Advancing the science of Staphylococcus aureus infection prevention in the neonatal intensive care unit.

Despite advances in infection prevention, infections due to Staphylococcus aureus continue to be an important cause of morbidity and mortality in NICUs. Whole genome sequencing (WGS) using advanced analytic approaches for defining transmission have shed new insights into transmission and persistence patterns in the NICU. WGS has also helped uncover important associations between transmission, persistence, and the risk of invasive infections. Current infection prevention strategies rely on hand hygiene compliance and surveillance and decolonization of patients positive for S. aureus. Parent dec

Surveillance of antimicrobial resistance in neonatal intensive care unit of Alexandria University Children's Hospital.

Antimicrobial drugs are fundamental to modern medicine. The rise and spread of drug-resistant microorganisms jeopardize our ability to manage common infections and perform critical medical interventions. The present work aimed to assess and analyze antimicrobial resistance and sensitivity in the neonatal intensive care unit of Alexandria University Children's Hospital to improve antibiotic choices in the neonatal intensive care unit. A medical record-based study of 1105 neonates with sepsis admitted to the NICU during the four-year study period (2019 to 2022) was conducted, with no exclusion c

Candida auris colonization among critically ill neonatal intensive care unit patients in Dhaka, Bangladesh.

Candida auris is a multidrug-resistant yeast causing invasive infection and healthcare-associated outbreaks globally. We aimed to estimate the extent of C. auris colonization and infection among neonates in selected neonatal intensive care units (NICUs) in Dhaka City. We conducted a prospective study from August 2021 to September 2022. Skin swabs and blood samples were collected from NICU patients. C. auris was identified using CHROMagar and VITEK-2. Patient characteristics and healthcare histories were recorded. Of 374 patients enrolled, 32 (9%) were colonized with C. auris, and one (0.3%) de

Arterial Thrombosis Following Umbilical Artery Catheterization: Visualising Uncommon Neonatal Intensive Care Unit (NICU) Complications.

BACKGROUND: Right internal iliac artery (RIIA) thrombosis is an extremely rare but serious complication associated with umbilical artery catheter (UAC) malposition in neonates in the Neonatal Intensive Care Unit (NICU). Timely diagnosis and appropriate management are essential to prevent long-term sequelae. CASE REPORT: We present the case of a term neonate with hypoxic-ischaemic encephalopathy (HIE), who developed RIIA thrombosis secondary to UAC malposition, highlighting the role of bedside Doppler ultrasound in diagnosis and monitoring, and the success of a conservative therapeutic approac

Can pharmacogenetics inform morphine dosing in the neonatal intensive care unit? A retrospective evaluation.

Acutely ill infants in the neonatal intensive care unit (NICU) present with complex medical conditions necessitating individualized therapeutic strategies. These infants exhibit significant variability in gestational age, birth weight, organ maturity and drug metabolism, increasing the risk of adverse drug reactions. Morphine is commonly prescribed in the NICU and is associated with a wide range of dosing as well as a high risk of adverse events. The objective of this retrospective study was to explore the potential relevance of pharmacogenomics to morphine dosing for our local level III NICU.

Reduction of Vancomycin Use in a Neonatal Intensive Care Unit: A Quality Improvement Project.

INTRODUCTION: Late-onset sepsis (LOS) is a common cause of neonatal morbidity and mortality. Professional organizations recommend avoiding empiric vancomycin use in neonates without risk factors for methicillin-resistant Staphylococcus aureus infection. We aimed to reduce the mean vancomycin antibiotic utilization rate (AUR) by 30% for 12 months in our neonatal intensive care unit (NICU). METHODS: We included neonates admitted to our level-3 NICU from March 15, 2023, to February 29, 2024, with suspected LOS in the intervention period. A multidisciplinary team used the Model for Improvement. I

Tachycardia-Desaturation Episodes in Neonatal Intensive Care Unit Patients with and without Bronchopulmonary Dysplasia.

Much attention has been paid to measuring physiological episodes of bradycardia-oxygen desaturation (BDs) in the neonatal intensive care unit (NICU). NICU patients also have spells of tachycardia-desaturation (TDs), but these have not been well-characterized. We hypothesized that TDs would be more common among infants with bronchopulmonary dysplasia (BPD). We aimed to quantify daily TDs compared to BDs in NICU patients across a range of gestational and postmenstrual ages (GA and PMA) and determine whether TDs are associated with BPD.We analyzed every 2-second heart rate (HR) and peripheral sat

Viscoelastic coagulation testing in Neonatal Intensive Care Units: advantages and pitfalls in clinical practice.

The expression "developmental hemostasis" indicates the age-related physiological changes occurring during the maturational process of the hemostatic system. Despite the quantitative and qualitative alterations, the neonatal hemostatic system is competent and well-balanced. Conventional coagulation tests do not provide reliable information as they only explore the procoagulants during the neonatal period. In contrast, viscoelastic coagulation tests (VCTs), such as viscoelastic coagulation monitoring (VCM), thromboelastography (TEG or ClotPro), and rotational thromboelastometry (ROTEM), are poi

Set Up to Fail? Barriers Impeding Resident Communication Training in Neonatal Intensive Care Units.

PURPOSE: Learning to navigate difficult clinical conversations is an essential feature of residency training, yet much of this learning occurs "on the job," often without the formative, multisource feedback trainees need. To generate insight into how on-the-job training influences trainee performance, the perspectives of parents and health care providers (HCPs) who engaged in or observed difficult conversations with Neonatal Intensive Care Unit (NICU) trainees were explored. METHOD: The iterative data generation and analysis process was informed by constructivist grounded theory. Parents (n =

Hypertension in the Neonatal Intensive Care Unit (NICU): A Case of Mid-Aortic Syndrome.

A term baby was born with findings of edema, harsh murmur, and hypertension. Pregnancy course was complicated by hydrops fetalis. Upon birth, blood work did not reveal any abnormalities, but an echocardiogram showed patient in high-output heart failure. A computed tomography (CT) chest, abdomen, and pelvis revealed narrowing of aorta in the thoracic region to distal iliac and renal arteries, consistent with mid-aortic syndrome. Mid-aortic syndrome, which results in the narrowing of thoracic or abdominal aorta, is a rare cause of hypertension, especially in newborns. This case elucidates the im
