Psychiatric diagnosis in cocaine abuse.

The Structured Clinical Interview for DSM-III, Axis I was administered to a consecutive series of 30 cocaine abusers entering outpatient treatment. Another mental disorder was diagnosed only if its onset occurred before the onset of any substance dependence disorder. Diagnoses were made on a lifetime basis without regard to hierarchy. Most cocaine abusers had one or more additional diagnoses. We propose a model of two subtypes of cocaine abusers: (1) a primary affective disorder group, which may be split into bipolar versus unipolar or into severely versus mildly depressed subgroups; and (2) a

Symptom development in childhood onset schizophrenia.

Symptom development from birth to 12 years of age was examined in 18 children who met DSM-III criteria for schizophrenia with onset before 10 years of age. Using a follow-back design, symptom development was rated at each of four age levels using a DSM-III Symptom Rating Scale and the Achenbach Child Behavior Checklist. Results revealed a gradual developmental unfolding of a broad spectrum of symptoms affecting social, cognitive, sensory and motor functioning and beginning many years before the appearance of schizophrenic symptoms--usually in early infancy. Prior to 6 years of age, severe lang

Neuroticism and personality disorder in depression.

Neuroticism and DSM-III personality disorder were studied in 39 depressed inpatients. Interrelationships between these variables and their relationship to depressive typology were compared. The relationship of neuroticism, DSM-III personality type and adequacy of personality to MAOI treatment are also examined. Neuroticism scores were unaffected by short-term treatment, and no differences in neuroticism were observed between melancholics and nonmelancholics, or between endogenous and nonendogenous depressives. Higher neuroticism scores were associated with DSM-III personality disorder. Persona

Practice parameter for the assessment and treatment of children and adolescents with eating disorders.

This Practice Parameter reviews evidence-based practices for the evaluation and treatment of eating disorders in children and adolescents. Where empirical support is limited, clinical consensus opinion is used to supplement systematic data review. The Parameter focuses on the phenomenology of eating disorders, comorbidity of eating disorders with other psychiatric and medical disorders, and treatment in children and adolescents. Because the database related to eating disorders in younger patients is limited, relevant literature drawn from adult studies is included in the discussion.

Randomized trial of brief motivational treatments for pathological gamblers: More is not necessarily better.

The efficacy of brief treatments for media-recruited pathological gamblers was tested in a randomized clinical trial design (N = 314). Two self-directed motivational interventions were compared with a 6-week waiting list control and a workbook only control. Brief motivational treatment involved a telephone motivational interview and a mailed self-help workbook. Brief motivational booster treatment involved a telephone motivational interview, a workbook, and 6 booster telephone calls over a 9-month period. Primary outcomes were gambling frequency and dollar losses. As hypothesized, brief and br

History of Peyronie's disease: from early descriptions to modern treatments.

INTRODUCTION: Peyronie's disease (PD) is a fibroproliferative disorder of the tunica albuginea characterized by localized plaque formation, penile curvature, and erectile dysfunction, most commonly affecting men between 40 and 70 years of age. Although François Gigot de La Peyronie is credited with the first formal description of the disease in 1743, depictions of penile curvature predate his description by millennia. OBJECTIVES: This review aims to provide a comprehensive understanding of the historical and contemporary evolution of therapeutic strategies for PD, highlighting the shift from

Characterizing pediatric blepharokeratoconjunctivitis in an underserved urban setting.

BACKGROUND: Pediatric blepharokeratoconjunctivitis (PBKC) is a chronic inflammatory disease of the eyelid margin with secondary conjunctival and corneal involvement. The purpose of this study was to characterize the clinical features, visual outcomes, and treatment of PBKC in a diverse, underserved metropolitan community in the United States. METHODS: A retrospective study was conducted of patients 16 years old or younger diagnosed with PBKC, who were identified using relevant Systematized Nomenclature of Medicine - Clinical Terminology (SNOMED CT) codes and manual chart review. RESULTS: A t

AI-driven quadruped robots: from fundamental locomotion to advanced biomimetic behaviors.

Quadruped robots have attracted increasing attention because they can traverse uneven terrain, support field deployment, and perform tasks that are difficult for wheeled or tracked platforms. Recent advances in artificial intelligence (AI) have further expanded their capabilities from manually designed gait control toward learning-based locomotion, perception-aware adaptation, dynamic motion skills, autonomous recovery, manipulation, energy-aware operation, fault diagnosis, and human-robot interaction. However, the literature on AI-driven quadruped robotics is distributed across diverse techni

Modern Therapies for Discoid Lupus Erythematosus: A Narrative Review.

BACKGROUND: Discoid lupus erythematosus (DLE) is the most common chronic form of cutaneous lupus erythematosus and can lead to scarring, dyspigmentation, and permanent alopecia. The disease often causes visible disfigurement and has a substantial psychosocial impact. Although many patients respond to standard treatment, a subset continues to experience persistent or relapsing disease that remains difficult to control. METHODS: A focused literature search was performed using PubMed, Google Scholar, Web of Science, and Scopus to identify English-language studies published from January 1990 thro

Treatment outcomes of cutaneous leishmaniasis due to Leishmania aethiopica: A systematic review and meta-analysis.

BACKGROUND: Leishmania aethiopica is a unique species that causes cutaneous leishmaniasis (CL), and studies evaluating treatment outcomes for this condition reported inconsistent findings. This study aimed to summarize the evidence on treatment outcomes of CL caused by L. aethiopica to support decisions or propose further study. METHODS: We searched PubMed, Scopus, and ScienceDirect. In addition, we searched grey literature on Google Scholar and performed manual searching on the reference list of articles. Two authors did the screening, selection, critical appraisal, and data extraction. With

INTRODUCTION: Shoulder pain is a common problem with an estimated prevalence of 4% to 26%. About 1% of adults aged over 45 years consult their GP with a new presentation of shoulder pain every year in the UK. The aetiology of shoulder pain is diverse and includes pathology originating from the neck, glenohumeral joint, acromioclavicular joint, rotator cuff, and other soft tissues around the shoulder girdle. The most common source of shoulder pain is the rotator cuff, accounting for over two-thirds of cases. METHODS AND OUTCOMES: We conducted a systematic review and aimed to answer the followi

[Interrater-reliability of the OPD-CA, the axes conflict and structure].

The reliability of the meanwhile widely used Operationalized Psychodynamic Diagnostics in childhood and adolescence (OPD-CA) is only rarely examined. By means of audiovisual recordings of OPD-CA-interviews with 39 adolescents in the context of a randomized-controlled psychotherapy study for the treatment female adolescents with bulimia nervosa and atypical bulimia nervosa the reliability of the axis conflict and the axis structure were examined. This was carried out by the calculation of Intraklassen-correlations of three raters. The rater agreements ranged from good to excellent, except for i

Preventing mania: a preliminary examination of the GOALS Program.

There is strong evidence of a relationship between goal dysregulation and mania. Building on these findings, we examined the feasibility of developing a mania prevention treatment program designed to improve goal regulation skills for those with bipolar disorder. Here, we describe the process of developing a manual, delivering the intervention to a series of cases, and then conducting a small open uncontrolled trial. All participants met diagnostic criteria for bipolar I disorder based on the Structured Clinical Interview for DSM-IV and were not currently experiencing episodes of depression or

The Phenomenology of Major Depression and the Representativeness and Nature of DSM Criteria.

How should DSM criteria relate to the disorders they are designed to assess? To address this question empirically, the author examines how well DSM-5 symptomatic criteria for major depression capture the descriptions of clinical depression in the post-Kraepelin Western psychiatric tradition as described in textbooks published between 1900 and 1960. Eighteen symptoms and signs of depression were described, 10 of which are covered by the DSM criteria for major depression or melancholia. For two symptoms (mood and cognitive content), DSM criteria are considerably narrower than those described in

Dealing with fear of blushing: a psychoeducational group intervention for fear of blushing.

UNLABELLED: The clinical impression is that people who fear blushing do not easily seek psychological help for their complaints. Therefore, we designed a low-threshold psychoeducational group intervention to reduce fear of blushing. The intervention followed a cognitive-behavioural approach, but in a course setting, e.g., with 'participants' and 'teachers' instead of 'patients' and 'therapists'. The effectiveness of the course in reducing fear of blushing and social anxiety was tested in a group of blushing-fearful individuals (n = 47) by using an uncontrolled study design. The course consiste
