Management of obstructive jaundice induced by a retained bullet in the common hepatic duct: A case report.

BACKGROUND: Traumatic injuries to the extrahepatic bile ducts are rare, with an incidence of 0.4%-0.6% in cholecystectomy procedures among adults, particularly following the introduction of laparoscopic cholecystectomy. Among these, the presence of foreign bodies within the biliary tree is exceptionally rare, with obstructive jaundice caused by a bullet lodged in the common hepatic duct being particularly uncommon. This case report aims to share the diagnostic process and the challenges in managing such a rare condition. CASE PRESENTATION: A 41-year-old female with a 13-year history of an acc

Intrabiliary Ascariasis: A Case Report of an Uncommon Cause of Acute Abdomen and Obstructive Jaundice in Children.

BACKGROUND: Intrabiliary ascariasis is an uncommon cause of biliary colic and obstructive jaundice among children due to the small size of the ampullary orifice. A high index of suspicion for patients living in an endemic area and radiologic examination are crucial for diagnosis and treatment. CASE PRESENTATION: A 12-year-old male Ethiopian child presented with colicky right upper quadrant pain, nausea, and vomiting for 3 days. Physical examination showed slightly icteric sclera and slightly tender hepatomegaly. Laboratory and ultrasound examinations were suggestive of obstructive jaundice se

A case of obstructive jaundice caused by metastasis of breast cancer to the intra/extrahepatic bile duct.

A 38-year-old woman was admitted to our hospital for a detailed examination of jaundice. Three years before, she had undergone a right total mastectomy and axillary lymph node dissection of her right breast because of cancer. Histopathological evaluation revealed invasive ductal carcinoma. Postoperatively, because multiple bone metastases were found, she underwent chemoradiotherapy. Endoscopic retrograde cholangiopancreatography was performed, which revealed widespread multiple stenoses with a smooth surface from the intrahepatic to the extrahepatic bile duct. A transpapillary biliary biopsy w

The role of percutaneous transhepatic biliary biopsy in the diagnosis of patients with obstructive jaundice: an initial experience.

OBJECTIVE: To evaluate the accuracy of percutaneous transhepatic biliary biopsy (PTBB) in patients with suspected biliary obstruction. MATERIALS AND METHODS: This was a retrospective analysis of 18 patients with obstructive jaundice who underwent PTBB. In each patient, three to ten fragments were collected from the lesion. The final diagnosis was confirmed in the pathology report. We also reviewed analyses of the results of laboratory tests performed before the procedure, as well as the Bismuth classification, clinical outcome, complications occurring during the procedure, access route, and m

Obstructive jaundice due to a blood clot after ERCP: a case report and review of the literature.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is one of the most frequently performed procedures for the treatment of biliary-pancreatic diseases. The most frequent complications of ERCP include pancreatitis, haemorrhage, perforation and cholangitis. While post-ERCP biliary bleeding leading to biliary obstruction is rare. CASE PRESENTATION: We herein report a case of exceptional post-ERCP cholangitis due to a blood clot in the common bile duct (CBD). This case involves a 75-year-old woman with a history of recurring upper abdominal pain. Abdominal computerized tomography (

[The surgical tactics of treatment of the choledocholithiasis, complicated by the obstructive jaundice, in patients with the altered bilioduodenal anatomy].

The treatment results of 69 patients with the altered bilioduodenal anatomy and choledocholithiasis, complicated by the obstructive jaundice, were analyzed. The anatomic changes were determined by the previous gastric resection or gastrectomy, biliodigestive anastomosis, bile duct strictures, pyloric stenosis, duodenal diverticulum or the Mirizzi syndrome. The surgical approach depended on the type and extent of anatomic changes. The endoscopic common bile duct decompression was possible in 82,6% of patients. The endoscopic bile duct stone removal was achieved only in 44,9% of patients, the ot

Endoscopic stenting in obstructive jaundice due to liver metastases: does it have a benefit for the patient?

BACKGROUND/AIMS: The goal of this report is to describe our experience with the palliative endoscopic treatment of jaundice occurring in the setting of liver and hilar metastases of a distant primary malignancy. MATERIAL AND METHODS: We retrospectively analyzed the clinical course of 29 consecutive patients with metastatic tumors not originating in the hepatobiliopancreatic area, who were treated by endoscopic retrograde cholangiopancreatography with endoprostheses insertion. RESULTS: We achieved a complete follow-up in 24 out of the 29 patients (11 women, median age 69 years). The primary t

A simple method for palliative diversion of the bile in patients with obstructive jaundice due to perihilar tumors.

A palliative procedure for diversion of bile in patients with obstructive jaundice due to nonresectable perihilar tumors is described. The procedure is accomplished by creating an internal fistula between a bile duct and the gallbladder and an anastomosis of the gallbladder to the jejunum. The procedure is simple and bleeding of the hepatic parenchyma is readily controlled and subsequently, there is a rapid fall in the bilirubin level.

[A case of obstructive jaundice caused by MALT lymphoma of the common bile duct and the papilla of Vater].

We report a rare case of mucosa-associated lymphoid tissue (MALT) lymphoma of the common bile duct and the papilla of Vater. A 66-year-old man was admitted to our hospital with right upper quadrant pain and jaundice. Endoscopic retrograde cholangiopancreatography and magnetic resonance cholangiopancreatography showed common bile duct stenosis. Further endoscopy revealed swelling of the papilla of Vater, with erosion at the ileal end. The patient was pathologically diagnosed with MALT lymphoma and treated for Helicobacter pylori eradication, after which the lesions improved. However, 1 year lat

Successful management of obstructive jaundice due to gallstones with eculizumab in a patient with paroxysmal nocturnal hemoglobinuria.

Paroxysmal nocturnal hemoglobinuria (PNH) makes patients susceptible to intravascular hemolysis and thrombosis, and it can be life-threatening in stressful situations. Eculizumab, a humanized monoclonal antibody that inhibits the complement protein C5, has been evaluated as a novel therapy for PNH. We herein describe the case of a 59-year-old Japanese woman with classic PNH, who had been successfully treated with eculizumab, but who later developed acute cholecystitis/cholangitis from gallstones. Although the severe obstructive jaundice requiring endoscopic therapy following cholecystectomy wa

Obstructive jaundice due to a pancreatic mass: a rare presentation of acute lymphoblastic leukaemia in an adult.

CONTEXT: To highlight a rare presentation of acute lymphoblastic leukaemia. CASE REPORT: A 39-year-old man presented with a 4 month history of weight loss and a 6 week history of upper abdominal pain radiating to the back with nausea and vomiting. Liver function tests showed an obstructive picture, full blood count was normal and on computerised tomography there was diffuse enlargement of the pancreas, with dilatation of the common bile duct and intra hepatic biliary radicles. Four weeks after presenting, the white cell count became elevated with blasts on the blood film and bone marrow biops

Giant Brunner's hamartomas of the duodenum and obstructive jaundice. An overview of the literature and suspicion of malignancy in a case.

BACKGROUND/AIMS: 150 cases of Brunner's gland hamartoma (BGH) have been reported in the literature. BGHs are benign and are thought not to cause bile obstruction. METHODS: In this case report, a 60-year-old male is presented with unexplained obstructive jaundice who was also known for over 17 years with diffuse adenomatous hyperplasia of Brunner's glands in the duodenum. Despite the benign preoperative diagnosis, the choice of treatment was Whipple's procedure due to suspicion of a coexisting malignancy. RESULTS: Pathological analysis of the resection specimen revealed multiple BGHs and an a

Intermittent dislocation of the liver. A syndrome associated with volvulus of the transverse colon and stomach and obstructive jaundice.

Displacement of the right lobe of the liver into the left upper abdominal quadrant can occur only if the diaphragmatic attachments are congenitally absent or traumatically disrupted. To our knowledge, the former situation has not been previously described in an adult patient. A young man with a lifelong history of unexplained intermittent, bizarre upper abdominal pain was found to have a freely movable right lobe of the liver that was attached only to the inferior vena cava. When this lobe was intermittently displaced into the left upper abdomen, volvuli of both the stomach and transverse colo
