Demographics, Types of Patient-Reported Allergic Diseases, and Anaphylaxis in Mastocytosis: A Single-Center US Experience.

BACKGROUND: Patients with mastocytosis are at increased risk of anaphylaxis. Idiopathic anaphylaxis and venom-induced allergy are commonly reported in mastocytosis; however, the incidence of other allergies has been less well studied. OBJECTIVE: To characterize the true prevalence of allergic disease including food, venom, contrast, and drug allergy compared with the number of patients who simply avoid potential triggers. We also gathered demographic information. METHODS: We performed a retrospective study of 259 consecutive pediatric and adult patients with a confirmed diagnosis of either c

Cross-reactivity between Anisakis spp. and wasp venom allergens.

BACKGROUND: Anisakiasis is caused by the consumption of raw or undercooked fish or cephalopods parasitized by live L3 larvae of nematode Anisakis spp. Larvae anchor to stomach mucosa releasing excretion/secretion products which contain the main allergens. It has been described that nematode larvae release venom allergen-like proteins among their excretion/secretion products. We investigated potential cross-reactivity between Anisakis and wasp venom allergens. METHODS: Two groups of 25 patients each were studied: wasp venom- and Anisakis-allergic patients. Sera from patients were tested by Imm

Semiochemicals from fungal associates of bark beetles may mediate host location behavior of parasitoids.

In laboratory olfactometer bioassays, females of two hymenopteran parasitoid species, Roptrocerus xylophagorum (Ratzeburg) (Hymenoptera: Pteromalidae) and Spathius pallidus (Ashmead) (Hymenoptera: Braconidae), were attracted to odors from bark or bolts of loblolly pine, Pinus taeda L., colonized by bluestain fungi (genus Ophiostoma) associated with the parasitoids' bark beetle hosts. Mock-inoculated bolts and bark were less attractive or unattractive in these bioassays. Bark infested with host larvae that lacked their fungal and other normal microbial associates was attractive to R. xylophagor

Venomous Bites, Stings, and Poisoning: An Update.

This article discusses the epidemiology, prevention, clinical features, and treatment of venomous bites by snakes, lizards, and spiders; stings by fish, jellyfish, echinoderms, insects, and scorpions; and poisoning by ingestion of fish, turtles, and shellfish. Invertebrate stings cause fatalities by anaphylaxis, secondary to acquired hypersensitivity (Hymenoptera, such as bees, wasps, and ants; and jellyfish), and by direct envenoming (scorpions, spiders, jellyfish, and echinoderms). Simple preventive techniques, such as wearing protective clothing, using a flashlight at night, and excluding v

[Wasp and bee stings].

Occasionally a dentist is confronted with a painful, red facial swelling which has suddenly appeared, with no signs of a dental cause. In addition to the other possible causes, one should consider the possibility of an insect bite, especially in the summer months. The consequences of an insect bite or sting are often underestimated. Hymenopterae (wasps and honeybees) stings are not generally serious in nature, but can result in severe systemic medical complications, such as an allergic reaction. Dentists who are confronted with a patient with an insect bite or sting must provide the patient wi

[Histamine liberation in vitro or histamine release (HR). Study of the threshold of positivity in allergy to hymenopteran venoms].

The technique of histamine release in vitro (HR) is only usable in diagnosis of hymenoptera venom allergy. The allergen extracts that are supplied for clinical (skin tests) or biological diagnosis contain significant amounts of histamine, falsify the results, especially HR, and reduce significantly the correlation between HR and other biological or cutaneous tests. Dialysis of the allergen extracts significantly improves the sensitivity and specificity of the technique and lowers the threshold of positivity of HR from 30% to 12%. Correlation of HR with other diagnostic parameters:clinical hist

Severe oesophagitis and oesophageal stricture secondary to anaphylaxis in a dog.

This report describes the development of oesophagitis and oesophageal stricture in a dog secondary to anaphylaxis. A 9-month-old male entire Brussels Griffon presented in anaphylactic shock after exposure to Hymenoptera species (Sp). The dog had a history of an anaphylactic reaction after exposure to Hymenoptera, successfully managed with antihistamines and dexamethasone. On this presentation, the dog was vomiting, lethargic, hypotensive and hyperlactatemic, with a mild elevation in serum alanine transaminase (ALT) concentration and gall bladder wall oedema. An adrenaline constant rate infusio

Minor interference of cross-reactive carbohydrates with the diagnosis of respiratory allergy in standard clinical conditions.

BACKGROUND: Immunoglobulin E (IgE) to N-glycans from plant and invertebrate glycoproteins induces extensive in vitro cross-reactivity. This study investigates the prevalence and diagnostic relevance of IgE to these N-glycans [cross-reactive carbohydrate determinants (CCDs)] in patients with suspicion of respiratory allergy. METHODS: A total of 1,025 adult subjects with symptoms of rhinitis and/or asthma from a reference allergy clinic were studied. Determinations included a structured questionnaire, skin prick tests (SPT), total IgE, a multiallergen IgE test and specific IgE (sIgE) to bromela

Follow-up of venom immunotherapy (VIT) based on conventional techniques and monitoring of immunoglobulin E to individual venom allergens.

OBJECTIVES: To assess the efficacy of venom immunotherapy (VIT) and monitor changes in in vivo and in vitro test results after 5 years of treatment and subsequent follow-up. To study the profile of immunoglobulin (Ig) E to individual allergens prior to treatment and 1 year afterwards. METHODS: We studied 562 patients with hymenoptera venom allergy (438 to bee, 124 to wasp), all of whom underwent immunotherapy with Apis or Vespula extract. The patients were followed up using conventional in vivo and in vitro tests, and in 51 cases, specific IgE against the main hymenoptera allergens was measur

Severe immediate type hypersensitivity reactions in 105 German adults: when to diagnose anaphylaxis.

BACKGROUND: There are no epidemiological data available on anaphylaxis in German adults and so far there has been no consensus on when to diagnose anaphylaxis, mainly due to a lack of generally accepted diagnostic criteria. Recently, an international expert group addressed this issue by suggesting new diagnostic criteria for anaphylaxis. We addressed the usefulness of the suggested diagnostic criteria in German adults and attempted to identify putative trigger factors. METHODS: Medical records were reviewed for patients seen in 2006 who had suffered any reaction that led to the suspicion of a

Diagnosis of venom allergy by flow cytometry. Correlation with clinical history, skin tests, specific IgE, histamine and leukotriene C4 release.

BACKGROUND: Potent allergens such as hymenoptera venoms are capable of inducing severe and life threatening clinical reactions. Percentage of false negative results obtained by the usual diagnostical methods is comprised between 10 and 25%. OBJECTIVE: Evaluation of the sensitivity and the specificity of cellular tests and particularly evaluation of a new flow cytometric method. METHODS: Forty-five allergic patients having experienced a local, a systemic reaction or an anaphylactic shock and 10 controls having undergone hymenoptera stings without clinical reactions were selected on the basis

Serum Mas-related G protein-coupled receptor X2 concentration in drug-induced immediate hypersensitivity reactions.

INTRODUCTION: Mas‑related G protein-coupled receptor X2 (MRGPRX2) has emerged as a mediator of mast cell activation in acute and chronic conditions. Exogenous ligands, such as neuromuscular blocking agents (NMBAs) and fluoroquinolones (FQs), can trigger MRGPRX2‑dependent activation and may augment immunoglobulin E (IgE)-mediated pathways. Although investigators have measured serum MRGPRX2 level in asthma, mastocytosis, and chronic urticaria, its role in immediate hypersensitivity reactions (IHRs) to FQs or NMBAs remains unclear. OBJECTIVES: We conducted this study to determine whether increas

Prospective evaluation of the diagnostic value of sensitive KIT D816V mutation analysis of blood in adults with suspected systemic mastocytosis.

BACKGROUND: Sensitive KIT D816V mutation analysis of blood has been proposed to guide bone marrow (BM) investigation in suspected systemic mastocytosis (SM). The aim of this prospective study was for the first time to compare the D816V status of the "screening blood sample" used to guide BM biopsy in suspected SM to the outcome of the subsequent BM investigation. METHODS: Fifty-eight adult patients with suspected SM were included. The outcome of sensitive KIT D816V analysis of blood was compared to the result of the BM investigation. RESULTS: Screening blood samples from 44 of 58 patients te

Patients with negative skin tests.

PURPOSE OF REVIEW: The aim of this article is to present and discuss the clinical problem of systemic anaphylaxis to Hymenoptera venoms in patients without detectable immunoglobulin E, as it appears in recent literature. Reported at variable frequency in large series of patients undergoing evaluation, systemic anaphylaxis was previously considered to reflect lost sensitization or to involve non-immunoglobulin E mediated mechanisms. Sporadic case-reports drew attention to the fact that severe or even fatal reactions may occur in patients with negative skin tests. RECENT FINDINGS: A breakthroug

Myocardial infarction following bee sting.

Bee stings are commonly encountered worldwide. Various manifestations after bee sting have been described. Local reactions are common. Unusually, manifestations like vomiting, diarrhoea, dyspnoea, generalised oedema, acute renal failure, hypotension and collapse may occur. Rarely vasculitis, serum sickness, neuritis and encephalitis have been described which generally develop days to weeks after a sting. Acute coronary syndromes after hymenoptera stings and other environmental exposures are referred to as the Kounis syndrome or allergic myocardial ischaemia and infarction. We report a 60 year

Case report. Bee sting brachial block.

A case of brachial plexus block is presented, following a bee sting in the posterior triangle of the neck. The onset of neurological symptoms was rapid as was their subsequent resolution. Delayed peripheral neurological symptoms believed to have an immunological basis have been reported in response to stings from bees and other Hymenoptera both in the central and peripheral nervous systems (Goldstein et al., 1960; Means et al., 1973; Bachman et al., 1982; Weatherall et al., 1987; Van Antwerpen et al., 1988), but to the authors' knowledge no similar case of immediate peripheral block has been r

Bee or Wasp Sting.

While jogging in a local park in Hong Kong, a 55-year-old, previously healthy man was stung on the ventral aspect of his right wrist. The tiny stinger was gently removed with nail cutters and examined under a microscope at 80x magni cation; plucking the stinger is ill- advised as this may inject more venom into the wounded site. Two days after stinging, the microscopic appearance of the stinger con rmed the diagnosis to be from a bee instead of a wasp or other insect. A simple method of con rming the nature of insect stings and an overview of Hymenoptera stings and their management are provide

Food deprivation alters reproductive performance of biocontrol agent Hadronotus pennsylvanicus.

Diet can influence parasitoid reproductive performance, and therefore, the efficacy of biocontrol programs. We evaluated the influence of food deprivation on the reproductive fitness and behavior of the egg parasitoid Hadronotus pennsylvanicus (Hymenoptera: Scelionidae), a prospective biocontrol agent for Leptoglossus zonatus (Heteroptera: Coreidae). Newly emerged female parasitoids were mated and provided host eggs every other day while being provisioned with various honey diet regimes or a consistent supply of water. When given frequent access to a honey diet, female parasitoids lived signif
