Ulcerating plantar keratoderma in association with systemic lupus erythematosus.

This report highlights the finding of ulcerative plantar keratoderma in two patients with systemic lupus erythematosus (SLE). Both patients suffered from painful plantar ulcerations and fissures; in one patient there was diffuse desquamation over the entire plantar surface, while the other patient's lesions were focal and accentuated over weight-bearing surfaces. Other etiologies for keratoderma including papulosquamoua disease, contact dermatitis, tinea and primary keratodermas were excluded. Both patients were resistant to multiple topical therapies including super-potent topical corticoster

Palmar spiny keratoderma associated with type IV hyperlipoproteinemia.

We report the case of a 35-year-old man with acquired palmar spiny keratoderma (SK) in association with type IV hyperlipoproteinemia. Cutaneous lesions appeared 11 years before with no history of any previous skin conditions, topical medication or systemic treatment with Vitamin A-derived drugs, x-irradiation, arsenic poisoning or prolonged sun exposure. Family aggregation for SK or other disorders of keratinization was ruled out. Blood chemistries demonstrated hypertriglyceridemia (422 mg/dl) and elevation in plasma concentration of the pre-beta fraction (VLDL) of lipoproteins pattern compati

Keratosis punctata palmaris et plantaris as an expression of focal acantholytic dyskeratosis.

An 11-year-old girl affected by keratosis punctata palmaris et plantaris, histologically showing focal acantholytic dyskeratosis, is described. This case demonstrates that keratosis punctata palmaris et plantaris may represent a new clinical expression of persistent multiple focal acantholytic dyskeratosis.

Hyperkeratosis palmoplantaris (Papillon-Lefèvre syndrome). A case report.

A 16-year-old female presented with a severe chronic periodontitis and with associated hyperkeratotic lesions of her palms and soles. Investigations revealed it as a case of hyperkeratosis palmoplantaris (Papillon-Lefèvre syndrome), which is a rare form of an ectodermal dysplasia. Previously it was speculated that this disease entity is not all that uncommon in Nigeria especially amongst the rural population. However, recent investigations revealed that this is not correct.

Acanthosis palmaris in a patient with bullous pemphigoid.

Acanthosis palmaris, a recently described dermatologic entity, is often associated with internal malignancy. We report the development, disappearance, and reappearance of this condition in a patient with bullous pemphigoid. Relevant screening tests did not reveal any internal malignant neoplasm. The significance of the appearance and disappearance of the condition is discussed.

Palmoplantar hyperkeratosis in mycosis fungoides.

The association of palmoplantar hyperkeratosis and mycosis fungoides (MF) has been described less frequently in the dermatologic literature. We present a patient with MF who developed hyperkeratosis of the palms and soles. Histologic examination of a specimen from the left palm showed microscopic changes of MF.

The benign acanthomas.

An important, yet neglected, problem in dermatopathology, is the evaluation of the benign acanthomas, the benign tumors of epidermal keratinocytes. The benign acanthomas may be simulated by lesions which are not benign (e.g. actinic keratosis), not tumors (e.g. normal plantar skin), or are not epidermal (e.g. dermatofibroma). In addition to normal (epidermoid) keratinization (e.g., seborrheic keratosis and related conditions), the variants of the benign acanthomas show a wide range of aberrant keratinization, including epidermolytic hyperkeratosis (epidermolytic acanthoma), dyskeratosis (warty

[Delayed manifestation of Costa's acrokeratoelastosis].

Acrokeratoelastoidosis, first described in 1952 bei O.G. Costa, belongs to the group of diffuse palmoplantar keratoses without associated symptoms. Probably it is an autosomal dominant trait. We observed a 49 year old female who has noticed her typical skin lesions for 2 years. The most marked histological features are proliferative hyperkeratosis, an almost complete lack of the fine subepithelial elastic fibre network and a marked rarefication of the coarse elastic fibres in the dermis.

Pipillon-Lefèvre syndrome: report of two familial cases.

Two cases with Papillon-Lefèvre Syndrome (PLS) are reported. They are two brothers belonging to a family which there is strong consanguinity. An autosomal recessive pattern of inheritance is suggested. Some pathological findings as congenital bronchiectasis observed in one of the patients as well as the PLS features are discussed and considered to be fortuitous defects. These observations are compared with those described in the literature.

Sezary syndrome with arthropathy. Report of a case.

A 65-year-old black female with Sezary syndrome had generalized intractable pruritus, erythroderma, alopecia, onychogryphosis, lumphadenopathy and hepatomegaly. Abnormal lymphocytes with large, convoluted and grooved nuclei (Sezary cells) were identified in the skin and peripheral blood. A striking feature of her disease was severe, deforming arthropathy of the hands and knees, a clinical finding which has been described previously in only one patient with Sezary syndrome. At necropsy no associated lymphoma was found.

Mutations in ASPRV1 Cause Dominantly Inherited Ichthyosis.

The discovery of genetic causes of inherited skin disorders has been pivotal to the understanding of epidermal differentiation, function, and renewal. Here we show via exome sequencing that mutations in ASPRV1 (aspartic peptidase retroviral-like 1) cause a dominant Mendelian disorder featuring palmoplantar keratoderma and lamellar ichthyosis, a phenotype that has otherwise been exclusively recessive. ASPRV1 encodes a mammalian-specific and stratified epithelia-specific protease important in processing of filaggrin, a critical component of the uppermost epidermal layer. Three different heterozy

Genetic variants in pachyonychia congenita-associated keratins increase susceptibility to tooth decay.

Pachyonychia congenita (PC) is a cutaneous disorder primarily characterized by nail dystrophy and painful palmoplantar keratoderma. PC is caused by mutations in KRT6A, KRT6B, KRT6C, KRT16, and KRT17, a set of keratin genes expressed in the nail bed, palmoplantar epidermis, oral mucosal epithelium, hair follicle and sweat gland. RNA-seq analysis revealed that all PC-associated keratins (except for Krt6c that does exist in the mouse genome) are expressed in the mouse enamel organ. We further demonstrated that these keratins are produced by ameloblasts and are incorporated into mature human ename

A mutation detection strategy for the human keratin 6A gene and novel missense mutations in two cases of pachyonychia congenita type 1.

Pachyonychia congenita type 1 (PC-1) is an autosomal dominant ectodermal dysplasia characterized by hypertrophic nail dystrophy, focal non-epidermolytic palmoplantar keratoderma and variable features of oral leukokeratosis and follicular keratosis. Previously, we have shown that this disease can be caused by mutations in type I keratin K16 and one mutation has been reported in its type II keratin expression partner, K6a. Mutation analysis for K6a has been hampered by the presence of multiple copies of the K6 gene in the human genome, of which some are expressed and others are pseudogenes. Here

CYP4F22-Related Autosomal Recessive Congenital Ichthyosis Associated With Hirschsprung Disease and Bartter-Like Renal Manifestations.

Autosomal recessive congenital ichthyosis (ARCI) is a heterogeneous group of inherited cornification disorders caused by defects in epidermal barrier formation. Mutations in CYP4F22 are an uncommon cause of ARCI and are associated with variable phenotypes including lamellar ichthyosis (LI) and congenital ichthyosiform erythroderma (CIE). This is a report of a genetically confirmed case of CYP4F22-related ARCI in a child born to consanguineous parents who presented with collodion membrane at birth followed by persistent ichthyosiform scaling and palmoplantar keratoderma. Genetic analysis identi

Papillon-Lefèvre Syndrome: Case Report of Two Sisters.

UNLABELLED: Papillon-Lefèvre syndrome (PLS), also known as keratosis palmoplantaris with periodontopathia and "hyperkeratosis palmoplantaris with periodontosis," is a rare autosomal recessive disorder, characterized by diffuse palmoplantar keratoderma and precocious aggressive periodontitis, leading to premature loss of deciduous and permanent dentition at a very young age. The aim of this article is to revisit PLS, address its diagnosis update and dental management, and to provide insight into the fascinating role of consanguinity in the etiology of this unusual illness. We report the case of

Biallelic pathogenic variants in the LSS gene cause congenital alopecia-cataract syndrome.

Biallelic variants in the LSS gene have been reported in individuals affected by alopecia-intellectual disability syndrome 4, cataract 44, hypotrichosis 14, and palmoplantar keratoderma-congenital alopecia syndrome type 2. The present report described a Chinese girl with congenital alopecia universalis, cataract, esotropia, and nystagmus caused by compound heterozygous variants of c.1025T>G (p.Ile342Ser) and previously unreported c.1011G>A (p.Pro337=) in the LSS gene. Minigene assay confirmed the synonymous variant Pro337= at the edge of exon 9 could produce a novel splice site, leading to a 4

Desmosomal genodermatoses.

Desmosomes are intercellular junctions that contribute to cell-cell adhesion, signalling, development and differentiation in various tissues, including the skin. Composed of a network of transmembranous and intracellular plaque proteins, pathogenic autosomal dominant or recessive mutations have been reported in 10 different desmosomal genes, resulting in a spectrum of phenotypes variably affecting skin, hair and heart. This review summarizes the molecular pathology and phenotypes that predominantly affect the skin/hair. Recent desmosomal genodermatoses described include lethal congenital epide
