Exchange transfusion and its morbidity in ten-year period at King Chulalongkorn Hospital.

The objectives of this study were to consider the rate of exchange transfusion (ET) in the newborns at King Chulalongkorn Hospital, Bangkok, … 8). ET causes high morbidity, therefore, it should be initiated only when the benefit of preventing kernicterus outweighs the complications associated with the procedure.

Clinical factors related to bilirubin encephalopathy in preterm infants: A case-control study.

OBJECTIVES: Bilirubin encephalopathy in preterm infants (pBE) is becoming a growing concern in Japanese neonatal intensive care units. Definitive conclusions regarding the risk factors of pBE remain elusive. This study aimed to identify the risk factors for pBE. METHODS: We performed a 1:2 matched-pair analysis, selecting infants with pBE from previous nationwide surveys and matching them with control infants without evident neurodevelopmental delay based on year of birth, gestational age, or birth weight. We compared demographic data, neonatal complications, laboratory data within the first

[Differentiation of hypoxic-ischemic encephalopathy and acute bilirubin encephalopathy with magnetic resonance imaging in neonates].

OBJECTIVE: To investigate the value of conventional magnetic resonance imaging (MRI) and diffusion weighed imaging (DWI) in the differentiation of hypoxic-ischemic encephalopathy (HIE) and acute bilirubin encephalopathy in neonates. METHODS: The MRI findings along with DWI characteristics in 15 neonates with HIE involving basal ganglia and in 18 neonates with acute bilirubin encephalopathy between November 2006 and June 2008 were retrospectively reviewed. RESULTS: On T1WI, only 5 patients presented hyperintensity in the globus pallidus in the HIE group, but 16 in the acute bilirubin encephal

Secondary causes of paroxysmal dyskinesia.

PxD are sudden, episodic, involuntary movement disorders that may include any combination of dystonia, chorea, athetosis, or ballism. The majority of reported cases are familial or idiopathic; however, there have been several reports of secondary PxD. We report 20 new cases of secondary, non-psychogenic PxD, and review 130 cases reported in the literature. The results suggest that although PxD is a rare disorder, secondary forms may be more common than previously recognized, accounting for 26% of all cases in our series. Secondary cases are notable for their variability in age of onset, the pr

The risk of bilirubin encephalopathy in neonatal hyperbilirubinemia.

Jaundice is the most common and one of the most annoying problems that can occur in the newborn. Although most jaundiced infants recover without any serious problem, there is always a risk of bilirubin encephalopathy during the period of hyperbilirubinemia. The relationship between encephalopathy and serum free bilirubin levels was investigated in 83 newborn infants (40 premature, 43 mature) with unconjugated hyperbilirubinemia. A complete physical examination was done in all patients, and signs of bilirubin encephalopathy were noted if present. The serum free bilirubin level exceeded 0.1 mg/d

[The risks of pyrimethamine-sulfadoxine combination in the prenatal treatment of toxoplasmosis].

Some of the alternative treatments to avoid termination of pregnancy in cases where the fetus is affected by toxoplasmosis is to treat it as soon as the diagnosis has been made. The authors who already have experience of using pyrimethamine with sulfadoxoine (Fansidar) in the post-natal treatment of congenital infection, thought after reviewing the literature that this association of drugs would be harmless if applied during pregnancy. The principal risk that arises in the fetus is the teratogenicity of each of the components of pyrimethamine and sulfadoxine and also their associations. In ani

Glucose-6-phosphate dehydrogenase deficiency in Chinese.

In a Chinese population 1,000 full-term male neonates and a further 117 jaundiced neonates of both sexes were studied in an investigation of the frequency of deficiency of erythrocyte glucose-6-phosphate dehydrogenase (G6PD). This enzyme was found to be deficient in 3.6% of male neonates. Correlation of the results with the birthplace of the 602 mothers who were known to come from Kwangtung province showed no significant differences in the frequency of the deficiency between certain parts of the province. The deficiency of G6PD in hemizygous males is profound but it is not associated with eryth

Altered glucose metabolic pattern in basal ganglia: 18F-FDG PET/CT semi-quantitative analysis of metabolic signatures in neonatal bilirubin encephalopathy.

Neonatal bilirubin encephalopathy arises from unconjugated bilirubin deposition in the brain. Conventional tests are limited: serum bilirubin does not reflect intracerebral load, and MRI T1 changes are late and confounded by myelination. We evaluated whether 18F-FDG PET detects early metabolic abnormalities and monitor progression. Twelve Sprague-Dawley rat were randomly assigned to bilirubin-exposed or control groups. The bilirubin-exposed group received intraperitoneal bilirubin from postnatal day 3. 18F-FDG PET/CT was performed on days 7 and 21, and regional standardized uptake values and c

Functional outcomes of children with dyskinetic cerebral palsy depend on etiology and gestational age.

OBJECTIVE: To elucidate the differences in etiology of dyskinetic cerebral palsy (DCP) between term-born and preterm-born children and its relationship to functional outcomes. METHODS: We determined the etiology of DCP based on the clinical course and brain MRI of 163 term-born and 136 preterm-born children. Information about genetic abnormality was also collected if available. Functional outcomes were compared between the two major etiologies in each group, i.e., hypoxic ischemic encephalopathy (HIE) and bilirubin encephalopathy (BE), using four standardized classification systems, i.e., Gro

The Turkish Neonatal Jaundice Online Registry: A national root cause analysis.

BACKGROUND: Neonatal jaundice (NNJ) is common, but few root cause analyses based on national quality registries have been performed. An online registry was established to estimate the incidence of NNJ in Turkey and to facilitate a root cause analysis of NNJ and its complications. METHODS: A multicenter prospective study was conducted on otherwise healthy newborns born at ≥35 weeks of gestation and hospitalized for only NNJ in 50 collaborator neonatal intensive care units across Turkey over a 1-year period. Patients were analyzed for their demographic and clinical characteristics, treatment op

Exchange transfusion in infants with extreme hyperbilirubinemia: an experience from a developing country.

OBJECTIVE: To determine the clinical and epidemiological features of infants with extreme hyperbilirubinemia who require exchange transfusion (ET). STUDY DESIGN: Term and near-term infants admitted to a paediatric hospital over a 5-year period in La Paz, Bolivia, were included in the study if they met the following criteria: age less than 30 days; total serum bilirubin (TSB) levels > or =428 micromol/L (> or =25 mg/dL) on admission and if they had undergone an ET. RESULTS: Fifty-six infants were identified during the study period. Most of them were exclusively breastfed (98.2%). Median age a

Effect of bilirubin on the activity and thermokinetic characteristics of brain (synaptosomal) membrane NO synthase.

NO synthase activity was found in the plasma (synaptosomal) membrane particles isolated from the homogenate of adult rat brain (without cerebellum) under conditions preventing the protease attack and formation of reactive oxygen species. The NO synthase discovered exhibited some properties of a neuronal constitutive integral membrane enzyme and was inhibited by N-nitro-L-arginine. NO synthase activity decreased when bilirubin entered the synaptosomal membrane in vitro. Bilirubin caused the shift of the transition temperature in the temperature dependence of NO synthase activity in Arrhenius pl

Universal Newborn Screening for Glucose-6-Phosphate Dehydrogenase (G6PD) Deficiency in a Safety-Net Well-Baby Nursery: A Quality Improvement Initiative.

Background: G6PD deficiency is among the leading causes of neonatal hyperbilirubinemia and kernicterus. Following a 2022 New York State Department of Health recommendation to test high-risk neonates, we implemented universal G6PD deficiency screening in the well-baby nursery. Objectives: We aimed to increase the proportion of infants screened from 0% to more than 75% within 6 months and to describe the prevalence of G6PD deficiency and the early outcomes of affected neonates. Methods: This quality improvement (QI) initiative, guided by the Model for Improvement, included a cross-sectional anal…
