Reconstruction of the popliteal artery after war injury.

During the war against Croatia in 1991-1992, 85 wounded with war injuries of major vessels in the extremities were admitted to the Department of Surgery, Osijek Clinical Hospital. These 85 included 13 (15.3%) wounded with injuries of the popliteal artery. In 12 cases an allograft was used as a substitute conduit. One early thrombosis and two infections occurred. Two amputations had to be performed because of massive soft tissue destruction and infection in the leg tissue. Our experience shows that an allograft is an acceptable choice for popliteal artery reconstruction in a highly contaminated

Neuromuscular complications of the lower extremity after thrombectomy in a patient with superficial femoral artery occlusion: Case series.

BACKGROUND: Ischemia reperfusion (IR) injury may result in rhabdomyolysis and compartment syndrome when blood supply returns after thrombectomy for patients with acute limb ischemia. OBJECTIVE: We highlight the value of early diagnosis and treatment in post-thrombectomy patients with IR injuries in their lower legs. CASE DESCRIPTION: Two patients received thrombectomy due to left superficial femoral artery occlusion. Both patients complained of left calf pain during ambulation at the 1- and 3-day follow up post-thrombectomy, as well as a heating sensation, swelling, weakness, and sensory cha

An individualised approach to assess the sidestep manoeuvre in male rugby union athletes.

OBJECTIVES: This study aimed to (1) quantitatively assess external knee abduction moments between legs, and (2) qualitatively assess anterior cruciate ligament injury risk between group mean and individual athlete data during the sidestep manoeuvre. DESIGN: Descriptive cross-sectional study. METHODS: Sixteen male academy-level rugby union athletes (20.4±2.7yr; 186.3±9.1cm; 99.1±14.4kg) performed three maximal effort sidesteps (>6.0ms-1) on each leg. Three-dimensional motion analysis was used to obtain external knee abduction moments, wherein the two legs were separated by the preferred and n

Correlation Between Core Stability and Plantar Pressure Distribution During Double-Leg Stance, Single-Leg Stance, and Squat Positions in Healthy Male Athletes.

Background: Core stability is a cornerstone of optimum athletic performance, and its reduction is a risk factor for athletic injuries. Evidence has shown that core impairments can alter lower-limb mechanics through the kinetic chains. Additionally, plantar pressure can be influenced by proximal conditions, such as core muscle fatigue. Therefore, this study aimed to investigate the correlation between core endurance and plantar pressure distribution (PPD) during double-leg stance, single-leg stance, and single-leg squat positions in healthy male athletes. Methods: A total of 21 healthy male rec

Ankle flexibility and injury patterns in dancers.

Lower-extremity injuries are common among dancers and cause significant absences from rehearsals and performances. For this study of lower-e … orted prior lower-limb injuries, the most common being ankle sprains (28% of all dancers). Previous leg injuries correlated significantly with lower dorsiflexion measurements and with more new injuries. Female students had greater ankle and first metatarsophalangeal flexibility. Modern dancers had greater ankle inversion. Ninety-four students sustained 177 inj …

Stingray Injury Complicated by Vibrio alginolyticus Wound Infection and Posterior Tibial Artery Pseudoaneurysm in a Returning Traveler.

Stingray injury is common in temperate and tropical waters worldwide. The majority of injuries are minor and can be managed with simple first aid at the shoreline; however, serious complications can occur. We report a case of Vibrio alginolyticus wound infection, retained foreign body, and posterior tibial artery pseudoaneurysm after stingray envenomation in a returning traveler. The patient presented with severe pain and dysesthesia associated with a puncture wound in the left lower leg 1 week after injury in Seychelles. Clinician awareness of infection and trauma-related complications of sti

Muscular Calf Injuries in Runners.

Calf pain is a common complaint among runners of all ages but is most frequent in masters athletes. This article focuses on injuries to the triceps surae or true 'calf muscles.' The most common calf injury is a tear of the medial gastrocnemius muscle (Tennis Leg) but other structures including the lateral gastrocnemius, plantaris and soleus also may be the cause of muscular pain. This article looks at the presentation, evaluation, and treatment of these injuries. We also highlight some examples of musculoskeletal ultrasound which is a valuable tool for rapid diagnosis of the cause and extent o

Frostbite injury: a paragliding accident at 5500 meters.

Frostbite as a result of mountaineering or arctic expeditions is a well-known problem. In this article a rare case of frostbite injury following a paragliding accident at high altitude is described. A cumulonimbus cloud formation drifted the patient up with a high velocity to approximately 5500 m. He acquired frostbite injuries of the neck, wrists, hands and left leg, affecting 5% of the total body surface area. The frostbite was initially treated by rewarming, with later debridement and local application of silver sulfadiazine and povidone-iodine dressings. Within three months all the injurie

Acute compartment syndrome of the lower extremity secondary to noncontact injury.

We retrospectively reviewed the cases of three patients with injuries similar to the mechanism of an ankle sprain which resulted in compartment syndrome of the lower extremity. All three patients presented with sharp, proximal, anterolateral pain in the leg after an indirect, twisting injury. None of the injuries involved direct contact. Two of the three athletes initially presented to local hospitals, where the injury was misdiagnosed as an ankle sprain. The mechanism appears to be a muscle strain or tear with resultant hemorrhage in the compartment. All three patients returned to high-level

The effect of gender on force, muscle activity, and frontal plane knee alignment during maximum eccentric leg-press exercise.

PURPOSE: To investigate for gender differences during eccentric leg-press exercise. Tears of the anterior cruciate ligament (ACL) are considered to be related to eccentric tasks, altered neuromuscular control (e.g., reduced co-contraction of hamstrings), and increased knee abduction (valgus alignment). Based on these observations and the fact that ACL tears are more common in women, it was hypothesized that men and women differ significantly with regard to key parameters of force, knee stabilization, and muscle activity when exposed to maximum eccentric leg extension. METHODS: Thirteen women

Functional deficits may be explained by plantarflexor remodeling following Achilles tendon rupture repair: Preliminary findings.

Achilles tendon ruptures are common injuries that often lead to long-term functional deficits. Despite the prevalence of these injuries, the mechanism responsible for limited function has not yet been established. Therefore, the purpose of this study was to present preliminary findings that support a hypothesis that skeletal muscle remodeling is the driving factor of poor outcomes in some patients. Biomechanical and ultrasonography assessments were performed on a patient that presented with poor functional outcomes 2.5 years after a surgically-repaired acute Achilles tendon rupture. Single-leg

Injuries from spider spines, not spider bites.

Most injuries from spiders are due to their bites. Skin and eye injuries have been reported following contact with hairs of some theraphosid spiders (tarantulas). Some large spiders have relatively large spines on their legs and it was hypothesized that these may cause injury in some cases of suspected spider bite, and may account for "splinters" uncommonly seen at bite sites. Fifteen cases of suspected injury from spider leg spines were recruited prospectively over 3y. In all cases "splinters" were seen at the site. In II cases therewas a definite bite in addition to the spine injury. There w

Multiple sequential posttraumatic pseudoaneurysms following high-energy injuries: case report and review of the literature.

A 15-year-old boy sustained severe multisystem injuries from a satchel charge while in his native village in southern Lebanon. After evacuation and resuscitation measures, he had successful intensive multidisciplinary surgical care. His long-bone fractures were stabilized by tubular external fixation systems, and his lacerated right tibialis anterior artery was grafted. Twenty days after injury, he developed a pseudoaneurysm of the left distal ulnar artery, which was surgically resected and the ulnar artery proximally ligated. Two weeks later, a pseudoaneurysm of the left peroneal artery, dist

Suspected vascular trauma of the extremities: the role of arteriography in proximity injuries.

We reviewed 72 patients with penetrating trauma to the extremities who underwent arteriography for proximity injury only. None of the patients had clinical evidence of vascular trauma. There were 62 males and ten females, with a mean age of 29.9 years. Gunshot wounds were the most common cause of injury (91.7%) and the thigh was the most common site of injury (47.2%). A normal arteriogram was found in 55 of 72 patients (76.4%). The remaining 17 patients (23.6%) had arteriographic abnormalities that did not warrant surgery. Only one patient was explored (1.4%) for spasm of the popliteal artery.

Free microneurovascular muscle transfer in limbs to provide motor power.

The experience gained in a series of 7 free microneurovascular muscle transfers to the limbs over a seven-year period has led us to conclude that: free gracilis microneurovascular flaps can be safely transferred and can aid restoration of movement following severe injuries of the limbs, including post-replantation cases; inclusion of a cutaneous island with the muscle is safe, improves cover, and simplifies closure following muscle transfer; tendon adhesions and joint stiffness are the greatest obstacles to transferred muscle function, and early passive extension is recommended; additional pro

Evaluation of Rotation Reduction Features in Infant and Extended-Use Convertible Child Restraint Systems during Frontal and Rear Impacts.

A correctly used child restraint system (CRS) is associated with a substantial reduction of injury and mortality risks in motor vehicle crashes and epidemiologic data suggests that toddlers are provided greater protection when restrained in a rearward-facing CRS compared to a forward-facing CRS. Some 'extended-use' European CRS models can accommodate children up to six years rearward-facing and have a support (load) leg and/or a pair of lower (Swedish) tethers to reduce rotation during frontal and rear impacts, respectively. Laboratory studies have found that a support leg reduces head and nec

Lateral extension of the free scapular flap.

The scope of the scapular free flap is increased by the development of a further technical variation. The standard scapular flap is lengthened by use of a lateral (axillary) extension, without compromise of shoulder movement. The technique was used in 23 unipedicled and 1 bipedicled free scapular flap transfers in adults and children. An average increase in flap length of 30% was achieved. Lateral extension of the flap facilitates harvesting of the flap and closure of the donor defect. It is particularly useful in cases where shortage of local tissues in flap recipient areas prevents primary c
