Fluoxetine in children and adolescents with mood disorders: a chart review of efficacy and adverse effects.

ABSTRACT The charts of 31 hospitalized children and adolescents (ages 9-18 years) with major mood disorders were retrospectively reviewed to examine the efficacy and side effects of treatment with fluoxetine. After treatment for a mean duration of 35 days, clinical improvement was seen in 74% of these patients; 54% had "much" to "very much" improvement as measured by the Clinical Global Impression scale (CGI). The most common adverse effects were hypomania-like symptoms (23%), irritability (19%), gastrointestinal upset (13%), and insomnia (13%). No EKG changes, blood pressure changes, antichol

Illness Uncertainty in Pediatric Irritable Bowel Syndrome: A Pilot Study.

BACKGROUND/OBJECTIVES: Illness uncertainty is inherent in irritable bowel syndrome (IBS); however, the factors that contribute to it and outcomes impacted by it have yet to be assessed in pediatric IBS. As such, the current longitudinal pilot study assessed the relationship between disease characteristics, illness appraisals, mood, and health-related outcomes. METHODS: 50 youth (ages 10-18) diagnosed with IBS completed self-report measures across three timepoints. Path analysis models specifying illness uncertainty as the mediator were constructed and assessed with Mplus using Bayesian Estima

A novel transgenic mouse for gene-targeting within cells that express corticotropin-releasing factor.

Corticotropin-releasing factor (CRF) orchestrates the mammalian endocrine, autonomic, and behavioral stress response and has been implicated in the pathophysiology of illnesses ranging from irritable bowel syndrome to mood and anxiety disorders. CRF is produced and released from a variety of cell types, making it difficult to distinguish the specific role of CRF from other neurotransmitters with which it colocalizes. To clarify the basic biology of the CRF neuron, we must be able to manipulate selectively CRFergic cells. Here we describe a novel transgenic mouse using 3.0 kb of the CRF promote

A preliminary study of increased amygdala activation to positive affective stimuli in mania.

OBJECTIVES: The present study in hypomanic and manic patients explored how amygdala responses to affective stimuli depend on the valence of the stimuli presented. METHODS: We compared 10 patients with 10 matched healthy control subjects. We measured blood oxygen level-dependent (BOLD) responses in the amygdala while subjects passively viewed photographs taken from the International Affective Picture System. After the fMRI session, subjects saw the pictures again and subjectively rated the emotional valence and intensity of each picture. RESULTS: Compared to healthy individuals, hypomanic or

Diagnosis and differential diagnosis of fibromyalgia.

Fibromyalgia is a chronic functional illness that presents with widespread musculoskeletal pain as well as a constellation of symptoms including fatigue, cognitive dysfunction, sleep difficulties, stiffness, anxiety, and depressed mood. The diagnosis of fibromyalgia, similar to other functional disorders, requires that organic diseases are not causing the symptoms. Systemic and rheumatic diseases can be ruled out by a patient history, physical examination, and laboratory investigations. Because there are no specific laboratory tests for fibromyalgia, the 1990 American College of Rheumatology (

Disruptive Mood Dysregulation Disorder Among Children and Adolescents.

Disruptive mood dysregulation disorder (DMDD) was introduced in the mood disorders section of DSM-5. Its primary symptoms are "severe, recurrent temper outbursts" (manifested verbally and/or physically) superimposed on and associated with chronic irritability. DMDD is a condition with an early age of onset (i.e., symptoms apparent by age 10, although the diagnosis cannot be made before age 6); however, nothing is known about the condition among adults in part because questions about temper outbursts (versus "often losing temper") are not consistently asked after 6 years of age. Other qualifier

Sensitivity to criticism and praise predicts schizotypy in the non-clinical population: The role of affect and perceived expressed emotion.

BACKGROUND: Schizotypy represents a cluster of personality traits consisting of magical beliefs, perceptual aberrations, disorganisation, and anhedonia. Schizotypy denotes a vulnerability for psychosis, one reason being psychosocial stress. High expressed emotion (EE), a rating of high criticism, hostility, and emotional over-involvement from a close relative, denotes psychosocial stress and vulnerability to psychosis, and is associated with schizotypy. This study aimed to decipher the relationship of schizotypy to perceived criticism and perceived praise in terms of affect and perceived EE.

The association of affective temperaments with smoking initiation and maintenance in adult primary care patients.

BACKGROUND: Smoking behaviour and its course is influenced by personality factors. Affective temperaments could allow a more specific framework of the role trait affectivity plays in this seriously harmful health-behaviour. The aim of our study was to investigate if such an association exists in an ageing population with a special emphasis on gender differences. METHODS: 459 primary care patients completed the TEMPS-A, Beck Depression Inventory (BDI) and Hamilton Anxiety Rating Scale (HAM-A). Subjects were characterized according to their smoking behaviour as current, former or never smokers.

Escitalopram and brain reactivity to aggressive stimuli in premenstrual dysphoric disorder.

BACKGROUND: Premenstrual dysphoric disorder (PMDD) is a condition linked to the menstrual cycle and characterised by cyclic emotional distress, irritability and anger, which can disrupt social functioning. Intermittent selective serotonin reuptake inhibitor treatment is effective in alleviating symptoms, yet the neural underpinnings of its efficacy in PMDD remain unknown. AIMS: This randomised, placebo-controlled trial aimed to evaluate the impact of intermittent escitalopram treatment on PMDD symptoms and aggressiveness, and on neural responses to social provocation. METHOD: Women with PMDD

Is Sertraline a Good Pharmacological Strategy to Control Anger? Results of a Systematic Review.

Introduction: Extensive research has made it possible to conclude that dysfunctions in serotoninergic transmission are associated with a tendency toward violence and behavioral dysregulations in humans. In this regard, it has been suggested that selective serotonin reuptake inhibitors (SSRIs), such as sertraline, which regulate the serotonin system, might reduce proneness to violence. Aims: This review aims to explore changes in feelings of anger-state (e.g., irritability and hostility) and anger expression as primary outcomes after sertraline treatment. Methods: Based on PRISMA quality criter

Pain and irritability in Pediatric ileocolic and small bowel intussusception: A comparative study.

BACKGROUND: Ileocolic intussusception (ILIC) represents a medical emergency in pediatric emergency medicine. Some patients suspected of ILIC actually have small bowel intussusception (SMBI), which may appear clinically similar and can only be differentiated from the ileocolic type by ultrasound. The study aimed to compare pain and irritability among children presenting with ILIC versus SMBI. METHODS: This retrospective cohort study included children aged 4 months to 4 years diagnosed with ILIC or SMBI at two tertiary pediatric EDs between January 2021 and May 2025. Pain was assessed at triage
