Plasma Mycobacterium tuberculosis cell-free DNA assay: a diagnostic tool for tuberculosis lymphadenitis.

BACKGROUND: Bacterial confirmation in suspected tuberculosis lymphadenitis patients is challenging. This study evaluates plasma Mycobacterium tuberculosis cell-free DNA as a diagnostic tool for tuberculosis lymphadenitis. METHODS: A quantitative PCR assay targeting IS6110, IS1081, and cyp141 genes was performed on plasma samples. The study included 95 tuberculosis lymphadenitis patients and 60 controls. Sensitivity of the plasma Mycobacterium tuberculosis cell-free DNA assay was assessed against fine needle aspiration GeneXpert Ultra, fine needle aspiration culture, and fine needle aspiration…

Two distinct clinical progressions of P67phox-deficient CGD, both commencing with cervical lymphadenitis.

We present two rare cases of p67phox-deficient chronic granulomatous disease (CGD) caused by compound heterozygous mutations in the NCF2 gene. They developed cervical lymphadenitis as the initial manifestation of CGD but had distinct clinical progressions. Patient 1 presented with aspergillous meningitis, an extremely rare manifestation of neurological involvement in CGD, which has not been reported before. Patient 2 presented with non-infectious inflammatory lymphadenitis is also very rare and has not been reported previously. These cases emphasize the importance of considering p67phox-defici…

Strongyloidiasis stercoralis coinfection is associated with altered iron status biomarkers in tuberculous lymphadenitis.

Soil-transmitted helminth [mainly Strongyloidiasis stercoralis (Ss)] and tuberculous lymphadenitis (TBL) coinfection in humans is a significant public health problem. We have previously shown that TBL+Ss+ coinfection significantly alters diverse cytokine, matrix metalloproteinase, and tissue inhibitors of metalloproteinase profiles. However, no data is available to understand the influence of Ss coinfection in TBL disease with respect to iron status biomarkers. Hence, we have studied the effect of Ss coinfection on the circulating levels of iron status (ferritin, transferrin [TF], apotransferr…

Extensive cervical lymphadenitis mimicking bacterial adenitis as the first presentation of Kawasaki disease.

Cervical adenitis >1.5cm in diameter is the less frequently observed criteria in patients with Kawasaki disease and it is usually found in association with other symptoms during the acute phase. Moreover, the finding of fever and lymphadenitis with intense local signs of inflammation and phlegmon is rarely seen as the initial manifestation of Kawasaki disease. We report the case of a 7-year-old boy who had cervical lymphadenitis with adjacent cellulitis and phlegmon mimicking bacterial adenitis as the first presentation of Kawasaki disease. The patient had fever, cervical lymphadenitis with ad…

Mycobacterial cervicofacial lymphadenitis in human immunodeficiency virus-infected individuals after antiretroviral therapy initiation.

OBJECTIVES/HYPOTHESIS: Mycobacterial infections are the leading cause of morbidity and mortality among human immunodeficiency virus (HIV)-infected individuals worldwide. Cervical lymph nodes are the most frequently affected extrapulmonary sites. Despite the substantial reduction in complications of HIV-tuberculosis coinfection, a proportion of individuals develop immune reconstitution inflammatory syndrome (IRIS), a term used for a clinical deterioration following initiation of antiretroviral therapy (ART). The objective of this study was to describe mycobacterial-associated IRIS in cervical l

Non-tuberculous mycobacterial cervical lymphadenitis in the immunocompetent child: diagnostic and treatment approach.

Non-tuberculous mycobacterial disease in the otherwise well child commonly presents as a localized, slow-progressing, cervicofacial lymphadenopathy. Despite recent advances, several aspects of the susceptibility to and the course of the disease remain to be better understood, and diagnosis and management of non-tuberculous mycobacterial lymphadenitis in children are often controversial. Differential diagnosis should include other infections, in particular tuberculosis, local cysts and malignancies. In the majority of cases, untreated disease progresses to spontaneous drainage, fistula formatio…

Diagnostic challenge in papillary thyroid carcinoma with cervical lymphadenopathy, metastasis, or tuberculous lymphadenitis.

Papillary thyroid carcinoma (PTC) is the most frequent histological subtype of thyroid cancer. Total thyroidectomy with neck dissection is recommended for treatment. Tuberculous adenitis is a common cause of lymphadenopathy in endemic areas. Therefore, tuberculous lymphadenitis should be considered in the etiology of enlarged lymph nodes when PTC patients with risk factors such as tuberculosis present with cervical lymph node enlargement. Detailed evaluation of the neck metastasis of patients with PTC is necessary to avoid postoperative complications due to neck dissection. We present a 55-yea…

Cervical Lymphadenitis Caused by Group D Non-typhoidal Salmonella Associated with Concomitant Lymphoma.

Non-typhoidal Salmonella species are important foodborne pathogens that can cause gastroenteritis, bacteremia, and subsequent focal infections. Non-typhoidal salmonellosis is problematic, particularly in immunocompromised hosts. Any anatomical site can be affected by this pathogen via hematogenous seeding and may develop local infections. However, cervical lymphadenitis caused by non-typhoidal Salmonella species is rarely reported. Herein, we have reported a case of cervical lymphadenitis caused by group D non-typhoidal Salmonella associated with lymphoma.

Cryptococcal lymphadenitis diagnosed by fine needle aspiration cytology: a review of 15 cases.

OBJECTIVE: To review the cases of cryptococcal infection presenting primarily with lymphadenopathy and diagnosed on fine needle aspiration cytology (FNAC) and to evaluate the various cellular reactions in this infection. STUDY DESIGN: Retrospective review of 15 cases of cryptococcal lymphadenitis diagnosed on FNAC between 1999 and 2007. RESULTS: A total of 15 cases of cryptococcal lymphadenitis were diagnosed, out of which 10 occurred in a setting of immunosuppression, and in 5 cases there was no obvious immunocompromised clinical setting. Eight patients had human immunodeficiency virus infe…

Mycobacterial culture of fine needle aspirate - a useful tool in diagnosing tuberculous lymphadenitis.

A prospective study was undertaken on suspected lymph node TB (LNTB) patients, to evaluate the diagnostic utility of mycobacterial culture of fine needle aspirate (FNA), in comparison with the cytological examination and acid fast staining. Eighty percent of 157 aspirates studied were positive by cytological examination; 18% by ZN smear and 45% were positive by culture. Twelve aspirates which were negative by cytological features yielded positive mycobacterial cultures; four out of these were from HIV positive patients. Our observations suggest that supplementing FNA cytology with mycobacteria

Surgical management of BCG vaccine-induced regional axillary lymphadenitis in HIV-infected children.

UNLABELLED: There are as yet no clear surgical guidelines for the management of BCG vaccine-induced regional axillary lymphadenopathy. OBJE … nfirms a high perioperative complication rate associated with the primary surgical treatment of BCG lymphadenitis in both HIV-positive and negative patients. Primary surgical treatment (incisional drainage or biopsy) is therefore not considered an ideal form of management in BCG lymphadenitis because of the high fistulisation and poor wound healing, especially …

Kikuchi's disease (necrotizing lymphadenitis) with cutaneous involvement associated with subacute cutaneous lupus erythematosus.

Necrotizing histiocytic lymphadenopathy (Kikuchi's disease) is a rarely observed clinical entity characterized by fever, and solitary or multiple lymphadenopathy predominantly in the posterior cervical region. Kikuchi's disease has been reported to precede, coexist with or follow the diagnosis of systemic lupus erythematosus. In only rare instances has its association with cutaneous lupus erythematosus without systemic involvement been reported. We report a 45-year-old woman who presented characteristic systemic and cutaneous manifestations of Kikuchi's disease. Several months later, after sun

Lymphadenitis and lymphoproliferative lesions associated with the human herpes virus-6 (HHV-6).

A newly described herpes virus, human herpes virus 6, (HHV-6), has been linked to exanthema subitum but beyond this its pathogenetic impact remains to be determined. A large body of evidence links it to various lymphoproliferative disorders and this study was conducted to identify forms of lymphoproliferation linked to HHV-6. We studied biopsy samples from 32 patients with disorders of the lymphatic system for the presence of HHV-6, both by polymerase chain reaction (PCR) and in-situ hybridization (ISH) methods, as well as Epstein-Barr virus (EBV) viral DNA, clonal rearrangements of the antige

Two cases of acquired toxoplasmic lymphadenitis. Light and electron microscopic and immunohistochemical studies.

We report two cases of acquired toxoplasmic lymphadenitis, one with toxoplasmic cysts and the organisms of Toxoplasma gondii and the other with the organisms only. These cysts and organisms were observed in paraffin-embedded sections, touch smears and ultrathin sections for electron microscopy. Touch smears were especially valuable for the quick and accurate diagnosis of toxoplasmic lymphadenitis. We also studied immature sinus histiocytosis (ISH) in these cases. The predominant cells of ISH were confirmed to be B lymphocytes immunohistochemically, the majority being positive for polyclonal su…

A rare case of upper gastrointestinal hemorrhage due to tuberculous lymphadenitis fistulized in the duodenum.

INTRODUCTION: Abdominal tuberculosis is less prevalent than pulmonary tuberculosis. Tuberculous lymphadenitis accounts for fewer than 5% of abdominal cases. An unusual complication is the fistulization of a tuberculous lymph node into the digestive tract, with an even rarer association with upper gastrointestinal bleeding. CASE REPORT: A 63-year-old male with a history of rectal adenocarcinoma surgery presented with severe gastrointestinal bleeding that required multiple transfusions. Endoscopic attempts failed to locate the bleeding source, but CT angiogram revealed a hemorrhagic necrotic me…

Unmasking tuberculous lymphadenitis in a middle-aged male with Hashimoto's thyroiditis: a case report.

BACKGROUND: Tuberculous lymphadenitis (TBL), a form of extrapulmonary tuberculosis (TB), poses significant diagnostic challenges in primary care, especially when presenting atypically or in patients with underlying autoimmune disorders. Hashimoto's thyroiditis is an autoimmune condition characterised by hypothyroidism and may predispose individuals to opportunistic infections, including TB, due to immune dysregulation. The interplay between autoimmune diseases and TB is complex and not widely understood, particularly in non-endemic regions. This case highlights critical diagnostic consideratio…
