INTRODUCTION: Awake fiberoptic intubation is a critical skill of emergency physicians in scenarios where rapid sequence intubation may be impossible or catastrophic. While the equipment to perform awake fiberoptic intubation has become more readily available to emergency physicians, inadequate training and lack of confidence are often cited as barriers to performing the procedure. To address this, we created and studied a simulation-based awake fiberoptic intubation curriculum with the goal of improving physicians' performance of this skill.
METHODS: A procedural checklist was developed and i

BACKGROUND: Managing difficult airways in prehospital settings is particularly challenging, especially in rural or resource-limited environments and during patient transport. Inexperienced providers performing endotracheal intubation (ETI) during ambulance transport may have lower success rates, highlighting the potential value of real-time expert support.
OBJECTIVES: This study aimed to evaluate the effectiveness of real-time teleguidance from a remote airway expert in improving ETI success among inexperienced operators using videolaryngoscopy (VL) in a moving ambulance environment.

BACKGROUND: Prolonged neonatal cardiopulmonary resuscitation (CPR) is critical for survival in cases of severe bradycardia due to asphyxia. High-performance CPR, emphasizing consistent compression depth and full recoil, improves survival rates.
PURPOSE: This pilot study explored how environmental and provider-specific factors-including bed height, positioning, and anthropometry-affect the consistency of neonatal CPR using the 2-thumb technique.
METHODS: Twenty-two neonatal intensive care unit providers performed 2-minute CPR trials on a neonatal simulator under 2 conditions: self-selected be

Direct in-water rescue is a safety-critical task in which towing technique and rescue-support equipment may influence both rescuer workload and task output. This study compared physical workload, towing performance and physiological economy across four mannequin towing techniques: Extended Arm Tow, Double Armpit Tow, "Sailor" Technique Tow and Rescue Tube Tow. Eighteen male water lifeguards (age 18.8 ± 1.3 years; V˙O2max 56.1 ± 5.0 mL·kg-1·min-1) completed randomized 50-m towing trials after an incremental swimming test to determine maximal oxygen uptake. Towing velocity, gas-exchange variable

Structural firefighters are exposed to an array of polycyclic aromatic hydrocarbons (PAHs) as a result of incomplete combustion of both synthetic and natural materials. PAHs are found in both the particulate and vapor phases in the firefighting environment and are significantly associated with acute and chronic diseases, including cancer. Using a fireground exposure simulator (FES) and standing mannequins dressed in four different firefighter personal protective equipment (PPE) conditions, each with varying levels of protective hood interface and particulate-blocking features, the efficacy of

A finite element (FE) model of an 11-month-old child's femur was developed to evaluate fracture risk in short-distance feet-first falls and bed falls. Pediatric material properties were applied to the FE model. Femur loading was derived from previously conducted fall experiments using a child surrogate where fall conditions (e.g., fall height, impact surface) were varied. Fracture thresholds based on principal stress and strain were used to examine potential for fracture. Peak stress/strain were significantly greater for feet-first falls from greater heights and onto harder impact surfaces. Fe

An investigation into the arm and body position required to obtain the blood pattern visible in the image of the Shroud of Turin was performed using a living volunteer. The two short rivulets on the back of the left hand of the Shroud are only consistent with a standing subject with arms at a ca 45° angle. This angle is different from that necessary for the forearm stains, which require nearly vertical arms for a standing subject. The BPA of blood visible on the frontal side of the chest (the lance wound) shows that the Shroud represents the bleeding in a realistic manner for a standing positi

The Stop the Bleed initiative empowers and trains citizens as immediate responders, to recognize and control severe hemorrhage. We sought to determine the retention of short-term knowledge and ability to apply a Combat Application Tourniquet (CAT) in 10 nonmedical personnel. A standard "Stop the Bleed" (Bleeding Control) course was taught including CAT application. Posttraining performance was assessed at 30 days using a standardized mannequin with a traumatic below-knee amputation. Technique, time, pitfalls, and feedback were all recorded. No participant had placed a CAT before the initial cl

We present a functional and patient-specific lumbar phantom for the training of spinal durotomy and dura closure under microscopic view, consisting of a lumbar model, pressurized dural surrogate, together immersed in a tissue-mimicking layer simulating fat, muscle and skin. The lumbar model was derived from a patient computed tomography scan, preserving the natural shape and curvature of the lumbar column. The inclusion of the simulated soft-tissue layer was critical for preserving the surgical ergonomics and presented a realistic view under the surgical microscope. As the success of dura repa

OBJECTIVE: The purpose of this study was to obtain non-censored rib fracture data due to three-point belt loading during dynamic frontal post-mortem human surrogate (PMHS) sled tests. The PMHS responses were then compared to matched tests performed using the Hybrid-III 50(th) percentile male ATD.
METHODS: Matched dynamic frontal sled tests were performed on two male PMHSs, which were approximately 50(th) percentile height and weight, and the Hybrid-III 50(th) percentile male ATD. The sled pulse was designed to match the vehicle acceleration of a standard sedan during a FMVSS-208 40 kph test.

Frontal vehicle collisions can cause injury to a driver's cervical muscles resulting from intense changes in muscle strain and muscle load. This study investigated the influence of collision forces in a sled test environment using a modified Hybrid III 50th percentile dummy equipped with simulated spring-type muscles. Cervical muscle responses including strain and load of the sternocleidomastoid (SCM), splenius capitis (SPL), and trapezius (TRP) were analyzed, and muscle injury was assessed. The SCM, SPL, and TRP suffered average peak muscle strains of 21%, 40%, and 23%, respectively, exceedin

Auscultation torsos are widely used to teach position-dependent heart sounds and murmurs. To provide a more realistic teaching experience, both whole body auscultation mannequins and torsos have been used in clinical examination skills training at the Medical Faculty of the University of Muenster since the winter term of 2008-2009. This training has since been extended by simulated patients, which are normal, healthy subjects who have undergone attachment of the electronic components of the auscultation mannequins to their chests to mimic pathophysiological conditions ("hybrid models"). OBJECTIVE: The development of the adaptive occupant restraint system (AORS) has led to an innovative way to optimize such systems for multiple size occupants. An AORS consists of multiple units such as adaptive air bags, seat belts, etc. During a collision, as a supplemental protective device, air bags can provide constraint force and play a role in dissipating the crash energy of the occupants' head and thorax. This article presents an investigation into an adaptive passenger air bag (PAB).
METHODS: The purpose of this study is to develop a base shape of a PAB for different size occupants us

Silicone models of cerebral aneurysms are used for evaluation of devices, training, or hemodynamic studies. We report preoperative simulations of endovascular treatment for a case with an unruptured wide-neck aneurysm of the anterior communicating artery using a patient-specific silicone model. Using a rapid prototyping system, we created a silicone model based on the vascular image obtained by three-dimensional rotational angiogram. The aneurysm and vessels formed a cavity in the silicone block model. We performed endovascular simulations using several difference devices and attempted possibl
