pump-electronic medical record (EMR) interoperability program at a hospital within a regional health system is described. SUMMARY: Smart pump-EMR interoperability involves a wireless, bidirectional connection by which infusion information transmitted by the EMR prepopulates infusion devices, reducing keystokes and opportunities for manual programming errors. The smart pumps transmit time-stamped infusion data to the EMR for nurse documentation. Use of interoperability technology forces the use of dose-error reduction software so that 100%

Integration of physical abuse clinical decision support into the electronic health record at a Tertiary Care Children's Hospital.

OBJECTIVE: To evaluate the effect of a previously validated electronic health record-based child abuse trigger system on physician compliance with clinical guidelines for evaluation of physical abuse. METHODS: A randomized controlled trial (RCT) with comparison to a preintervention group was performed. RCT-experimental subjects' providers received alerts with a direct link to a physical abuse-specific order set. RCT-control subjects' providers had no alerts, but could manually search for the order set. Preintervention subjects' providers had neither alerts nor access to the order set. Complia

Factors leading to cesarean section delivery at Felegehiwot referral hospital, Northwest Ethiopia: a retrospective record review.

BACKGROUND: Cesarean section is the commonest obstetric operative procedure worldwide. When used appropriately cesarean sections can improve infant and/or maternal outcomes. However, when used inappropriately the potential harm may exceed the potential benefit of cesarean section. Appreciating the limited information in this area the current study assessed the rate and factors associated with cesarean section in Felegehiwot referral hospital, Bahir Dar, northwest Ethiopia. METHOD: The study was a retrospective analysis of eligible patient records that included 2967 pregnant women who had unde
