Atrial fibrillation is a typical disturbance of heart rate in patients with mitral valvular disease. Correction of the mitral valvular disease by operation of replacement (or plasty) of the mitral valve normalizes the intracardial hemodynamics resulting in clinical improvement of the patient's state and prognosis for the nearest and long-term period. Atrial fibrillation aggravates the course of the heart disease, is a substantial factor associated with thromboembolic complications. The heart rate dynamics after mitral valve replacement was assessed in 94 patients. The sinus rhythm in patients a

A series of 19 patients with a labyrinthine fistula caused by cholesteatoma was analysed. Two patients presented with acute suppurative labyrinthitis and meningitis. A canal wall down procedure was performed in all but one patient. Of the 17 patients with preserved inner-ear function, the cholesteatoma matrix over the fistula was removed in 10 resulting in severe sensorineural hearing loss in one of these. The matrix was left in situ in 7 patients and hearing was maintained or improved in all of them. Post-operatively, 2 patients suffered from vestibular disturbances, the matrix covering the f

We report the ultrastructural pathology of myelinated axons in mice infected experimentally with the Fujisaki strain of Creutzfeldt-Jakob disease (CJD). Initially the myelin sheath was separated into several concentric bands, and cellular processes penetrated between layers of myelin and lifted away the outermost lamella. Then a complicated labyrinth of the concentric cellular processes, clearly belonging to either astrocytes or macrophages, invested myelinated axons. In terminal stages axons completely denuded of myelin were seen in the center of concentric networks of cellular processes. Mye

The effects of high frequency ultrasound on the inner ear structures after irradiation of the lateral semicircular canal have been analysed by means of light-and electronmicroscopy. Sudden as well as gradual morphological changes in the vestibular sensory epithelium are described. Damage to the connective tissue stroma, blood vessels and nerve fibres characterized the chronic phase. Both cochlear morphology and hearing remained normal after irradiation of some behaviorally conditioned animals. The temperature rise in the labyrinth during continuous irradiation was studied. The experiments indi

The diagnostic pathway in a patient with vertigo starts with the accurate evaluation of medical history followed by a general physical and neurological examination. This step can often lead to the identification of the correct cause of the disease or, at least, to a distinction between peripheral and central vertigo. Neuroradiological investigations have to be considered as elective diagnostic procedures and include: computed tomography (CT), magnetic resonance (MR), MR angiography (MRA), angiography. For the diagnosis of peripheral vertigo, benign paroxysmal positional vertigo, labyrinthitis,

The occurrence of monosymptomatic courses of Menière's disease is not generally accepted, although cochlear forms are described in literature. Retrospectively, we evaluated the case histories of 108 patients with Menière disease, who developed the typical trias in the further course of the disease. Among these 108 patients, we found 39 with a true monosymptomatic course, i.e., in each case we could exclude the participation of either the cochlea, or the labyrinth by exact clinical investigations. Two-thirds of the patients first noticed the impairment of hearing, one-third the vertigo. Basing

Recently Minor and co-workers described patients with sound- and pressure-induced vertigo due to dehiscence of bone overlying the superior semicircular canal. Identifying patients with this "new" vestibular entity is important, not only because the symptoms can be very incapacitating, but also because they are surgically treatable. We present symptoms and findings for three such patients. On exposure to sounds, especially in the frequency range 0.5-1 kHz, they showed vertical/torsional eye movements analogous to a stimulation of the superior semicircular canal. They also showed abnormally larg

The aim of our work was the presence of the second phase nystagmus after rotatory, caloric and optokinetic irritation; estimation of this nystagmus magnitude and the clinical application of the second phase nystagmus. The investigation was performed in 30 healthy subjects and 65 cases of the peripheral and central vestibular pathology. Our results show that the second phase nystagmus appears more often after strong vestibular or visual irritation; the largeness of this reaction is bigger than the first reaction nystagmus. Only the asymmetry of the second phase nystagmus has the clinical apply.

Quantitative analysis of electro-oculographic recordings of eye movement in response to precise visual and vestibular stimuli makes possible the differentiation of three categories of vestibular syndromes due to pathological changes in three different parts of the visual vestibulo-ocular reflex arc: (1) decreased vestibulo-ocular reflex gain (e.g., decrease in slow component velocity), but normal fast components and visual-vestibular interaction (labyrinth and eighth nerve); (2) normal slow component velocity but abnormal fast components to all stimuli (pontine or medullar reticular formation)

Pneumolabyrinth is a condition in which air is present in the vestibule and/or in the cochlea. Air inside the inner ear structures is uncommon, and is not detected even in otic capsule-violating fractures or in transverse fractures of the temporal bone. It is rarely described in the literature. We present a longitudinal computed tomography (CT) study of a significant pneumolabyrinth due to temporal bone trauma in a 31-year-old man. Routine CT of the cranium performed 3 hours after the accident showed air inside the vestibule (pneumolabyrinth). Two days later, a new CT study was performed. The

Vestibular sensory organs were examined ultrastructurally in two cases of delayed endolymphatic hydrops. The patients, two women, 27 and 16 years of age, suffered profound sensory hearing loss in childhood and experienced severe, recurrent vertigo. Specimens were obtained by a translabyrinthial vestibular nerve transection. We studied the utricular macula and the lateral and posterior cristae in one case, and the utricular macula, the anterior and posterior cristae, and the vestibular ganglion in the other. The otoconia and the otoconial membrane, the sensory epithelia, and the vestibular gang

Acute and persistent viral infection of the inner ear of rats was induced with hemorrhagic fever with renal syndrome (HFRS) virus following intraperitoneal inoculation. Newborn (within 24 h after birth) rats were inoculated intraperitoneally (i.p.) with HFRS virus (B-1 strain), and the cochlea was examined using an immunohistochemical technique for the localization of viral antigens 1 week (acute infection) and 1 year (persistent infection) after infection. Virus-specific antigens were detected in the inner ear structures of the rats suffering from both acute and persistent infection. Morpholo

Dizziness as defined herein will include an illusion of motion caused by various degrees of ischemia to the vestibular pathway or its interconnecting pathways. "Syndrome," such as the lateral medullary syndrome, denotes a macroinfarct, while a microinfarct or an area of incomplete infarct (where there may develop an incomplete degeneration of the neural tissue secondary to the arteriolar microatheromatous stenosis) may cause only one neurologic deficit, such as dizziness per se as the only symptom. However, the latter may presage a larger and more debilitating neurologic deficit. The transcran
