Intraocular Pressure after Phacoemulsification with Retained Lens Fragments in the IRIS® Registry (Intelligent Research in Sight).

PURPOSE: To describe intraocular pressure (IOP) changes after cataract surgery resulting in retained lens fragments (RLFs). DESIGN: Retrospective clinical cohort study. PARTICIPANTS: Eyes in the IRIS® Registry (Intelligent Research in Sight) that underwent stand-alone phacoemulsification resulting in RLFs. METHODS: Daily mean IOP was calculated from postoperative days 1 through 90. An IOP spike was defined as a daily mean IOP > 30 mmHg and > 10 mmHg above baseline IOP within the first postoperative week. A linear mixed model determined when the postoperative daily mean IOP stabilized, and a

Phacoemulsification with perfluorocarbon liquid using a 23-gauge transconjunctival sutureless vitrectomy for the management of dislocated crystalline lenses.

BACKGROUND: We describe the modified technique of phacoemulsification in dislocated crystalline lenses. METHODS: After the vitreous and posterior hyaloid membrane were removed using a 23-gauge three-port vitrectomy, perfluorocarbon liquid (PFCL) was injected at the posterior pole in order to fill the vitreous cavity. The dislocated lens floated on the PFCL, and the injection was ceased once the lens had risen to the iris plane. The lens was then removed from the anterior chamber using standard phacoemulsification procedures. During the phacoemulsification, the PFCL provides a support undernea

[Conjunctival cyst: subconjunctival luxation of the lens following ocular contusion - a case report].

PATIENT: Presentation of a 75-year-old female with subconjunctival lens luxation, that was initially misdiagnosed as conjunctival cyst. Initial examination had been difficult due to vascularized stromal corneal scars and a large hyphema. The ocular history revealed an ocular contusion 11 months ago. Aphakia with subconjunctival lens luxation, iris incarceration, iridodialysis and covered scleral rupture were diagnosed by inspection, palpation, slit-lamp biomicroscopy, gonioscopy and ultrasound biomicroscopy. The lens and incarcerated iris tissue were removed, the scleral wound was sutured and

Weill-Marchesani syndrome in three generations.

BACKGROUND: Weill-Marchesani syndrome is a rare systemic connective tissue disorder consisting of brachymorphy, brachydactyly, ectopia lentis, spherophakia and glaucoma. METHODS: We report 6 patients with Weill-Marchesani syndrome (with or without ocular involvement) in three generations, identified by screening 26 members of two families. This is the largest family in the literature showing an autosomal dominant pattern of inheritance. RESULTS: Presenile vitreous liquefaction was present in all the younger cases. Weill-Marchesani syndrome was full-blown in two cases in the third generation,

Marfan syndrome diagnosed in patients 32 years of age or older.

The Marfan syndrome, a generalized inherited disorder, is usually diagnosed in young patients and is associated with a poor prognosis. With use of our diagnostic-retrieval system, we identified 28 patients with the Marfan syndrome who were 32 years of age or older at the time of diagnosis. These patients had at least two of four major diagnostic criteria for the Marfan syndrome--a confirmed family history, a long-limbed habitus, dislocated lenses, and disease of the aortic root. In these relatively older patients, some clinical findings (the ocular disorder and the family history) corresponded

Long-term follow-up after scleral lens fixation in patients with Marfan syndrome.

BACKGROUND: The dislocation of the crystalline lens is a common finding in patients with Marfan syndrome (MFS). Scleral intraocular lens (IOL) fixation is an accepted treatment method of this complication. To now, no long-term data on scleral IOL fixation in MFS exist. METHODS: We present a retrospective study of 27 eyes of 17 MFS patients that underwent scleral lens fixation at our clinic between 1999 and 2012. These patients are compared to an age- and surgeon-matched group of 31 eyes of 27 patients who underwent the same procedure for reasons other than MFS. RESULTS: The median age in the

Aniridic scleral fixation of intraocular lens: The answer to vision loss with photophobia!

BACKGROUND: Aniridia is defined as missing iris tissue which can be partial, subtotal, or total. Characteristic clinical symptoms include photophobia and decreased visual acuity due to an increased light perception. In addition to this, disturbing cosmetic problems are prevalent. Even after implantation of an intraocular lens, patients often tend to be unsatisfied. PURPOSE: The answer to this problem lies in the implantation of an aniridic scleral fixation of intraocular lens (SFIOL), which has a central optical axis that acts as the refractive lens and a peripheral rim of a hyperpigmented ar

A comparison of sutureless flanged fixation and 4-point Gore-Tex fixation for scleral-fixated intraocular lenses: a pilot study.

PURPOSE: Scleral-fixation of intraocular lenses (IOLs) provides an option for eyes that lack sufficient capsular support for in-the-bag IOL placement. The latest techniques for lens fixation include use of a novel suture material, Gore-Tex, and a sutureless method, with flanged intrascleral fixation. The purpose of this pilot study was to compare these methods in terms of anatomic and clinical outcomes. METHODS: A total of 35 eyes of patients 18-60 years of age who presented with aphakia, subluxated lens, or ectopia lentis were randomized into two groups. Group A (15 eyes) underwent flanged i
