A sixty nine-year-old woman was admitted to the hospital because of further examination of hypercalcemia. On July 1990, she complained of general fatigue and loss of appetite. She was pointed out to have hypercalcemia (15.1mg/dl), urolithiasis, and renal insufficiency. CT films of the chest showed swelling of the mediastinal lymphnodes and CT of the abdomen nephrocalcinosis. Ga-scintigraphy demonstrated an abnormal accumulation of gallium in the mediastinum. Levels of the parathyroid hormone was normal. Levels of the serum calcium (13.7mg/dl), angiotensin converting enzyme (30.4IU/L) and 1.25 …

We describe an elderly man who presented with hypercalcemia associated with suppressed intact parathyroid hormone (PTH) levels. Despite renal insufficiency the circulating 1,25-dihydroxyvitamin D (1,25(OH)2D) was in the upper part of the normal range. Known causes of hypercalcemia were absent and mild hypercalcemia with suppression of intact PTH persisted until after bilateral hip replacement for severe arthritis (1 year after presentation). After hip replacement the ionized calcium normalized, intact PTH normalized, and 1,25(OH)2D decreased markedly. We believe the abnormalities in mineral ho…

Hypercalcemia is a common complication of certain advanced malignancies and although not therapeutically difficult, its presence raises complex ethical issues. Treatment of this condition is most easily justified when the patient is not terminal, the benefits are tangible, and the patient agrees with therapy. Withholding treatment is defensible medically, legally, and morally when a terminal situation is present, when drawbacks exceed the benefits, and when an informed patient declines therapy. Hypercalcemia is one of a number of conditions of dying that can be controlled by the physician. The…

The nephrotic syndrome developed in a 47-year-old woman in association with severe hypercalcemia (23.5 mg/dl) from primary hyperparathyroidism. Other causes for hypercalcemia were sought and were not found. The nephrotic syndrome remitted spontaneously within two weeks of normalization of the serum calcium level. Kidney biopsy specimens showed deposition of electron-dense material, thought to be calcium, in the glomerular basement membranes initially and in the mesangium as well six months later.

Two patients with the acquired immunodeficiency syndrome and disseminated cytomegalovirus infection developed hypercalcemia associated with suppressed parathyroid gland activity. Neither patient had evidence of a malignant, endocrinologic, granulomatous, or drug-related cause for hypercalcemia. Increased osteoblastic bone resorption induced by cytomegalovirus infection may have been the cause for the hypercalcemia. Physicians should be alert for the occurrence of endocrinologic and metabolic problems in immunosuppressed patients.

Circulating vitamin D metabolite concentrations, i.e. 25-(OH)D, 24,25-(OH)2D, 1,25-(OH)2D have been assayed in 14 hypercalcemic children. Results are as follows: a) Children with vitamin D intoxication (n = 2) had elevated serum 25-(OH)D and 24,25-(OH)2D concentrations but their 1,25-(OH)2D concentrations were similar to those found in normocalcemic children (10-110 pg/ml); b) Children with familial idiopathic hypercalcemia and hypocalciuria (n = 5), children with hypercalcemia and either Bartter's syndrome (n = 1), hemangiomatosis (n = 1), osteopetrosis after medullary graft (n = 1), also had…

Cytogenetic examinations on multiple peripheral blood cultures of a patient with papillary thyroid carcinoma and hypercalcemia revealed with following features: (1) The average frequency of cells with aberrations was 11.6%, considerably higher than in controls. Among metaphases with chromosomal abnormalities, 4.5% has chromosome-type aberrations. (2) One homolog of chromosome 11 showed a fragile site in the proximal end of the long arm, and in three metaphases the segment distal to the fragile site showed "branched morphology." (3) The rate of sister chromatid exchanges was within normal limit…

Heartburn is a frequent and sometimes initial complaint in hyperparathyroidism, and it is often relieved by successful parathyroid surgery. Four of five patients with primary hyperparathyroidism and heartburn obtained relief of symptoms and had an increase in lower esophageal sphincter pressure after successful operative treatment. Four of five volunteers undergoing calcium infusion exhibited a decrease in lower esophageal sphincter pressure after about 2.5 to 3 hours of infusion. Calcium infusion in a treated patient who had an increase in lower esophageal sphincter pressure postoperatively r

Differentiating between primary and ectopic or pseudohyperparathyroidism may be difficllt, but certain aspects of the patient's clinical history as well as laboratory tests may be helpful. We present an unusual case report of a patient who had massive splenomegaly secondary to a localized histiocytic lymphoma. On the basis of a lower serum parathormone level for a given serum calcium increase combined with normal serum phosphorous and chloride values, the diagnosis of psuedohyperparathyroidism was made and was confirmed when the patient's serum calcium level and temperature returned to normal

A case of severe hypercalcemia secondary to carcinoma of the lung is described in which hypokalemic alkalosis, renal failure and pancreatitis were also present. The relative importance of the few bone metastases found at autopsy is considered, and a probable endocrine-like effect of the tumour in the development of the hypercalcemia is postulated. Treatment of the hypercalcemia included administration of corticosteroids and disodium EDTA, peritoneal dialysis and subtotal parathyroidectomy; the most effective of these was peritoneal dialysis. Subtotal parathyroidectomy failed to produce a furth…

A 38-yr-old primigravida presented at 30 + 2 wk gestation with pregnancy-induced hypertension and was found to have severe hypercalcemia (adjusted calcium 3.19 mmol/L). Intravenous fluid therapy produced only transient improvement, and recurrent hypercalcemia required repeated inpatient management. PTH was suppressed, while both 25(OH)D₃ and 1,25(OH)₂D₃ (calcitriol) were elevated. Imaging and laboratory investigations revealed no evidence of malignancy or granulomatous disease. Delivery by elective cesarean section was undertaken at 35 wk gestation. The neonate developed transient hypocalcemia…

Hypercalcemia in infants is a rare but potentially serious condition characterized by elevated serum calcium levels. We report a case of a two-month-old female presenting with poor feeding, lethargy, irritability, and failure to thrive, and she was found to have hypercalcemia. On examination, she had a weight of 3.1 kg (-3.94 SDS) with normal facial features. Laboratory investigations revealed elevated serum calcium, low phosphate, suppressed parathyroid hormone (PTH), and high 1, 25-dihydroxyvitamin D levels. Renal ultrasonography revealed bilateral medullary nephrocalcinosis. Clinical exome …

Graves' disease (GD) is the leading cause of childhood hyperthyroidism, resulting from excessive thyroid hormone production. In some cases, it can cause alterations in mineral homeostasis, including calcium, phosphorus, and magnesium, which are often overlooked. Hyperthyroidism increases osteoclastic bone resorption, and mild to moderate hypercalcemia occurs in approximately 20-50% of affected patients, typically resolving with appropriate therapy. Although uncommon, symptomatic hypercalcemia in the setting of GD requires immediate evaluation and management. An 8-year-11-month-old girl was bro…

Background and aim Hypercalcemia is a recognized metabolic complication in patients with pulmonary tuberculosis (TB), attributed to extrarenal synthesis of 1,25-dihydroxyvitamin D (1,25(OH)₂D or calcitriol) by activated macrophages within granulomatous lesions. Biochemical characterization, including parathyroid hormone (PTH) measurement, is essential for excluding alternative etiologies. Serum calcitriol measurement is critical for directly confirming the postulated mechanism. The aim of the study was to determine the prevalence, severity pattern, biochemical profile, including serum calcitri…

Tirzepatide is a glucagon-like peptide-1 receptor (GLP-1R) agonist and glucose-dependent insulinotropic polypeptide receptor (GIPR) agonist that is used for diabetes mellitus and weight loss. Common adverse effects include gastrointestinal symptoms such as nausea and vomiting, pancreatitis, and acute kidney injury. Hypercalcemia is rarely reported in the literature and has previously been reported in patients taking GLP-1R agonists with concomitant thiazide diuretic or exogenous calcium supplement. We report a 70-year-old female with obstructive sleep apnea started on tirzepatide three months …

Milk-alkali syndrome is a condition caused by ingesting an excessive amount of calcium and absorbable alkali. It is described by the triad of elevated blood calcium levels, acute kidney injury, and metabolic alkalosis. While it was historically rare, it is starting to become more common due to the use of over-the-counter calcium-containing medications for the treatment of acid reflux and osteoporosis. We present a case of a 51-year-old male who presented with altered mental status and generalized weakness. He was noted to have severe hypercalcemia and acute kidney injury with a history of rece…
