Mental health and substance use disorders at delivery hospitalization and readmissions after delivery discharge.

BACKGROUND: The objective was to assess mental health and substance use disorders (MSUD) at delivery hospitalization and readmissions after delivery discharge. METHODS: This is a population-based retrospective cohort study of persons who had a delivery hospitalization during January to September in the 2019 Nationwide Readmissions Database. We calculated 90-day readmission rates for MSUD and non-MSUD, overall and stratified by MSUD status at delivery. We used multivariable logistic regressions to assess the associations of MSUD type, patient, clinical, and hospital factors at delivery with 90

BSAFER: A Web-based intervention for drug use and intimate partner violence demonstrates feasibility and acceptability among women in the emergency department.

BACKGROUND: Addressing violence along with drug use change goals is critical for women with coexisting intimate partner violence (IPV) and substance use disorders (SUDs). METHODS: This was an acceptability and feasibility study of BSAFER, a brief Web-based program and booster phone call addressing violence and drug use. A screening survey identified women with recent drug use and IPV in the emergency department (ED). Participants were randomized to BSAFER or a Web-based control program and booster call providing education about home fire safety. Program completion, usability, satisfaction, an

Who knows best: the patient or the provider? A nursing perspective.

Who knows best: the patient or the provider? My opinion, based on a review of the literature and practical experience, is that the patient, the provider and the system each offer a unique perspective that we can draw upon in ensuring quality care across the continuum. Nurses have a unique body of knowledge and skill that they bring to each interaction with the patient. They must have an awareness of the patient's and the system's expectations and interact and negotiate realistic expectations for each. The maintenance of balanced expectations and the measurement of effectiveness will continue t

Strategies to Improve Patient-Centered Care for Drug Use-Associated Infective Endocarditis: JACC Focus Seminar 2/4.

Drug use-associated infective endocarditis (DUA-IE) is a major cause of illness and death for people with substance use disorder (SUD). Investigations to date have largely focused on advancing the care of patients with DUA-IE and included drug use disorder treatment, decisions about surgery, and choice of antibiotics during the period of hospitalization. Transitions from hospital to outpatient care are relatively unstudied and frequently a key factor of uncontrolled infection, continued substance use, and death. In this paper, we review the evidence supporting cross-disciplinary care for peopl

Development of a Trigger Tool to Identify Adverse Events and Harm in a Neuropsychiatry Setting.

BACKGROUND OBJECTIVES: Adverse drug events (ADEs) present the greatest risk of harm to patients in hospitals, especially those receiving neuropsychiatric treatment. The objective of the present record-based study was to test the appropriateness of the neuropsychiatry trigger tool (NPTT) to identify and measure harm due to adverse events (AEs). METHODS: A total of 1324 clinical case notes of discharged patients from 2017 to 2018 with a hospital stay >24 hours to <70 days were examined. RESULTS: One hundred forty-four (10.88%) patients experienced 166 AEs. A total of 854 triggers (range, 1-12

Ethical dilemmas and nursing.

Professional responsibilities, tradition, and personal conscience along with legal, philosophical, and religious convictions dictate nursing interventions. Inevitably, these factors embrace life-sustaining therapies; however, in view of complications, prognosis, pain and suffering, and their own views of quality of life, some patients express wishes inconsistent with life-sustaining measures. In other situations, the health care provider as well as the patient may view heroic efforts as more debilitating than resortative. Resolving the conflict while preserving the patient's best interests req

Review of medical discharge summaries and medical documentation in a metropolitan hospital: impact on diagnostic-related groups and Weighted Inlier Equivalent Separation.

BACKGROUND: Accurate and comprehensive clinical documentation is crucial for effective ongoing patient care, follow up and to optimise case mix-based funding. Each Diagnostic Related Group (DRG) is assigned a 'weight', leading to Weighted Inlier Equivalent Separation (WIES), a system many public and private hospitals in Australia subscribe to. AIMS: To identify the top DRG in a general medical inpatient service, the completeness of medical discharge documentation, commonly missed comorbidities and system-related issues and subsequent impact on DRG and WIES allocation. METHODS: One hundred an

Determinants of hospital utilization: the content of medical inpatient care in Israel.

BACKGROUND: The increasing utilization of general internal medicine hospital wards in Israel during the last decade is a source of concern for health policy makers. OBJECTIVES: To report on the distribution of selected main and secondary diagnoses among GIM inpatients, and to estimate the proportion of disorders for which appropriate care in the community will reduce the need for hospital admissions and re-admissions. METHODS: Data from the Health Information and Computer Services of the Israel Ministry of Health (national hospitalization database) for a one year period were analyzed by dist

Managing emotions. Accenting the positive might not produce the highest satisfaction payoff.

The relationship between patient emotions during hospitalization and their retrospective satisfaction varies across service elements, across emotions, and is influenced by the temporal positioning of both emotions and the service element. Marketers should concentrate on reducing negative emotions attributed to the provider (e.g., hostility and frustration) and to the hospitalization situation (e.g., distress and depression), according to the results of the authors' study. Providers also should attempt to alleviate negative emotional states connected with arousal (e.g., jittery and excited). Po

Preferences for physician sex among adults attending Syrian public teaching hospitals: A multicenter cross-sectional study.

BACKGROUND: Physician sex can influence communication, comfort, and care-seeking. Evidence from conservative contexts suggests preferences are strongest in intimate examinations and vary by specialty. Local data from Syria are limited. METHODS: We conducted a multicenter cross-sectional survey in major Syrian public teaching hospitals. Adults attending participating hospitals, including patients and accompanying companions, completed an interviewer-administered questionnaire adapted from prior work and reviewed for clarity in Arabic, capturing preference for physician sex (male, female, no pr

Improvement in Substance Use and Quality of Life in Patients Engaged in a Peer Navigator Program for Opioid Use Disorder.

BACKGROUND: Opioid use disorder (OUD) is associated with high emergency department (ED) utilization, overdose, and unmet social needs. ED-based peer navigator (EDPN) programs may improve engagement in care and address social determinants of health, but data on their clinical and behavioral impact are limited. OBJECTIVE: To assess changes in substance use, quality of life, patient satisfaction, and healthcare utilization among patients enrolled in an EDPN program. METHODS: We conducted a retrospective review of self-reported data using the Substance Abuse and Mental Health Services Administra

Predicting Satisfaction of Parents of Pediatric Patients: Perceived Quality of Providers' Communication Mitigates Negative Effects of Shorter than Desired Consultations.

This research investigated the predictors of satisfaction for parents of pediatric patients after a clinical consultation. Specifically, we assessed whether perceptions of their provider's communication quality influenced the degree to which their (dis)satisfaction with consultation length associated with their provider rating and intent to recommend the provider's office. Using patient satisfaction survey data collected after initial clinical visits to a pediatric hospital (N = 12,004), we found that communication quality was a stronger predictor for those who were dissatisfied with their con

Interdisciplinary expectations of the medical social worker in the hospital setting.

Nearly 500 physicians, nurses, and social workers affiliated with four general hospitals participated in a mailed questionnaire survey designed to compare interprofessional expectations of the medical social worker role in the hospital. As hypothesized, physicians and nurses expect the exclusive role of the social worker to be more limited to working with families, resolving social-environmental problems, and providing referrals than social workers expect of their role. In general, physicians and nurses do not disagree with medical social workers on what social workers expect to do in the hosp

Building continuity in handovers with shorter residency duty hours.

As junior doctors work shorter hours in light of concerns about the harmful effects of fatigue on physician performance and health, it is imperative to consider how to ensure that patient safety is not compromised by breaks in the continuity of care. By reconceptualizing handover as a necessary bridge to continuity, and hence to safer patient care, the model of continuity-enhanced handovers has the potential to allay fears and improve patient care in an era of increasing fragmentation. "Continuity-enhanced handovers" differ from traditional handovers in several key aspects, including quality o

Workplace violence directed at nursing staff at a general hospital in southern Thailand.

This study aimed to document the characteristics of workplace violence directed at nursing staff, an issue which has rarely been studied in a developing country. Two study methods, a survey and a key informant interview, were conducted at a general hospital in southern Thailand. A total of 545 out of 594 questionnaires sent were returned for statistical analysis (response rate=91.7%). The 12-month prevalence of violence experience was 38.9% for verbal abuse, 3.1% for physical abuse, and 0.7% for sexual harassment. Psychological consequences including poor relationships with colleagues and fami

[Nursing patients with learning and physical disabilities in hospitals - a systematic literature review].

People with learning and physical disabilities require special attention and nursing care during hospitalisation. A systematic literature research in the databases PUBMED, CINAHL and Cochrane Library (1990-2011) was conducted, 17 relevant publications could be found. The following problems were identified. The situation of people with learning disabilities in hospital is characterised by communication barriers between patients and health care professionals. Furthermore, the emotional situation of patients has to be emphasised. In the foreign environment of a hospital, they suffer from fear and
