Urine antigen-negative disseminated histoplasmosis mimicking post-transplant lymphoproliferative disorder.

A 50-year-old woman with a history of kidney transplant presented with 2 days of abdominal pain after 6 months of recurrent streptococcal pharyngitis, fevers, weight loss and a new rash on her chest and back. Her examination was notable for a unilateral tonsillar exudate and 2-3 mm pink papules with a fine scale over her chest and back. CT of the abdomen and chest demonstrated several large lymph nodes, and laboratory investigation revealed new cytopenias and elevated transaminases. Urine antigen testing for Histoplasma capsulatum was negative, but a fungal complement fixation panel was reacti

Treatment and outcomes in classic Hodgkin lymphoma post-transplant lymphoproliferative disorder in children.

Classic Hodgkin lymphoma post-transplant lymphoproliferative disorder (HL-PTLD) has been rarely reported in children, with limited data available to guide treatment decisions. We report a retrospective review of five children diagnosed with classic HL-PTLD following solid organ transplant between 2007 and 2013 at Stanford University. Patients were treated with Stanford V chemotherapy and involved field radiation therapy. With a median follow-up of 7.2 years (range, 4.7-10.5 years) since diagnosis, all patients remain in remission from HL-PTLD and free from graft failure. In this series, combin

Mycosis fungoides - A cutaneous lymphoproliferative disorder in a patient treated with fingolimod for multiple sclerosis.

Fingolimod was the first oral disease-modifying drug approved for the treatment of relapsing-remitting multiple sclerosis (MS). It has previously been associated with rare cases of lymphoma. Here we describe the first case of mycosis fungoides - a cutaneous lymphoproliferative disorder, in an MS patient treated with fingolimod. who developed histologically confirmed mycosis fungoides 3 years after starting fingolimod. The drug was withdrawn and the patient was treated with radiotherapy and surgical excision with remission. This report points to a possible association between fingolimod and ski

Lymphocytoma cutis on the inguinal region: report of a rare case of benign lymphoproliferative disorder.

Lymphocytoma cutis, or benign reactive lymphoid hyperplasia, is an inflammatory skin lesion that mimics clinically and histologically malignant lymphoma. Most cases are idiopathic, but they may also be triggered by multiple factors, such as insect bites, tattoos, injections and herpes zoster. Clinically, the lesions are erythematous, soft papules, plaques or nodules, usually located on the upper limbs and face. The diagnosis is mainly based on histopathology and immunohistochemistry. Corticosteroid injections, cryosurgery, PUVA therapy, radiotherapy and surgery can be therapeutic options in ca

Post-transplant lymphoproliferative disorder--case reports of three children with kidney transplant.

INTRODUCTION: Post-transplant lymphoproliferative disorder (PTLD) is a heterogeneous group of diseases, characterized by abnormal lymphoid proliferation following transplantation. It is a disease of the immunosuppressed state, and its occurrence is mostly associated with the use of T-cell depleting agents, and also intensification of immunosuppressive regimens. In the majority of cases, PTLD is a consequence of Epstein-Barr virus (EBV) infection and is a B-cell hyperplasia with CD-20 positive lymphocytes. The 2008 World Health Organization classification for lymphoid malignancies divides PTLD

Association of immunosuppressive maintenance regimens with posttransplant lymphoproliferative disorder in kidney transplant recipients.

BACKGROUND: The association of immunosuppressive regimens (ISRs) with posttransplant lymphoproliferative disorder (PTLD) may be related with the Epstein-Barr virus (EBV) recipient serostatus. METHODS: We selected primary kidney transplant recipients from Organ Procurement Transplant Network/United Network for Organ Sharing database (2000-2009) who were discharged with a functioning graft and were receiving an ISR including an antiproliferative drug and a calcineurin inhibitor as follows: mycophenolate mofetil (MMF)/mycophenolate sodium+tacrolimus (TAC), MMF+cyclosporine A (CsA); mammalian tar

Posttransplant lymphoproliferative disorder presenting as multiple cystic lesions in a renal transplant recipient.

We report a case of a 67-year-old man who experienced allograft dysfunction following a renal transplantation from a donation after cardiac death. The postoperative course was initially complicated by episodes of E. coli urinary sepsis causing pyrexia and a raised creatinine level. Ultrasound scanning 5 weeks posttransplant revealed mild hydronephrosis with several parenchymal cystic areas measuring up to 2 cm with appearances suggestive of fungal balls. Aspirated fluid again grew Escherichia coli, and this was treated with the appropriate antimicrobial therapy. The patient continued to have e

Persistent complement-dependent anti-AnWj in a lymphoproliferative disorder: a case study and review.

AnWj is a high-incidence antigen present on the red blood cells (RBCs) of greater than 99 percent of the general population. A 58-year-old man underwent autologous hematopoietic stem cell transplantation (HSCT) for stage IVa mantle cell lymphoma. This procedure was complicated by failure to engraft, necessitating ongoing support with blood components. After a 2-month period of uneventful transfusion support, the patient experienced increasingly severe reactions with fever and evidence of intravascular hemolysis, including hemoglobinuria. Testing revealed a complement-dependent anti-AnWj. Pheno

The autoimmune lymphoproliferative syndrome: A rare disorder providing clues about normal tolerance.

The autoimmune lymphoproliferative syndrome (ALPS) is characterized by chronic, non-malignant lymphoproliferation, autoimmunity often manifesting as multilineage cytopenias, and an increased risk of lymphoma. While considered a rare disease, there are currently over 250 patients with ALPS being followed at the National Institutes of Health in Bethesda, Maryland. Most of these patients have a mutation in the gene for the TNF receptor-family member Fas (CD 95, Apo-1), and about one-third have an unknown defect or mutations affecting function of other signaling proteins involved in the apoptotic

Cutaneous presentation of plasmablastic post-transplant lymphoproliferative disorder in a 14-month-old.

We present a 14-month-old female child who developed multiple erythematous nodules on her abdomen 5 months after liver and small bowel transplantation. Skin biopsy revealed a dense infiltrate of large cells in the dermal and subcutaneous layers with frequent mitotic figures. The cells were noted to have abundant cytoplasm, prominent nucleoli, and open chromatin. Immunohistochemical stains were positive for CD138, CD56, Ki67 (>90%), and lambda chain restriction. Rare mature B cells (CD20) and rare T cells (CD3) were noted. She was diagnosed with high-grade post-transplant lymphoproliferative di

[Methotrexate-associated lymphoproliferative disorder presenting as oral ulcers in a patient with rheumatoid arthritis].

Methotrexate-associated lymphoproliferative disorders are a heterogeneous group of lymphoid proliferations or lymphomas that develop in patients with autoimmune diseases treated using methotrexate. These lymphoproliferative disorders are often associated with Epstein-Barr virus infection and occasionally regress after the withdrawal of methotrexate therapy. The lymphoproliferative disorder in this case was diffuse large B-cell lymphoma, unusually presenting as oral ulcers in a 79-year-old woman on treatment with methotrexate for longstanding rheumatoid arthritis. Latent membrane protein 1 posi…

Diagnosis and treatment of post-transplantation lymphoproliferative disorder in pediatric heart transplant patients.

PTLD is a severe complication in transplant recipients. Detection of increased EBV load in the peripheral blood acts as a surrogate marker for increased risk of PTLD development. We analyzed the time course of the disease, its severity, the organs involved, and mortality rates in our institutional experience of pediatric heart transplantation. This paper identifies risk factors for PTLD and describes the different ways of diagnosing and treating the disease. PTLD was screened for in 146 pediatric heart transplant patients using a retrospective analysis in patients who received transplantation

Native kidney post-transplant lymphoproliferative disorder in a non-renal transplant patient.

PTLD is an important post-transplant complication. Although PTLD affects kidney allografts after renal transplantation, it has not been reported in native kidneys of other solid organ recipients. Herein, we report a child who underwent an orthotropic liver transplant for cryptogenic cholestatic hepatitis and developed fever, generalized lymphadenopathy, chronic EBV viremia, and lymphatic PTLD. Subsequently, she also developed gross hematuria and nephrotic range proteinuria. Kidney histology revealed EBV-positive mononuclear infiltrates within the renal parenchyma consistent with PTLD. Severe autoimmune hemolytic anemia following rituximab therapy in a patient with a lymphoproliferative disorder.

Rituximab (chimeric anti-CD20 IgG1 monoclonal antibody) is effective in the treatment of relapsed/refractory low-grade lymphomas of B-cell origin as well as in diffuse large B-cell lymphoma. Several reports also demonstrated the efficacy of rituximab for the treatment of autoimmune cytopenia, especially for cold agglutinin disease. We report the first case, to our knowledge, of rituximab-related autoimmune hemolytic anemia. The pathophysiological mechanisms remain unknown, although the drug could act through massive cytokines liberation after destruction of CD20 positive cells by rituximab.

Use of radiation therapy in posttransplant lymphoproliferative disorder (PTLD) after liver transplantation.

Posttransplant lymphoproliferative disorder (PTLD) is a common and life-threatening complication of immunosuppression used to prevent rejection of solid organ and bone marrow transplants. There is no standardized treatment algorithm, but numerous management strategies are available. We describe a patient who developed a solitary lymphoproliferative lesion in the porta hepatis 9 months after orthotopic liver transplant. Following reduction in immunosuppression with no response, she was treated with involved field radiotherapy utilizing CT-based treatment planning. A partial radiographic respons

Methotrexate-Associated Lymphoproliferative Disorder Presenting As Disseminated Metastatic Malignancy: A Case Report.

Methotrexate-associated lymphoproliferative disorder (MTX-LPD) is a rare but relevant complication of long-term methotrexate (MTX) therapy that can closely mimic infection or disseminated malignancy. We report the case of a 65-year-old woman with a 10-year history of systemic lupus erythematosus (SLE) treated with MTX, who presented with progressive back pain, constitutional symptoms, pancytopenia, and extensive multiorgan disease. Fluorodeoxyglucose positron emission tomography (FDG PET) demonstrated widespread hypermetabolic lesions involving the lungs, liver, left adrenal gland, and skeleto

Hydroa Vacciniforme-Like Lymphoproliferative Disorder in the Elderly: A Case Report and Diagnostic Challenges.

BACKGROUND: Hydroa vacciniforme-like lymphoproliferative disorder is a rare Epstein-Barr virus -associated T/NK cell proliferation predominantly. CASE PRESENTATION: A 72-year-old female presented with persistent skin lesions and high fever. The atypical distribution of lesions, along with the spontaneous regression of some of them, and the positive Staphylococcus aureus culture, initially made the diagnostic process complicated. However, skin biopsies revealed CD3+CD8+ T-cell infiltration, partial loss of CD5 expression, a high proliferative index, and positive Epstein-Barr virus-encoded RNA

Posttransplant lymphoproliferative disorder in vascularized composite allotransplantation: a systematic review with institutional cases.

BACKGROUND: Posttransplant lymphoproliferative disorder (PTLD) is a serious complication, yet its presentation and consequences after vascularized composite allotransplantation (VCA) remain poorly characterized. Disease control must be balanced against graft preservation, rejection risk, and allograft loss. METHODS: A systematic review was performed in accordance with PRISMA (Preferred Reporting Items for Systematic reviews and Meta-Analyses) 2020 across PubMed, Scopus, and Embase to identify PTLD after skin-bearing human VCA. Extracted variables included recipient and graft characteristics,

Case Report: Post-transplant lymphoproliferative disorder mimicking Baló's concentric sclerosis.

Post-Transplant Lymphoproliferative Disorder (PTLD) is a lymphoproliferative disease that occurs in recipients of solid organ or hematopoietic stem cell transplants, resulting from the malignant transformation of benign proliferative lymphoid or plasma cell tissue due to immunosuppression. As an immunodeficiency-associated lymphoproliferative disorder, PTLD can affect various organs, including lymph nodes, the gastrointestinal tract, lungs, kidneys, and the central nervous system. The overall prognosis is generally poor. This case report describes a 37-year-old female patient presenting with m
