Changes in total ghrelin within the somatotropic axis in severe burn patients: comparison of those with inhalation injury and those without inhalation injury.

OBJECTIVE: The aims of this study were to clarify changes in total ghrelin within the somatotropic axis in severe burn subjects with or without inhalation injury as well as the responsiveness of GH, IGF-1, and IGFBP-3 to the different severity of burn injuries. DESIGN: Twenty-three patients with severe burn injuries (>30% of 2nd degree burns or >10% of 3rd degree burns) were classified into 2 groups according to inhalation injury: group I with inhalation injury (n=9) and group II without inhalation injury (n=14). The evaluations of serum GH, IGF-1, IGFBP-3, and total ghrelin were done on post…

Takotsubo Syndrome Following Inhalation of K4: A Case Report.

Takotsubo syndrome (TS), also known as stress cardiomyopathy, is a clinical condition characterized by transient left ventricular dysfunction in the absence of coronary artery disease findings. The most widely accepted mechanism suggests a trigger, either physical or emotional stress, which causes catecholamine release and results in myocardial stunning and contractility alterations. Clinically and analytically, it can mimic acute coronary syndromes. Synthetic cannabinoids, such as the inhaled drug K4, are more powerful than phytocannabinoids and act as strong agonists of cannabinoid receptors

Inhalation Injury: Update on Pathogenesis, Emerging Hazards, and Management.

Inhalation injury, the pulmonary and systemic damage caused by a single or brief exposure to toxic gases, vapors, aerosols, or particulate matter, encompasses exposure categories ranging from structural-fire smoke and industrial chemical releases to wildfire smoke, earthquake-generated dust, and novel consumer products. Relevant exposures differ markedly in timescale, and four entities are distinguished: acute inhalation injury; subacute toxic pneumonitis, which follows repeated exposure peaks or sustained exposure over weeks to months; chronic inhalational disease; and population-level enviro…

Ethanol Inhalation for Respiratory Infections due to Enveloped Viruses.

Ethanol has demonstrated high efficacy in inactivating enveloped viruses in vitro and in vivo (in animal and human studies). The inhalation route has been a significant method of drug administration for respiratory disorders since ancient times. Infections with enveloped viruses cause many respiratory diseases. This concise review explores the general structural characteristics of enveloped viruses and examines the potential role of inhaled ethanol as a low-cost therapy for respiratory diseases. Current literature data suggest that ethanol inhalation could be beneficial in treating respiratory…

Sudden Death Induced by Acute Inhalation of Aerosolized Carfentanil.

Bilateral Inhalation of carfentanil (CRF) aerosol could rapidly induce death in humans and CRF aerosol exposure induces respiratory depression in animals; however, the dynamic progression of cardiorespiratory failure leading to sudden death induced by acute CRF aerosol exposure remains unclear. This study aimed to establish a rat model that allows dynamically characterizing the cardiorespiratory failure prior to death following acute exposure to a lethal concentration of CRF aerosol. Two groups of anesthetized and spontaneously breathing rats were exposed to aerosolized vehicle and CRF (4 mg/m…

Foreign Body Ingestion or Inhalation Management for Dental Offices.

The inadvertent ingestion or aspiration of dental instruments-particularly implant-related components-poses serious medical risks and legal implications. However, there is a lack of standardized protocols or professional guidelines for these adverse events. This article reviews current literature, highlights common preventive practices, and proposes a practical, step-by-step management algorithm. Emphasis is placed on early identification, emergency response, and, most importantly, appropriate referral to minimize patient harm. The authors, drawing from clinical and legal experience, advocate

Syncope and Methemoglobinemia Preceded by Amyl Nitrite 'Popper' Inhalation.

INTRODUCTION: Methemoglobinemia represents an uncommon but potentially serious cause of presentation to the emergency department, resulting in hypoxemia and even death. The symptoms and clinical findings in this condition can be nonspecific and therefore methemoglobinemia can be easily missed if the clinician is not familiar with it. This report presents a case caused by recreational drug use which has rarely been documented previously. CASE REPORT: A 23-year-old male with a history of asthma presents to the emergency department for an episode of syncope after inhalation of amyl nitrite "popp…

Nicotine Inhalation and Suicide: Clinical Correlates and Behavioral Mechanisms.

BACKGROUND AND OBJECTIVES: Extensive evidence links smoking and suicide independently of psychiatric diagnoses, but there are questions abou … rtionately to the number of cigarettes smoked per day. Rapid nicotine uptake into the brain through inhalation of conventional cigarettes, electronic cigarettes (e-cigarette), or even second-hand smoke can facilitate long-term sensitization and short-term impulsivity. Both impair action regulation and predispose to negative affect, continued smoking, and sui …

Asthmatic allergen inhalation sensitises carotid bodies to lysophosphatidic acid.

The carotid bodies are multimodal sensors that regulate various autonomic reflexes. Recent evidence demonstrates their role in immune reflex regulation. Our previous studies using the allergen (ovalbumin) sensitised and exposed Brown Norway rat model of asthma suggest that carotid bodies mediate asthmatic bronchoconstriction through a lysophosphatidic acid (LPA) receptor (LPAr)-protein kinase C epsilon (PKCε)-transient receptor potential vanilloid one channel (TRPV1) pathway. Whilst naïve carotid bodies respond to LPA, whether their response to LPA is enhanced in asthma is unknown. Here, we sh

Incidence of Laryngotracheal Stenosis after Thermal Inhalation Airway Injury.

Inhalation injury is independently associated with burn mortality, yet little information is available on the incidence, risk factors, or functional outcomes of thermal injury to the airway. In patients with thermal inhalation injury, we sought to define the incidence of laryngotracheal stenosis (LTS), delineate risk factors associated with LTS development, and assess long-term tracheostomy dependence as a proxy for laryngeal function. Retrospective cohort study of adult patients treated for thermal inhalation injury at a single institution burn critical care unit from 2012 to 2017. Eligible p…

Cerebellar infarction following accidental inhalation of toluene-mixed paint.

We report a case of atypical cerebellar infarction following accidental inhalation of toluene mixed paint. An unconscious 57-year-old housewife with hypertension arrived at our emergency department by ambulance. She had been rescued from a basement (30 m3) 12 h after exposure to paint containing toluene (34%). On arrival, she was comatose (E1V1M1) with a mild fever (37.4 °C). Physical examination showed chemical burns on her buttocks and rales on the left lung. Initial arterial blood gas with 15 L/min of oxygen showed a pH of 7.142, PCO2 of 47.3 mmHg, and PaO2 of 204.7 mmHg. She received endot…

Indications of early intubation for patients with inhalation injury.

AIM: For patients with inhalation injury, the indications for early intubation are diverse. The purpose of this study was to identify the most reliable symptoms, physical findings, and medical examinations with which to determine the indications for early intubation in patients with inhalation injury. METHODS: We retrospectively collected patient data from medical records. Collected data included age, sex, burn size, symptoms, physical findings, carboxyhemoglobin levels (COHb), and bronchial wall thickness (BWT) determined from chest computed tomography images. We analyzed the relationships b…

Protective Role of Eosinophils and TNFa after Ozone Inhalation.

INTRODUCTION: Exposure to ozone induces deleterious responses in the airways that include shortness of breath, inflammation, and bronchoconstriction. People with asthma have increased airway sensitivity to ozone and other irritants. Dr. Allison Fryer and colleagues addressed how exposure to ozone affects the immune and physiological responses in guinea pigs. Guinea pigs are considered a useful animal model for studies of respiratory and physiological responses in humans; their response to airborne allergens is similar to that in humans and shares some features of allergic asthma. Fryer and col

Inhalation of two putative Gulf War toxins by mice.

We employed our inhalation methodology to examine whether biomarkers of inflammation and oxidative stress would be produced in mice following inhalation of aerosols containing carbonaceous particles or the vapor of pesticides prevalent during the first Gulf War. Exposure to two putative Gulf War Illness toxins, fine airborne particles and the pesticide malathion, increased biomarkers of inflammation and oxidative stress in Friend virus B (FVB) female mice. Mice inhaling particles 24 h before had increased lung lavage and plasma Leukotriene B4 (LTB4) (a biomarker of inflammation) and PGF2α (a b…

Development of an inhalation unit risk factor for cadmium.

An inhalation unit risk factor (URF) was developed for cadmium. The URF is based on excess lung cancer mortality in a key epidemiological study of cadmium smelter workers (Park et al., 2012). The Park et al. (2012) study is an update of the Thun et al. (1985) cohort that was previously used to derive a URF in USEPA (1985). Park et al. re-analyzed the cadmium smelter worker population (near Denver, CO) using more detailed work history information, a revised cadmium exposure matrix, a detailed retrospective exposure assessment for arsenic (potential confounder), and updated mortality data (throu…

Fatal Intoxication with Toluene Due to Inhalation of Glue.

Two cases of fatal intoxications with toluene due to glue sniffing are described. In case 1, the autopsy did not indicate cause of death, while in case 2, the cause of death was determined to possibly be due to mechanical asphyxia by drowning. As the decedents had a history of glue sniffing, toxicological analyses were performed. Using gas chromatography with flame ionization detector and gas chromatography/mass spectrometry (GC/MS) with headspace method, toluene was detected in biological samples. Toluene ranged from 3.81 to 20.97 μg/g, with the highest concentrations observed in liver and br
